1 JOB DESCRIPTION Title: Cardiac Catheterisation Laboratory Specialist Nurse Grade: Band 6 Location: Reports to: Responsible to: Acute Adult Services, Medical Cardiology Sister/Charge Nurse Cardiac Cath Lab Nursing Services Manager Summary of Role The post holder is responsible for the assessment of care needs, the implementation and evaluation of local and regional cardiac patients attending the Cardiac Cath Lab in both elective and emergency situations, for adult and paediatric patients. The postholder is required to teach and support other nursing and non-nursing staff in specialist procedures. The post holder is required to contribute to the on-call rota that operates to respond to out of hours emergencies. Main Duties Deliver and maintain individual patient care to agreed standard. Participate in the training and supervision of registered nurses, student nurses, post basic students and auxiliary staff in the delivery of patient care. Prepare reports for and receive reports from the day/night nursing team, paramedics, referral hospitals and other healthcare support teams. Co-operate with medical and paramedical staff to ensure that a high standard of patient care is given. Adhere to the DHSS guidelines for the safe handling, administration, storage and custody of medicinal products. Participate in and support appropriate nursing research and assist colleagues to utilize evidence based practice. 1

2 Participate in audit to ensure optimum standards of care. Be responsible for continually updating knowledge and skill base in relation to your own personal development of nursing. Communicate with patients and relatives, providing support, empathy and reassurance to assist in their understanding of care, treatment and progress in elective and emergency situations. Assist with diagnostic and interventional procedures being carried out. Utilise advanced knowledge of equipment and techniques to assist with specialist procedures Assist with paediatric procedures that require specialist knowledge. Be designated to take charge occasionally, of appointed area or equivalent sphere of nursing care, in the absence of the person who has continuing responsibility, taking account of the role and competency of staff when delegating work. Be responsible for the assessment and planning of care needs, the implementation and evaluation of a programme of care that is evidence based and ensures involvement with the patient, family and carers. Actively participate in the multi-disciplinary on-call team working in the Cath Lab or across hospital site, responding to out of hours cardiac emergencies including Primary PCI procedures for the treatment of Myocardial Infarction. Assess patients health care needs using highly developed nursing knowledge and skills and acting upon assessed information. Assess patients for early signs of post procedural complications and act upon findings. Assist medical staff in breaking bad news to both relatives and patients with compassion and sensitivity. Act as a resource to colleagues within the Cardiac Cath Lab, throughout the hospital and across the regional area. Organise own workload to ensure that interests of patients are met. Management/Administrative Participate in the off duty/annual leave arrangement to ensure that the Department has adequate cover. Liaise with other disciplines to ensure that there are adequate supplies to meet the department s needs. 2

3 Ensure a safe environment for patient care and in the absence of same communications with the appropriate department. Adherence to hospital policies Ensure smooth operational nursing of the department in the absence of the Ward Manager who has continuing responsibility. Assist in orientation and induction programmes for new members of staff. Participate in department based assessments and progress reports for learners. Participate in Staff Development Performance Review for trained staff. Assist in promotion of good interpersonal relationships. Promote effective teamwork. Assist in developing and reviewing new protocols. Responsibilities Clinical Leader Provide leadership that facilitates professional decision making and effective team working. Demonstrate clinical expertise. Contribute to and influence practice development within the clinical area. Undertake clinical audit, ensuring that systems are in place that are implemented and evaluated annually. Ensure adherence to the DHSS and NMC guidelines for the safe handling, administration, storage and custody of medicinal products. Ensure that there is a communication strategy between staff and relatives to assist in their understanding of care, treatment and progress of the patient. Maintain own personal and professional development in accordance with NMC code of professional conduct and within the scope of professional practice. Participate effectively as part of the multi-professional team providing an expert nursing perspective. 3

4 Communicate effectively with all members of the multi-professional team to ensure that all patients needs are met. Act as role model for professional standards of leadership, supervision and clinical management. Liaise with the divisional/principal nurse and nursing services manager regarding professional nursing issues. Ensure staffs are aware of and work in line with Trust policies. Assist with the investigation of facts in adverse incidents. Education, Research and Practice Development Ensure that student nurses and post-basic students receive relevant clinical experience, teaching and supervision in the Department. Attend in-service lectures, study days and courses, to comply with Trust policy. Participate in, supports and encourages research and evidence based practice for the enhancement of patient care. Assist in identification of learning needs of staff and, in collaboration with Cath Lab Sister, the clinical education facilitator, ensure programmes are in place to meet these needs. Deliver formal/informal teaching sessions and support others in providing education to patients, relatives and colleagues. Is responsible for ensuring that his/her own continuing professional development is in keeping with Prep requirements. Encourage reflective practice through action learning, clinical supervision and peer review. Assist with orientation and induction of new members of staff including preceptorship for newly qualified and newly appointed nurses. Contributes to the development of a learning culture within the Cath Lab, encouraging all staff to participate in life-long learning initiatives. Actively supports and participates in the Trust s appraisal programme. Support nursing staff to develop management and leadership skills through appropriate delegation, supervision and role modelling. 4

