3 JOB DESCRIPTION Title: Diabetes Nurse Band: Band 6 Reports To: Team Leader - Community Diabetes Nursing Team Southern Sector Accountable To: Long Term Conditions Services Manager Professionally Accountable To : Executive Director Of Nursing Via The Lead Nurse JOB ROLE The post holder will work as part of the Community Diabetes Nursing Team for the Southern Sector of the Western Health and Social Care Trust. The post holder will be required to provide expert care and treatment to enable people with diabetes to successfully manage their condition while minimising the risk of developing complications associated with the condition. The post holder will be responsible for the provision of high quality clinical care to people with diabetes supported by education, support, advice and counselling to their families and carers. The post holder will also contribute to the provision of specialist education and training in the effective care and treatment of people with diabetes to health care professionals. They will also work collaboratively with colleagues from primary and secondary care, and service users to develop the service and improve the quality of care. DUTIES AND RESPONSIBILITIES. CLINICAL 1.0 To perform comprehensive assessment of health needs of the person with diabetes. Plan care and treatment in consultation with the person and their family and ensure that the agreed care plan is implemented and evaluated.

4 2.0 To demonstrate clinical skills and knowledge in the assessment process, diagnosis, treatment and management of complications. This will include nonmedical prescribing when the appropriate skills, knowledge and competence is acquired. 3.0 To undertake biochemical tests, interpret results, inform the appropriate nursing and medical colleagues and recommend or adjust insulin or other medication regiemes within agreed protocols. 4.0 To work in collaboration with other professionals and agencies to meet the holistic needs of the patients. 5.0 To establish and maintain effective communication with patients, family, carers and other health and social care professional using a wide range of specialist communication skills includign the provision of out of hours contact and advice. 6.0 To manage nurse-led clinics to provide specialist care and advice for referred patients. 7.0 To refer patients to other specialist services when deemed appropriate. 8.0 To maintain accurate records of the care and treatment provided to the patients within the service. 9.0 To ensure that all activity relating to patient contact is recorded on the approved database (currently Diamond Database) To ensure the appropriate and effective use of all resources and technology to support the care of people with diabetes e.g. telemonitoring, and medical equipment To ensure the implementation of the quality assurance scheme/diagnositc equipment on blood glucose monitoring within the Western Trust To work in collaboration with Community Pharmacists to provide medicines management support and advice To proactively link with colleagues within the Long Terms Conditions managaement programmes e.g. Self Management Programmes To work closely with colleagues in Acute Hospital, Long Term Conditions, Diabetes Specialist Team, Intermediate Care and Community Services in the

5 management of complications of diabetes with particular emphasis on those people identified as needing frequent hospital attendance To provide advice on diabetes conditions, support and education for people with diabetes, their relatives, carers, the general public, health professionals, statutory and voluntart bodies with regard to the management of symptoms associated with chronic diseases. QUALITY 1.0 To promote a culture of continuous improvement which focuses on the provision of high quality, safe and effective care and enable staff to maximise their potential underpinned by effective communciation and team working across disciplines. 2.0 To participate in improving the delivery of care for people with diabetes at local and regional level. 3.0 To ensure compliance with the Trust s Clinical and Social Care Governance policies and processes, including risk assessment, accident and incident reporting and complaints management. 4.0 To ensure that diabetes management operates with a clear sense of direction with clear aims and objectives and operates to achieve these goals. 5.0 To ensure that staff comply with the requisite controls assurance standards with particular reference to medicines management, infection prevention and control, management of medical devices and environmental cleanliness. 6.0 To comply with audit systems in place to monitor, evaluate and improve diabetes services ensuring that practice is evidence based. 7.0 To work in accordance with performance management arrangements to provide accurate data and information to inform the achieive of service objectives. SERVICE DELIVERY AND PERFORMANCE 1.0 To work in accordance with all relevant legislative requirements and delegated statutory functions. 2.0 To work collaboratively across organisations including General Practitioners and other primary care colleagues, integrated locality teams, intermediate

6 care, integrating working practices in diabetes services reventing duplication and promoting seamless care in the best interest of people using the service. 3.0 To support the collection and evaluation of information to support clinical and service audit. Maintain accurate and up to date patients records, databases and specialist reporting in accordance with Trust Policies, Professional Guidance and Legislation such as the Data Protection Order. 4.0 To work in accordance with palliative care practices and policies to ensure that services are delivered efficiently and effectively providing best value to people and organisations. 5.0 To respond to feedback from service users to improve the delivery and quality of diabetes services to the Trust population. INFORMATION AND COMMUNICATION 1.0 To ensure the effective use of Trust information systems to capture service and practitioner activitiy. 2.0 To support the interface between hospital, primary, community and social care settings. 3.0 To communicate effectively with a range of health and social care professionals, patients and service users. 4.0 To ensure that records pertaining to patient care reflect an accurate and contemporaneous record of care and treatment received by the post holder. 5.0 To support the development of clincial and professional networks for patients with diabetes. 6.0 To engage in Trust processes to identify the learning needs of the team. 7.0 To contribute to the planning and delivery of structured diabetes education programmes for patients carers and health care professionals on all aspects of diabetes and its management.
