1 PERSONNEL SPECIFICATION POST: Learning Disability Nurse - Band 5 (Hospital and Community) DEPARTMENT: NURSING (ADULT) LOCATION: LEARNING DISABILITY SUB-DIRECTORATE DATE: March 2013 FACTORS ESSENTIAL DESIRABLE QUALIFICATIONS AND/OR EXPERIENCE Level 1 Registered Nurse (Learning Disability branch) on Live NMC Register A willingness to undertake PG Dip Specialist Nursing Practice (Learning Disability Option) Course as and when places become available and course entry criteria are met by candidate (Community Post) 6 months or more experience working within a community learning disability team. A Diploma/Certificate in Community Nursing (Learning Disability) A full current driving licence and access to a car is essential for posts within Community Teams/Facilities. Consideration will be given to alternative travelling proposals in respect of applicants with a disability who cannot hold a licence.

3 COMMUNITY LEARNING DISABILITY SERVICES JOB DESCRIPTION Post: Community Nurse Learning Disability (CNLD) Grade: Band 5 Responsible to: Reports to: Community Head of Service Community Team Leader ROLE The post holder will work under the guidance and supervision of the CNLD Band 6/Team Leader in the management and delivery of care of a caseload, ensuring a high standard of individualised nursing, social and therapeutic care to persons with a learning disability residing within the Western Health and Social Care Trust. Working within a multi-disciplinary team the post holder will carry a caseload acting as the keyworker or care manager under the guidance and supervision of the CNLD Band 6/Team Leader. The post holder is responsible for the assessment of needs, development of care plans, and review of same in keeping with WHSCT and regional policy, procedure and guidelines. The post holder will carry out relevant forms of care without direct supervision and may be required to demonstrate procedures and act as mentor to nursing students. KEY REQUIREMENTS 1. Professional 1.1 To adhere to the NMC Code of Professional Conduct. 1.2 To ensure high quality evidence based care is delivered to clients on the basis of assessed need. 1.3 To monitor standards and progress of individuals care as directed in care plans and report significant changes to CNLD Band 6/Team Leader. 1.4 To act as keyworker, or care manager in keeping with individual client needs in accordance with associated guidelines and standards.

4 1.5 To identify the needs of the clients by completing both comprehensive nursing assessments and other relevant assessments as and when required and ensuring appropriate individual care plans are implemented and evaluated. 1.6 To participate as an integral member of a multi-disciplinary team in referral discussion meetings, multi-disciplinary case discussion, reviews and meetings as appropriate. 1.7 To carry out risk screening and assessment in keeping with operational guidelines and to develop, implement and co-ordinate risk management plans. 1.8 To facilitate admissions to Lakeview Hospital on the basis of accessed need, continue engagement throughout admission and engage in active discharge planning so as to invoke timely discharge for client. 1.9 To participate in the effective transition of children and young people into adult learning disability services on the basis of assessed need and across other service and agency interfaces in keeping with established procedures and guidelines To give support to the families and carers of clients, and to fulfil associated statutory obligations under the Carers and Direct Payments Act (Northern Ireland), 2002 on behalf of the Trust To assist in the implementation of safeguarding vulnerable adults procedures and to carry out associated duties in keeping with operational guidelines To comply with WHSCT Infection Prevention & Control policy and procedures To participate in the implementation of multi-disciplinary team assessment and care planning as appropriate Under the guidance of and supervision from CNLD Band 6/Team Leader manage a caseload and determine work priorities in accordance with best practice and in keeping with WHSCT policies, procedures and guidelines To participate in formal professional supervision as a supervisee, in line with the Trust s Supervision Policy for Nurses To maintain high quality accurate manual and computer records of all aspects of clients care within agreed time scales To establish and maintain efficient methods of reporting and communicating at all levels within agreed timescales To liaise with other professionals and agencies as appropriate To comply with regulations for storage, control and administration of medicines within the community settings.

