1 JOB DESCRIPTION JOB TITLE: Registered Nurse Adults BAND: Band 5 HOURS: Full time/ part time/ Bank 1 DIRECTORATE INITIAL LOCATION: REPORTS TO: ACCOUNTABLE TO: This recruitment is for positions Trust Wide. (If appointed to the waiting list you will be invited to indicate which of the Trusts main geographical patches you are prepared to work in. When an offer is made you will be employed for a particular site and may be required to move wards / teams within that site / geographical patch) JOB SUMMARY Postholders will work within Adult nursing service within the Southern Health & Social Care Trust, providing person centred nursing care and support to patients, their families and carers. The postholder will be expected to work flexibly across acute sectors and community as required and as appropriate. Participate as part of a team and using a person centred approach, assess, plan, implement and evaluate care delivered. KEY DUTIES & RESPONSIBILITIES The following are Key Duties & Responsibilities of the postholder. PROFESSIONAL RESPONSIBILITIES 1. Comply with all policies, procedures, clinical protocols, standards and guidelines based on legislation, evidence based nursing practice and NMC guidelines for conduct and professional practice. 1 For those successful at interview details of the hours they are interested in working will be collated and as far as is reasonably practicable, taken account of when offers are being made. However applicants should be aware the Trust provides a 24/7 service and this requires a level of flexibility which may include evening, Night time and weekend work, as well as daytime work including early morning shifts.

2 2. To participate in the development of clinical and Social Care Governance in the context of his / her sphere of responsibility. 3. Provide quality care to adults in partnership with family and carers. 4. Report to and liaises with the nurse in charge / team leader who is responsible for supervision of workload. 5. Ensures accurate record keeping as per Trust policy and NMC Guidelines. 6. Adhere to the Trust Area Child Protection and Vulnerable Adult Policies and Guidelines. 7. To co-operate and work effectively with other professionals in the multi-disciplinary team and community services to ensure continuity of care. 8. Adhere to the Medicines Management Code and NMC Standards for the safe keeping and administration of medication. 9. Adhere to procedures and protocols on moving and handling in line with current legislation. 10. Contribute to the accuracy of statistical returns. 11. Be cost effective in the use of resources 12. Co-operate fully with the implementation of the Trusts Health & Safety arrangements, reporting accidents/incidents to the nurse in charge / team leader, and initiating a full investigation in a timely manner, whilst taking responsibility to complete appropriate documentation. 13. Monitors and evaluates standards of care and reports to nurse in charge / team leader. 14. Accept responsibility for own professional development. 15. Maintain an awareness of clinical and professional developments in nursing and participate in nursing research to enhance patient care. 16. To actively participate in the Trust KSF and Clinical Supervision. 17. Attend staff meetings, training sessions and courses as required. 18. Flexibility in working will be required to meet the needs of this post. CLINICAL RESPONSIBILITIES 21. Work as a member of the multidisciplinary team ensuring effective communication with other team members, manager / team leader to promote patient centred care.

3 22. Assess the health and nursing care needs of adults and older people within the individual care environment. To develop, implement and evaluate nursing care plans in partnership with the patient / client, family and carers, whilst working in close cooperation with other disciplines. 23. Promote self care and independence, where appropriate and prepare the patient / client for independent living whenever possible. 24. Assist in the training of qualified staff, nursing auxiliaries, health care assistants, and student nurses, ensuring compliance to the NMC standards to support learning and assessment in practice. 25. Provide support, guidance and supervision to junior staff as required. 26. Participate in induction and teaching programmes to students and members of staff to include supervision, preceptorship and training and those undertaking NVQ qualification. 27. Administer medications, treatments and interventions as prescribed and to monitor their effects. 28. Assess and prescribe nursing equipment within existing resources and advise patients / clients / carers on the use of same. Document any areas of unmet need to inform areas of future service delivery. 29. Maintain and develop professional expertise and knowledge in his / her specific field of nursing. GENERAL REQUIREMENTS The post holder will be required to: 30. All such records are public records and are accessible to the general public, with limited exception, under the Freedom of Information act 2000 the Environmental Information Regulations 2004 and the Data Protection Acts Employees are

4 required to be conversant with the Trusts policy and procedures on records management and to seek advice if in doubt. 34. Take responsibility for his/her own ongoing learning and development, including full participation in KSF Development Reviews/appraisals, in order to maximise his/her potential and continue to meet the demands of the post. 35. Represent the Trust s commitment to providing the highest possible standard of service to patients/clients and members of the public, by treating all those with whom he/she comes into contact in the course of work, in a pleasant, courteous and respectful manner. 36. Understand that this post may evolve over time, and that this Job Description will therefore be subject to review in the light of changing circumstances. Other duties of a similar nature and appropriate to the grade may be assigned from time to time. 37. Be willing to work flexibly to meet the needs of a 24/7 service, which may include evening, night time and weekend work with an expectation to rotate between different areas as required This Job Description will be subject to review in the light of changing circumstances and is not intended to be rigid and inflexible but should be regarded as providing guidelines within which the individual works. Other duties of a similar nature and appropriate to the grade may be assigned from time to time. It is a standard condition that all Trust staff may be required to serve at any location within the Trust's area, as needs of the service demand.

5 PERSONNEL SPECIFICATION JOB TITLE: DIRECTORATE SALARY Registered Nurse Adults 20,710-26,839 per annum pro rata Ref No: March 2010 Notes to applicants: 1. This specification is being used for the creation of a waiting list to posts within the Trust as they arise. Posts will be filled in accordance with the Trusts protocols for waiting list management. ESSENTIAL CRITERIA these are criteria all applicants MUST be able to demonstrate either at shortlisting or at interview. Applicants should therefore make it clear on their application form whether or not they meet these criteria. Failure to do so may result in you not being shortlisted. The stage in the process when the criteria will be measured is stated below; The following are essential criteria which will initially be measured at Shortlisting Stage although may also be further explored during the interview stage; QUALIFICATIONS / EXPERIENCE 1. Currently a Registered Nurse Level 1 Adult - on the Live NMC Register 2. Applications will also be accepted from those completing pre-registration training and due to register with the NMC in September KNOWLEDGE & SKILLS The following are essential criteria which will be measured during the interview stage. 2. Demonstrate a commitment to delivering the optimum standard of patient centred/client centred care 3.
