1 JOB DESCRIPTION Title of Post: Paediatric Diabetes Clinical Nurse Specialist Grade/ Band: Band 7 Directorate: Reports to: Accountable to: Location: Hours: Specialist Hospitals, Women & Child Health Directorate Assistant Service Manager, RBHSC Service Manager, RBHSC Royal Belfast Hospital for Sick Children 37.5 hours per week Job Summary The post holder will be expected to work across care settings as an expert in this clinical field, to deliver specialist clinical diabetes services to children and young people and their families. The post holder will be required to be part of an on call rota providing telephone advice to parents, children and young people regarding the use and ongoing management of insulin pump therapy. Strategic working will be required to influence the development of the service both locally and regionally. Key result areas/ Main Responsibilities Support guidance and clinical nursing expertise will be available to all children and young people with diabetes, their families, carers and other professionals through provision of advanced clinical skills, education, research and leadership to achieve the best quality of life and outcomes for this group of children. Clinical Responsibilities Manage own defined caseload, work autonomously and/or lead a team to achieve person centred care. Communicate complex, sensitive information effectively and empathetically to patients and their families. Assess, plan, implement and evaluate holistic specialist diabetes nursing care in the hospital, home and school setting in partnership with parents, carer s and colleagues in the educational sector. Work in partnership with families empowering parents and children to ensure that most of the management of diabetes occurs at home thus reducing the level of hospital stay at the time of diagnosis and minimising readmission to hospital.

2 Where appropriate, visit children in their school environment to provide education, support and guidance. Act as a source of specialist advice for children, young people and families on the nursing care and management of diabetes, including the provision of basic dietary advice and the management of acute complications Provide specialist teaching for children, young people and families facilitating the development of self care skills and knowledge, at the time of diagnosis and in planned, ongoing, age appropriate education, both individually and in groups Undertake nurse-led clinics and baseline assessments and annual reviews. Liaise with primary care colleagues and other colleagues in secondary and tertiary care services as appropriate to ensure continuity of care Formulate policy, standards and specialist diabetes care delivery strategies and protocols Lead the implementation and evaluation of specialist diabetes care innovations across the Trust to meet regional and national standards Take a lead role with the nominated paediatrician in the implementation of individual pathways of care to deliver insulin pump and continuous glucose monitoring to children and young people with diabetes on an ongoing basis. Establish and maintain best practice for transition into adult diabetes services Lead and develop specific specialist education programmes for nursing and other professional across all care sectors including education, as well as patients and their families Adhere to DHSSPS and NMC guidelines for practice requirements and standards, for example, safe handling, administration, storage and custody of medicinal products Ensure the Twinkle database is updated for each patient for every clinical intervention Undertake a supplementary prescribing remit, where qualified Managerial responsibility To provide expert guidance and ongoing support to all nursing teams involved in patient care. To lead and participate in multi-disciplinary service development, acting as an advocate for the child and family in the co-ordination of all aspects of patient care To lead and advise on nursing issues and resources available in order to maintain service provision and ensure patients receive optimum care. To support the development of business plans with multi-professional team that underpins the strategic development of the service. To contribute to the day to day running and co-ordination of the department working with other disciplines in order to maintain an operational response to daily events. To monitor the safe and effective use of all equipment and supplies To contribute to ensuring the implementation of local and national policies for health and safety, risk management, personnel issues and professional issues including the NMC professional code of conduct, Trust policies and procedures

3 To give specialist advice, lead and participate in service development including pathways of care and clinical policies in order to optimise service provision within given resources and planned future service changes. To participate in the recruitment and selection of staff. Education To lead and participate in the supervision and teaching of all grades of staff, carrying out clinical supervision, preceptorship and mentorship as required. To attend training and updating sessions as appropriate and to ensure team attends ongoing training and education by appraisal and Supervision. To assess and evaluate the educational needs of children, young people and their carers about their condition including medication and treatment. To provide verbal and written information about the condition that is age and culturally appropriate. To advise on national and international policymaking and to participate in regional national and international workshops and conferences. To be a lead resource providing input to patient groups locally and nationally as appropriate. Professional To provide effective clinical leadership. To maintain a professional attitude and work in accordance with the NMC Code of Conduct. To be aware of own learning needs and limitations and to maintain professional expertise by embracing the concept of lifelong learning To lead and participate in relevant nursing research and to establish links with other paediatric diabetic nurse specialists nationally and internationally. To be aware of national developments which may affect local care provision. To participate in Trust-wide groups in the implementation of nursing strategies across the Trust. Environmental Cleaning Strategy The Trusts Environmental Cleaning Strategy recognizes the key principle that Cleanliness matters is everyone s responsibility, not just the cleaners. Whilst there are staff employed who are responsible for cleaning services, all Trust staff have a responsibility to ensure a clean, comfortable, safe environment for patients, clients, residents, visitors, staff and members of the general public Infection Prevention and Control The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff. This includes: Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO 5 moments ); Using the correct 7 step hand hygiene technique; Being bare below the elbows when in a clinical environment;

5 Following Trust policies and the Regional Infection Control Manual (found on intranet); Wearing the correct Personal Protective Equipment (PPE); Ensuring correct handling and disposal of waste (including sharps) and laundry; Ensuring all medical devices (equipment) are decontaminated appropriately i.e cleaned, disinfected and/or sterilised; Ensuring compliance with High Impact Interventions On occasions a specific location may be detailed within the Job Description & Specification, however, as and when considered necessary or appropriate, you may be required to transfer to or undertake other duties in any location specified by the Trust in order to meet fluctuations or priorities in work demands. 6 PERSONNEL SPECIFICATION. Title of Post: Paediatric Diabetic Clinical Nurse Specialist Band/ Grade of Post: Band 7 Reports to: Accountable to: Assistant Service Manager, RBHSC Service Manager, RBHSC Essential Criteria: Be a registered nurse on part 1 (child) of the NMC live register Minimum of 4 years post registration experience including 1 year at Band 6 ( or equivalent) plus 2 years post- registration experience, excluding courses in providing Diabetes care to children and young people OR At least 5 years post registration experience at Band 5 (or equivalent) plus 2 years post-registration experience, excluding courses in providing Diabetes care to children and young people. Evidence of post-registration training and development specific to Paediatric Diabetes Hold a specialist Practice Qualification in Diabetes. Evidence of ongoing personal and professional development Evidence of excellent communication and interpersonal skills Current full driving licence NOTE:

7 Where educational/professional qualifications form part of the criteria you will be required, if shortlisted for interview, to produce original certificates, and one photocopy of same, issued by the appropriate authority. Only those certificates relevant to the shortlisting criteria should be produced. If educational certificates are not available an original letter and photocopy of same detailing examination results from your school or college will be accepted as an alternative. If successful you will be required to produce documentary evidence that you are legally entitled to live and work in the United Kingdom.
