1 HUMBER MENTAL HEALTH TEACHING NHS TRUST JOB DESCRIPTION JOB TITLE SPECIALIST NURSE BAND 6 REPORTS TO OPERATIONALLY RESPONSIBLE TO PROFESSIONALLY RESPONSIBLE TO Family Support Team Manager Clinical Lead LOCATION JOB PURPOSE To support the Lead Nurse in providing clinical and professional leadership to nurses within own area of responsibility. To support the Lead Nurse and other colleagues with the implementation of policy, protocol, service/practice development and change within own area of responsibility To continue to demonstrate specialist clinical skills in practice and provide support/advice re service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies. DUTIES AND RESPONSIBILITIES Communication/Relationship Skills (complex information, barriers to understanding)) To lead and present complex, sensitive and challenging clinical and professional communication processes that may impact on service users, carers, staff, the team, the organisation, partner agencies and members of the public, where there are barriers to understanding To communicate complex information to users and carers regarding personal, psycho-social problems in an empathic and supportive way. This will involve using a range of inter-personal skills in situations that can be emotional, and require a satisfactory outcome, e.g. impact of diagnosis To support the Lead Nurse with the implementation of care pathways/ care bundles, and provide effective professional and managerial communication with all staff in own area of responsibility KSF Dimension Communication Level 3

2 Personal, Professional Development To maintain, as part of continual professional development, up to date specialist clinical knowledge/skills in this clinical field, using information to effect change in practice and ensuring the effective dissemination of new knowledge Work closely with the Lead Nurse to provide evidence based nursing advice to peers and junior staff Maintain and demonstrate specialist theoretical and practical knowledge of a range of clinical interventions, procedures and practices relevant to the clinical area Analysis, Problem Solving and Service Planning (complex judgements with a range of options) To assess, plan and advise on complex challenging and high risk clinical situations to ensure safety of patients, carers, staff and the general public Regularly deal with difficult situations, using complex problem solving expertise to interpret, analyse and resolve the situation to a satisfactory outcome Demonstrate specialist skills in assessing the needs of patients and their carers, based on the analysis of complex facts and situations. Interventions will be planned on the assessment and interpretation of this information KSF Dimension Personal and People Development Planning and Organisational Skills (of a number of tasks and activities) To plan care for patients e.g. organising appointments, treatments, interventions, care programme approach (CPA) reviews Responsible for managing and prioritising own workload, balancing the needs of patients and service requirements/competing priorities Quality Physical Skills Maintain up to date training and knowledge of violence and aggression Up to date competency in medication management, e.g. dexterity and accuracy for giving injections Safety

3 Patient Care (develops specialised programmes of care within service area) Develops and maintains the patients control over decision making, assess the patients commitment to the jointly determined plan of care, and fosters personal responsibility for health Deliver care interventions and specialist advice in a variety of settings, that promote patient flexibility and choice, and which are sensitive to cultural preferences Develops specialised programmes of care. Provides specialist advice and support to patients and their families regarding treatments, diagnosis, therapeutic interventions and options available to them To act as CPA care co-ordinator for patients Policy and Service Development Implementation (implements policy for own work area) To contribute to the implementation of policy and practice, informing the Lead Nurse of any barriers to implementation within own area of responsibility/specialism To assist the Lead Nurse, operational managers and other colleagues with policy, protocol, service/practice development implementation and change e.g. Clinical Governance, business planning, nursing strategy To contribute the nursing perspective in reviewing, developing and monitoring operational/clinical policy within own area of responsibility/specialism Evaluates patients responses to the health care provided and the effectiveness of the care Responsible for Financial and Physical Resources (personal duty of care) Ensure operational managers are informed of the need for relevant clinical equipment (e.g. medication management and physical health screening) Responsible for Human Resources (Day to day supervision of staff) To provide regular structured supervision and performance development reviews (PDRs) for nursing staff in own area of responsibility Responsible for identifying and addressing nursing practice issues are addressed in own area of responsibility e.g. poor performance, fitness for practice, competency, work related stress and support the Lead Nurse on the development of action plans to address these : HWB2, HWB3, HWB4, HWB7 Equality and Diversity Service Improvement Level 3 Quality Level 3 Service Improvement Communication Personal & People Development Level 3 G1 Learning & Development Level 2 G6 People Management Level 2

4 To provide preceptorship for newly registered nurses as and when required to do so Contribute to the recruitment, appointment and retention of nurses within own area of responsibility Be a practising mentor/on the live register of mentors Support the Lead Nurse with the continuous professional development (CPD) of nurses in own area of responsibility Responsible for Information Resources (of personally generated information) To provide accurate and timely information to inform Trust/National databases as required Support the Lead Nurse in ensuring that nurses within team/service area have adequate skills and resources to implement record keeping standards Maintain up to date training and basic IT skills, and demonstrate a good working knowledge of confidentiality and data protection Research and Development (audits/surveys as necessary for own work) To work with nurses in own area of responsibility to analyse the outcomes of audit and develop action plans for change To contribute to research projects within the nursing profession and clinical speciality, both locally and nationally To support and enable staff to undertake research projects e.g. literature searches, health needs analysis Freedom to Act (guided by principles and broad occupational policies) To work within professional code of conduct, organisational policy and guidance To provide specialist advice and high level of clinical competency in order to inform and enable nurses to take clinical actions based on the analysis of the specific challenges/risk situation e.g. clinical risk management IK1 Information and Knowledge Level 1 Quality STANDARD PARAGRAPHS It is the responsibility of each member of staff to maintain confidentiality at all times Staff must be aware of and adhere to the provisions of the Safety at Work Act and to ensure their own safety and the safety of colleagues and patients. Staff must also be aware of the action to be taken in the event of fire and must attend a fire lecture annually

5 Manual Handling Training must be attended on Induction and bi-annually thereafter. Staff who are required to move patients are required to attend advanced training annually Other mandatory training requirements that are relevant to the post must be decided and agreed during Personal Development Reviews (PDRs) and a training plan developed Staff should be aware of their individual responsibilities under the Trust s Equal Opportunities Policy and ensure that they adhere to the provisions of the policy Equality and Diversity: staff should recognise and respect the diversity of all staff, colleagues, service users, carers and the public. All staff are expected to contribute to a working environment which promotes and responds positively to difference and diversity Staff should ensure that their actions support the aim of the Trust in relation to ensuring that all children up to the age of 18 years are protected from significant harm, abuse and neglect, within the framework of multi-agency Child Protection Services established in the Guidelines and Procedures issued by the Area Child Protection Committee Staff must be aware of their individual professional responsibilities to develop their practice and deliver care through a Clinical Governance framework (i.e. CPD, Audit, Supervision) Staff should demonstrate through practice and practical understanding, the importance of the continual development of individual, team and service wide quality improvement Staff will be required to work flexibly to meet the needs of the service/organisation, whilst working within the culture of improving working lives and working time directive
