1 Title Location Macmillan Lung Cancer Clinical Nurse Specialist Hospital Supportive & Specialist Palliative Care Team (HSSPCT) Grade 7 Reports to Responsible to HSSPCT Nursing Team Leader HSSPCT Nursing Team Leader Role Summary: The postholder will work as an integral part of the Hospital Supportive & Specialist Palliative Care Team (HSSPCT) but with overall responsibility for the provision of specialist nursing advice and suppport for hospital based patients with lung cancer, throughtout the period of investigation, diagnosis, treatment and terminal care. As part of the multi-disciplinary team the post holder will participate in the provision of an integrated Lung Cancer specialist service providing clinical expertise, contributing to the decision s, influencing diagnosis, investigations, treatments, information and management of adults with Lung Cancer malignancies. He/she will assist in the co-ordination of the patient s journey from diagnosis to discharge. Timely onward referral to other professionals and agencies; follow up, palliative and terminal care. The post holder will demonstrate advance practice and leadership in relation to the provision of specialist nursing advice with regard to Lung cancer management and care. Support and facilitate high quality specialist care to patients, clients, relatives and carers which are compliant with the standards and measures as set in the Improving Outcomes Guidance. The post holder will work collaboratively with the multidisciplinary teams and other agencies in relevant healthcare locations to provide high quality specialist care, education, 1

2 advice and information to patients with complex lung cancer & palliative care needs. He/she will be actively involved in education, audit and research. The post holder will demonstrate clinical expertise and leadership in relation to the provision of specialist palliative care and facilitate the development of skills, knowledge and competencies of staff caring for patients with lung cancer needs. The post holder will supports healthcare teams to implement research evidence into practice, promotes and participates in nursing audit, contributing to a culture of critical inquiry. The post holder will contribute to the future development of the specialist palliative care service. Represent the Trust by participation on relevant committees, working parties and project teams at local, regional and National level. The post will contribute to the development of the policy agenda both within and outside the Trust using appropriate influencing skills to shape service delivery. Establish and maintain effective communication with various individuals and groups on highly complex potentially stressful topics in a range of situations. The post holder will make sure that people affected by cancer are aware that they are interacting with a Macmillan Professional and know about the full range of resources and services available through Macmillan. Specific Responsibilities: 1 Clinical Leadership 1.1 Provide professional and clinical leadership in all issues related to lung cancer to patients, carers, relatives, nurses and other professionals throughout the Trust. Develop good working relationships with relevant stakeholders within the hospital. Acts as a specialist resource in Lung Cancer care at any point along the pathway of the patient s illness appropriate to patient need. 1.2 Have relevant expertise and knowledge related to investigation, treatment options and intervention in order to provide patients with necessary information to maximise informed consent. 1.3 Acts as a Key Worker for a clinical caseload, working to coordinate care and provide easy access for on-going support for patients. Initiate on-going referrals for further assessment, diagnostic tests and to other professionals as appropriate for the group of patients. 1.4 Provide specialist advice and support for health care professionals empowering them to further develop their skills in lung cancer care. 1.5 Promote collaborative working relationship and effective communication between all members of the multi-professional teams co-ordinators, relevant 2

3 stakeholders within Belfast Health & Social Care Trust and other referring/receiving Trusts. 1.6 Establish and maintain open communication networks with multi-professionals teams within acute, community, statutory and voluntary through a variety of means including face to face, telephone contact, information technology and written correspondence. 1.7 In a consultative role facilitate and promote high quality care for patients with lung cancer and families. Demonstrate skills of leadership, supervision and clinical expertise for other professionals in relations to all aspects of lung cancer. 1.8 Participate in care management planning meetings and case conferences in relation to future management or discharge arrangements. Liaise with community services to ensure continuity of patient care. 1.9 Actively contribute to multi professional team meetings and discussions acting as the patients advocate when relevant. Present patients when required, discuss treatment choices and relay outcomes from the multi- professional team discussion to patients when appropriate Demonstrate high levels of autonomy and advocacy within a multiprofessional team. Manage a patient caseload identifying priorities and competing demands around highly complex and sensitive situations To be aware of emerging evidence and advances in clinical practice for patients with lung cancer, ensuring they are incorporated into the service delivered Promote excellence in clinical practice advising and supporting other members of the multi-professional team. Facilitate the use of best practice and evidence based care Make professionally autonomous decisions to develop an appropriate plan of care and in consultation with other healthcare colleagues advise and/or refer appropriately to other disciplines and services Undertake advanced holistic assessment to identify patients, emotional, psychological, social and spiritual needs utilising advanced nursing practice skills such as clinical assessment Use on a daily basis highly developed communication skills to explore sensitive, stressful and sometimes complex issues with patients, which also impact on families, carers and health care professionals Regularly manage potential and actual hostile and antagonistic situations caused by highly distressing circumstances e.g. breaking bad news, collusion, ethical issues, and anxious, angry, distressed patient/carers Develop effective communication strategies for patients who have barriers to communication including the need to use language interpreters Communicate effectively with patients, relatives and carers to assess information and support needs. Provide accurate up to date information 3

