1 JOB DESCRIPTION SECTION IDENTIFICATION Job Title: Responsible to: Hospice Band: Department: Location: Registered Nurse Clinical Services Clinical Services Manager Band 6 Day Therapy Unit Nottinghamshire Hospice SECTION 1 JOB SUMMARY The post holder will be an experienced nurse, who acting within their professional boundaries, will provide holistic care for patients who are referred to Nottinghamshire Hospice. This will involve history taking, nursing assessment, care planning, treatment and evaluation of their care. The post holder will work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policies and procedures. The post holder will be required to participate in a nurse in charge rota for day and to visit patient on other locations e.g. their home, to complete assessment for hospice care. The post holder will provide a positive role model for team members and to promote excellent care and compassion for all who use the hospice services. 2 SECTION 3 KEY WORKING RELATIONSHIPS Hospice Physician Clinical Services Manager GPs District Nurses Multidisciplinary Team SECTION 4 MAIN DUTIES AND RESPONSIBILITIES OF THE POST Infection Control: To be the Hospice lead for infection control, under the direction of the registered manager, attending meetings and proactively influencing and maintaining a high degree of cleanliness in line with legislation and evidence based practice e.g. NICE. To contribute to the development, implementation and monitoring of infection control policies and procedures. To contribute to and undertake infection control audits as required by the Clinical Services Manager Clinical Responsibilities: To provide a high standard of care through written care plans for the patients, which includes to plan, assess, implement and evaluate the care given to the patients To participate in a nurse in charge rota for day therapy. This includes coordinating daily activities to ensure the patients receive safe care. To monitor all aspects of patient care, (including diet and physical activity) to ensure the highest possible standard of care is maintained To prepare and present patient case files in case conference and as part of the multidisciplinary team, partake in the evaluation of individualised care packages to ensure patient outcomes are being met and ensure any actions agreed are carried out. To establish a therapeutic relationship with patients and their relatives to ensure that, as far as possible, the family and / or carers are included in the care provided. To have clinical responsibility for an allocated group of patients To be familiar with, adhere to and participate in the development of clinical policies and procedures To accept and assess new patient referrals for hospice services. Provide support to families and other carers, prior to and following a death, involving other agencies or disciplines as appropriate. To keep accurate records in accordance with the NMC guidelines on documentation and Professional Code of Conduct. To proactively provide evidence based practice to ensure the hospice is able to provide high quality, cost effective care.

3 To contribute to the health and safety and wellbeing of all patients, carers and self by being aware of and adhering to Nottinghamshire Hospice policies and guidelines on health and safety at work, fire regulations and infection control. Communication: To liaise closely and share information (in accordance with patient s wishes) with those who are involved in the care and discharge of patients. To be responsible for ensuring members of the disciplinary team are aware of the needs of new patients and changes to the needs of existing patients. To maintain good relationships with the carers of the patient giving explanations for care and emotional support as necessary. To support and comfort the bereaved and to facilitate appropriate bereavement support. To liaise with the members of other services e.g. Support Services to ensure provision of spiritual/counselling care of patients and to relate how patients are engaging with any activities programmes. To participate regularly in staff meetings. To ensure safe and proper arrangements are in place for the clinical needs of patients with regards to Hospice transport. To liaise and communicate with other professionals and the general public in order to raise awareness and promote hospice services Audit and Quality To be aware of new development in Palliative and End of Life Care. As part of a team initiate, promote and participate in clinical audit and service users feedback programmes To actively participate in the monthly journal club. SECTION 5 MANAGEMENT To develop effective working relationships with all members of the multidisciplinary team in the hospice. To foster positive working relationships with healthcare professionals across Nottinghamshire. To manage and maintain a safe clinical environment. To manage allocated resources effectively. Ensure competence in the use and function of equipment report any malfunction. To maintain accurate and timely patient records both manually and electronically. To be responsible for conducting supervision and appraisals for Palliative Nursing Assistants and to identifying their learning and training needs. To report accidents, incidents, concerns and complaints relating to patients or their

