1 JOB DESCRIPTION POST: LOCATION: Rehabilitation Assistant Stroke Rehab/Elderly Rehab/ Fracture Rehab Team Belfast Trust (rotational through Intermediate Care) GRADE: Band 3 REPORTS TO: RESPONSIBLE TO: Co-Ordinator of Rehab Schemes, Rehab Scheme Therapists and Stroke Scheme Therapist Manager Rehabilitation Schemes JOB SUMMARY/MAIN PURPOSE: Philosophy of Rehabilitation Scheme To offer elderly clients on the threshold of Care Management an intensive period of rehabilitation to enable their individual potential to be maximised. Rehabilitation Assistants will provide a combination of care and rehabilitation under the supervision of Therapists. This care/rehabilitation may take place in Intermediate Care Facilities or in clients own home. Intermediate Care have commissioned nursing home beds in a number of facilities and rehabilitation assistants will also be involved in working in these facilities on a rotational basis, this will be under supervision of Intermediate Care Therapists. Philosophy of Stroke Rehabilitation Scheme The Community Stroke Rehabilitation Service offer individuals, who have been admitted to hospital following a stroke, the choice to have their rehabilitation in their own home. Each client s rehabilitation plan is based on their individual aspirations and goals. The Stroke Team is comprised of, Occupational Therapist, Physiotherapist, Speech & Language Therapist, Social Worker, Rehabilitation Assistants and Clerical Support. The successful candidate will work on a rotational basis with all 3 Rehabilitation Teams, dependent on the needs of the service.

2 Philosophy of Fracture Rehabilitation Scheme To facilitate early discharge from hospital by offering older people a home based fracture rehabilitation service or rehabilitation in an Intermediate Care facility. Main Duties/ Responsibilities 1. To work in accordance with Philosophy and aims of Rehabilitation Scheme 2. To work under supervision of Phyiso/OT and Speech & Language Therapist. To carry out specific tasks as identified in each client individual rehab/care plan 3. To work at all times to a rehabilitation philosophy enabling clients to be as independent as possible 4. To work flexibly between 8am and 10pm. To offer person centred rehab plans designed to individual client goals (NB: 6 hours each day which will be 8am 2pm or 4pm 10pm, an unpaid break of 15 minutes can be taken between 10.30am 11.30am and finish time will be 2.15pm or between 6.30pm 7.30pm and finish time will be pm). Staff will be rostered for weekend work. 5. To carry out or assist individual clients in a range of daily living tasks, these might include: Mobilising Transferring Toileting Household Tasks Washing & Dressing Feeding Social Activity Kitchen Practice Shopping Laundry Communication Techniques Cleaning Prep. Of Meals Carry out intimate, personal care, which might include catheter care, showering, bathing, etc 6. To give support to carers and family 7. To be aware of individual clients changing needs and to communicate these to Team Therapists/Line Manager 8. To participate in individual client review meetings and team meetings 9. To maintain accurate records and undertake reports as required

3 10. To undertake appropriate training/courses as required, this may include NVQ3 in Diagnostic and Therapeutic Support 11. To work with clients who have complex needs and this may include clients with challenging behaviour or clients who are palliatively ill 12. To work in accordance with Cross Infection Policy Guidelines 13. To work with Therapists, Co-Ordinator and other professionals to minimise risk, taking account of duty of care to clients, other staff and to self and to use equipment as directed in care/rehabilitation plan Staff will also be required to maintain equipment stock levels in stores and Intermediate Care Facilities and to deliver equipment to clients home on request of therapists or co-ordinators 14. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environment Regulations 2004 and Data Protection Act Employees are required to be conversant with the Belfast Health & Social Care Trust policy and procedure on records management and to seek advice if in doubt. Environmental Cleaning Strategy The Trust s Environmental Cleaning Strategy recognises the key principle that Cleanliness matters are everyone s responsibility, not just the cleaners. Whilst there are staff employed who are responsible for cleaning services, all Trust staff have a responsibility to ensure a clean, comfortable, safe environment for patients, clients, visitors, staff and members of the public. Infection Prevention and Control The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff. This includes:- Cleaning hands either with soap and water or a hand sanitizer at the appropriate times (WHO 5 moments ); Using the correct 7 step hand hygiene technique; Being bare below the elbows when in a clinical environment ; Following Trust policies and the Regional Infection Control Manual (found on intranet); Wearing the correct Personal Protective Equipment (PPE); Ensuring correct handling and disposal of waste (including sharps) and laundry; Ensuring all medical devices (equipment) are decontaminated appropriately i.e. cleaned, disinfected and/or sterilised; Ensuring compliance with High Impact Interventions Clause: This job description is not meant to be definitive and may be amended to meet the changing needs of the Belfast Health & Social Care Trust.

5 JOB SPECIFICATON POST: Rehabilitation Assistant Stroke Rehab/Elderly Rehab/Fracture Rehab Team LOCATION: Belfast Trust (rotational through Intermediate Care) ESSENTIAL CRITERIA Or 1. Minimum 1 years paid experience working in directly providing care for clients who may be elderly or young physically disabled individuals who require assistance with personal care One year s paid experience working as a Therapy Assistant in undertaking rehabilitation with elderly or physically disabled individuals 2. Full current driving licence with access to a car and willing to work across all Trust areas 3. Willing to work flexibly between 8am 10pm undertaking both am and pm shifts which includes weekend working 4. All Rehabilitation Assistants will rotate around the 3 teams and will be required to work in any of these teams to meet service need 5. Ability to work unsupervised 6. Ability to demonstrate good communication/interpersonal skills 7. Good record keeping/report writing skills 8. 3 GCSEs at Grade C or above, including English Language 9. Willing to undertake NVQ 3 in Diagnostic and Therapeutic Support Aware and all other training required to fulfil this developing role 10. Willing to attend regular team meetings/reviews as required by Therapist/Line Managers

6 This post involves working in regulated activity with vulnerable groups. As such, postholders will be required to register with the Independent Safeguarding Authority. NOTE: Where educational/professional qualifications form part of the criteria you will be required, if shortlisted for interview, to produce original certificates, and one photocopy of same, issued by the appropriate authority. Only those certificates relevant to the shortlisting criteria should be produced. If educational certificates are not available an original letter and photocopy of same detailing examination results from your school or college will be accepted as an alternative. If successful you will be required to produce documentary evidence that you are legally entitled to live and work in the United Kingdom. This documentation can be a P45, payslip, National Insurance Card or a birth certificate confirming birth in the United Kingdom or the Republic of Ireland. Failure to produce evidence will result in a non appointment.