5 1.20 To ensure confidentiality at all times To assess, monitor and review clients respite care needs and ensure all relevant documentation is reviewed in accordance with the Residential Regulation and Residential Minimum Standards To engage in continuous professional development. 2. Educational/Research 2.1 To impart professional nursing knowledge to both colleagues within the multidisciplinary team and nursing students. 2.2 To keep abreast of developments within Learning Disability Nursing and actively seek to advance his/her practice and ensure that high quality evidence based care is delivered. 2.3 Contribute to nurse education to both pre-registration and post-registration levels and participate in the provision of relevant programmes of experience by teaching and supervising during placement. 2.4 To participate in In-service training programmes. 2.5 To participate in Health Promotion/Education as required. 2.6 To participate in and contribute to audit and research activity. 3. Managerial 3.1 To engage with and assist the Team Manager in the assurance of safe, effective and efficient community learning disability team services. 3.2 To adhere to the established policies and procedures of the Western Health and Social Care Trust and ensure their implementation. 3.3 Ensure that the most effective and efficient use is made of existing resources, equipment and appliances and to highlight unmet need and service pressures. 3.4 The management of clients care will be within established procedures and reflect the NMC Code of Professional Conduct for Nurses, Midwives and Health Visitors. 3.5 To arrange and monitor Domiciliary Care and Direct Payments on the basis of assessed need in accordance with DHSSPSNI guidelines and criteria and in keeping with established WHSCT systems and processes.

6 3.6 To attend staff meetings, study days, training sessions etc and promote multidisciplinary and whole system working. 3.7 To report all accidents or untoward incidents immediately to the Team Manager and complete the appropriate form in accordance with WHSCT policy on management of risk. 3.8 To respond to complaints in accordance with HSC policy and procedure and to assist managers in investigation and resolution as required. 3.8 To provide statistical and information returns as required. Develop and maintain good communication with all levels of management and build partnerships with the wider HPSS, NHS and external organizations in the promotion of the Trust. 3. 9 JOB DESCRIPTION GRADE: DIRECTORATE: Band 5 Staff Nurse Learning Disability RESPONSIBLE TO: REPORTS TO: Head of Service Line Manager ROLE: The post holder is responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder is expected to carry out all relevant forms of care without direct supervision and may be required to demonstrate procedures to and supervise qualified and unqualified staff, including students. DUTIES AND RESPONSIBILITIES 1. PROFESSIONAL To monitor standards of care and report significant changes to Nurse Manager. To continually assess care needs, develop plans, implement and evaluate individual nursing programmes of care. To ensure assigned duties are carried out to maintain a high standard of care. To supervise the work of nursing staff and develop staff skills based on identified needs. To participate in quality assurance and implement outcomes to improve the quality of care. To prepare reports for and receive reports from Nurse Manager and other staff regarding patients /clients conditions and well being. To work with the broader multi-disciplinary team ensuring patient/client care and treatment is carried out to a high standard.

10 To participate in and contribute to patient/client progress reviews with multidisciplinary team. To complete Learning Disability specific client assessment tools, e.g., Star Profile, Gunzberg, Mini Passad. To ensure that current legal requirements for the administration and storage of medicines are adhered to. To implement all agreed policies and report effectiveness of same as appropriate. To participate in the training of learners and ensure that agreed learner objectives are met. To keep abreast of changes in learning disability nursing and to attend in-service training and courses as part of continuing professional development. To ensure confidentiality is maintained at all times. 2. GENERAL MANAGEMENT To ensure reception/admission of patients in accordance with agreed policy. To report all accidents or untoward incidents immediately to the manager and complete appropriate forms. To ensure that catering, cleaning, portering is satisfactory and report problems as and when they arise. To report complaints and participate in investigations, when necessary. To monitor use of supplies and ensure most effective use of same. To maintain good communication with relatives and significant others regarding patients needs and welfare. To ensure safe-keeping of patients belongings and valuables within agreed policies. To adhere to agreed policies and procedures including standing financial instructions. 3. PERSONAL To encourage good practice and set as a role model, give leadership, giving support to junior staff and encouraging team spirit. To participate in the orientation and induction of new staff and in the development of good interpersonal relationships.

11 To participate in staff development and performance review. To co-operate with nurse educators in the pursuit and maintenance of a high standard of learning for students/trainees.