4 relatives as per hospice policy and procedures, and assist with any investigation as appropriate. To actively participate in clinical recruitment, induction, supervision and performance appraisal. To ensure compliance with Nottinghamshire Hospice policies and procedures e.g. Health & Safety, SECTION 6 PROFESSIONAL DEVELOPMENT, EDUCATION AND TRAINING To keep abreast of current advances in nursing practice. To take responsibility for attending mandatory training in line with organisational policy and current legislation To take responsibility for own personal and professional development, identifying training and education needs during supervision and annual appraisal. Promote a learning environment for patients, students nurses and other healthcare professionals To participate in the development and delivery of training to ensure that staff and volunteers are informed and competent to meet the expectations of their role. To assist in the introduction of new team members and influence good communication through the dissemination of knowledge, To be the practice learning lead for students. This includes mentoring students on placement and being the Practice Learning Team representative for the Hospice. SECTION 7 WORKING CONDITIONS / EFFORT Physical skills required include a range of dexterity and sensory skills necessary for patient care and well-being, e.g. o o o Moving and handling of patients using equipment in a safe manner including transferring patients to and from their homes. Effective operation and monitoring of equipment (oxygen concentrators, feed pumps, etc.). Monitoring of changes to patient s condition and symptoms and reporting same. Support relationships with other members of the multi-agency / multi-disciplinary team and community partners. Ability to contribute to care planning and input required to deliver appropriate packages of care. The ability to empathise with patients, carers and colleagues and to make effective judgments regarding an appropriate level of psychological support. The ability to form positive relationships with patients and carers in a professional courteous and sensitive manner.

NO-SMOKING POLICY From 1 st July 2007 the law prescribes that employers can no longer allow employees to smoke in smoking rooms and employers can no longer segregate smokers and non-smokers within the building. The organisation is now enforcing this policy. The organisation has developed this policy in consultation with its stakeholders to help assist and providing a safe, healthy and comfortable environment for its employees, customers and any other visitors to its premises. The policy complies with the health bill and applies to staff at all levels. The Building and Transport Supervisor is responsible for implementing and maintaining the policy on behalf of the Chief Executive. The policy applies to all visitors and patients and will also apply to company vehicles. Employees who smoke may want to take the opportunity to give up smoking and every support will be given by the employer to support this process. 6 SECTION 9 - CONFIDENTIALITY AND FREEDOM OF INFORMATION Information relating to patients records, diagnosis and/or treatment of patients, staff records, or information concerning contracts, tenders and other commercially sensitive matters etc. are considered to be confidential and must not be divulged without prior authority other than in accordance with the provisions of the Policy on raising concerns about Health Care Services as may be amended from time to time. Breaches of confidentiality will result in disciplinary action, and may result in dismissal. Managers are also required as a condition of this Contract to represent the views of the Hospice in any dealing they may have with employees, their representatives, the media, general public or other organisations in which he/she may come into contact. In addition to the above confidentiality requirements you must also comply with all aspects of the law concerned with information handling. For this purpose, the relevant legislation is the Freedom of Information Act This Act places a legal duty on all staff to comply with the rights of the public to access information. 7 PERSONAL SPECIFICIATION REGISTERED NURSE COMMUNITY SERVICES ESSENTIAL DESIRABLE ASSESSMENT METHOD Qualifications Registered as a RN with NMC Evidence of CPD and reflection to meet revalidation requirements Recognised qualification at degree level or equivalent Post registration EOLC course or equivalent Teaching, mentoring experience NMC registration check Original certificates Experience Previous NHS experience. Minimum of 2 years post registration experience Experience in managing long term conditions/end of life care Evidence of working autonomously and as part of a team Proven ability to evaluate the safety and effectiveness of their own and health care workers nursing care. Experience of assessing clinical practice Previous experience of managing a team Experience of working in the community Experience of providing clinical supervision Application Form /CV Knowledge Understanding of evidence based practice Understanding of their own accountability arising from the NMC Code of Conduct Understanding of the health needs of people in the last year of life.