5 Observe any signs of ill health or stress factors in staff assigned to the area and reports as appropriate to the nursing services manager. Demonstrate effective decision making that is patient centred and consistent with Trust policies. Ensure systems are in place to provide a safe environment for patients care within the risk management and clinical governance strategy. Adhere to all Belfast Trust policies. Ensure that systems are in place for safe use of medical devices and that incidents/accidents on faulty devices are recorded, reported and investigated in accordance with Trust policy and ensuring that staff avail of training provided. Systems and Equipment Systems Telephone system. Ability to access using Outlook Express. Ability to access intranet for organizational policies and procedures. Ability to access internet for evidence based practice guidelines and research. Equipment Use of Specialised equipment: Intra aortic balloon pump Defibrillator Trans Aortic-Valve Implantation (TAVI), valve preparation Pacemaker Lead Extraction Volumetric infusion pump and syringe drivers Blood oxygen saturation measurement machine 5

6 ACT machine Glucometer ECG machine External arterial compression devices Generic: Manual handling equipment Physical, Mental, Emotional and Environmental Demands Physical Skills Administration and management of intravenous therapies Dexterity for use of specialised equipment required in cardiac catheterisation and related procedures. Scrubbing technique Venepuncture/Cannulation skills Performs manual defibrillation Physical Demands Frequent moderate physical activity whilst positioning patients both, conscious or unconscious. Occasionally encountering confused and agitated patients. Standing/walking for the majority of the shift. Requirement to wear protective lead aprons for prolonged periods throughout the day. Mental Demands Concentration required when calculating drug dosages, titrating drugs and setting up and changing infusion pumps which contain drugs which are vital to maintaining patient s blood pressure and other parameters whilst subject to frequent interruptions from patients/relatives/junior team members. 6

7 Concentration required when checking documents/case-notes. Concentration required when observing unstable Cath Lab patients whose conditions are continually changing and are unpredictable. Prolonged concentration required on every shift when caring for a Cath Lab patient requiring haemodynamic monitoring, resuscitation etc. Emotional Demands Frequently communicating with and caring for distressed, anxious, worried patients/relatives. Occasionally caring for patients/relatives during and immediately after receipt of bad news. Communicating with the multidisciplinary team in order to enable effective patient management. Occasional exposure to verbal aggression. Working Conditions Frequently exposure to body fluids. Records Management Used as part of their business within the Belfast Health and Social Care Trust, including patient/client, corporate and administrative records whether paper based or electronic and also including s. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environment Regulations 2004 and Data Protection Act Employees are required to be conversant with the Belfast Health and Social Care Trust policy and procedure on records management and to seek advice if in doubt. Environmental Cleaning Strategy The Trusts Environmental Cleaning Strategy recognizes the key principle that Cleanliness matters is everyone s responsibility, not just the cleaners Whilst there are staff employed who are responsible for cleaning services, all Trust staff have a responsibility to ensure a clean, comfortable, safe environment for patients, clients, residents, visitors, staff and members of the general public. Infection Prevention and Control The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff. This includes:- Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO 5 moments ); Using the correct 7 step hand hygiene technique; Being bare below the elbows when in a clinical environment; Following Trust policies and the Regional Infection Control Manual (found on intranet); Wearing the correct Personal Protective Equipment (PPE); Ensuring correct handling and disposal of waste (including sharps) and laundry; Ensuring all medical devices (equipment) are decontaminated appropriately ie cleaned, disinfected and/or sterilised; Ensuring compliance with High Impact Interventions. 9 JOB SPECIFICATION Title: Cardiac Catheterisation Laboratory Specialist Nurse Grade: Band 6 ESSENTIAL CRITERIA Must be a current employee of the Belfast Trust Registered Nurse on Part 1 of the NMC Register (General Adult Branch) Must have at least 2 years experience as RGN on NMC Register in Cardiac Cath Lab or 2 years relevant cardiology experience and 1 year Cath Lab experience Demonstrate the knowledge and skills to assist in all procedures undertaken in the Cardiac Cath Lab inclusive of paediatric procedures ILS training Demonstrate the ability to communicate clearly and confidently Demonstrate a knowledge and understanding of Clinical Governance Experience of teaching and mentoring others Actively participate in rostered off duty shifts, inclusive of the on-call rota NOTE: Where educational/professional qualifications form part of the criteria you will be required, if shortlisted for interview, to produce original certificates, and one photocopy of same, issued by the appropriate authority. Only those certificates relevant to the shortlisting criteria should be produced. If educational certificates are not available an original letter and photocopy of same detailing examination results from your school or college will be accepted as an alternative. 9

This documentation can be a P45, payslip, National Insurance Card or a birth certificate confirming birth in the United Kingdom or the Republic of Ireland.