4 tailored to individual needs taking into account patient/carers level of understanding Signposting to other sources of information and support and referring onto specialist support when required e.g.: Lung Cancer Support Services, Specialist Palliative Care Unit and Support Services, Macmillan Information and Support Services, Specialist Benefits Advisors and BHSCT Trust Chaplains Provide specialist advice regarding patient care to relevant staff and make recommendations including management of pain, breathlessness and symptoms Utilise specialist knowledge and advice from team colleagues including consultants in palliative medicine, palliative care pharmacist regarding treatment options/choices for patient with complex palliative care needs Contribute to the development of local policies and protocols to support the delivery of care to patients with lung cancer To engage locally and regionally in service improvement programmes providing specialist advice through direct involvement in consultations and feedback process. As a clinical expert represent the Trust on relevant committee, working parties and project teams at local, regional and national level. 2 Education, Practice Development and Research 2.1 Demonstrates advanced knowledge, skills and innovative practice in specialist area of lung cancer. 2.2 Identifies and responds to the educational needs and requests from the multiprofessional staff within the hospital setting. 2.3 Contributes to the development, delivery and evaluation of lung cancer and palliative care education programmes for nurses and all other professionals groups as appropriate. 2.4 Participate as requested in the delivery of education programmes for local universities and other agencies at post registration level. 2.5 Act as an expert resource and facilitator by providing mentorship and support to professional colleagues undertaking post graduate lung cancer study. 2.6 Participate in the development and delivery of reflective practice and mentorship within the multi-professional team. 2.7 Takes responsibility for own personal and professional development by participating in an annual appraisal using the Belfast Health and Social Care Trust Personal Contribution Framework. Proactively assess own education and development needs and identify structured ways in which these may be met in individual Personal Development Plan. 4

5 2.8 Participate and initiate nursing audit or projects, disseminate and implement relevant research findings to reduce the theory practice gap in relation to evidence based lung cancer care. 2.9 Promote patient and public involvement to improve service provision, drive innovation and change by actively seeking views of service users and carers. 3 Management 3.1 Identifies the requirements of this specialist role and ensures appropriate provision and effective management of workload. Working autonomously to manage his/her own caseload of patient whilst working as part of the multiprofessional team. 3.2 Develop service standards and audit performance against these standards. 3.3 Collates and records quantitative and qualitative data to provide evidence of productively, outcomes and quality, through audit and research and contributes to the Supportive and Specialist Palliative Care Team Annual Report demonstrating the evidence. 3.4 Sets realistic objectives and works towards achieving them within an agreed time scale. 3.5 Ensures accurate documentation in patients clinical notes on discussions and decision made regarding patient care. 3.6 Works within the guidelines set out in the Belfast Health and Social Care Trust Policies and Procedures. 3.7 Promote a positive relationship with all stakeholders within the Trust. Ensure effective internal and external communication channels are established and maintained. Informally and formally network within multi-professional teams within Trust and outside Trust. 3.8 Participate in the selection, interviewing and recruitment of staff as required identifying future workforce requirements. 3.9 Meet regularly with the Hospital Supportive and Specialist Palliative Care Team Leader to review lung cancer developments within the Trust and Regionally Diffuse and resolve any concerns of complaints of patient/carers where possible using excellent communication and negotiation skills Assist the Hospital Supportive and Specialist Palliative Care Team Leader to investigate and respond to written complaints learning from the experience of users to inform how service provisions can be improved To monitor, order, control and store resources according to the requirements of the team ensuring efficient and effective use of material resources/supplies within the team. 5

Infection Prevention and Control The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff has a part to play in making this happen. Staff must comply with all policies in relations to Infection Prevention and Control and with on-going reduction strategies. Standard Infection Prevention and Control Prevention and Control 6

7 Precautions must be used at all times to ensure the safety of patients and staff. This includes:- Cleaning hands either with soap and water or a hand sanitizer at the appropriate times (WHO 5 moments ) ; Using the correct 7 step hand hygiene technique; Being bare below the elbows when in a clinical environment Following Trust policies and the Regional Infection Control Manual (found on intranet); Wearing the correct Personal Protective Equipment (PPE); Ensuring correct handling and disposal of waste (including sharps) and laundry; Ensuring all medical devices (equipment) are decontaminated appropriately i.e. cleaned, disinfected and/or sterilised; Ensuring compliance with High Impact Interventions. Portfolio Evidence of post registration palliative care/cancer care or relevant subject Evidence of continued professional development Further post graduate study & evidence of professional development of courses related to cancer & palliative care, E.g.: Clinical Examination Skills in Health Assessment Certificates Non-Medical Prescribing. Certificate in Counselling. RELEVANT EXPERIENCE Minimum of 5 years post registration experience, Previous experience of working as a Application form Interview 8
