1 JOB DESCRIPTION JOB TITLE: BAND: LOCATION: REPORTING TO: Prescribing Support Assistant Temporary/Secondment Opportunity (for 12 months in the first instance with the possibility of an extension) Band 4 ( 18,838-22,016 per annum) TBC Duties may require travel to various locations throughout NI Medicines Management Dietitian RESPONSIBLE TO: Assistant Director Integrated Care Head of Pharmacy and Medicines Management JOB SUMMARY The post holder will provide support primarily to the Medicines Management Dietitian to improve the quality and cost effectiveness of prescribing. The post holder will independently correlate pre-clinical assessment data within GP practices and undertake audits of prescribing as well as facilitating prescribing change at individual practice level. The post holder will be required to work as part of the medicines management team and work with dietetic services, GP practices, care homes, community nursing, community pharmacists and other primary care staff patients and carers where appropriate to facilitate agreed prescribing changes. Main Responsibilities 1. The post holder will work primarily with the Medicines Management Dietitian(s) to identify, discuss and prioritise practices for change. 2. They will develop a plan of work to ensure that the collation of audits/pre-clinical assessment data is coordinated within the Medicines Management Team. 3. The post holder will be responsible for the effective and efficient delivery of their workload. 4. He/she will correlate pre-clinical assessment data for patients registered with individual practices.

2 5. He/she will perform a baseline and final audit of prescribing and other agreed patient health related data/markers. 6. The post holder will be required to work as part of the medicines management team and work with dietetic services, GP practices, care homes, community nursing, community pharmacists and other primary care staff, patients and carers where appropriate to facilitate agreed prescribing changes. 7. Provide counselling, advice and education to patients and their carers following clinician referrals as appropriate in line with Dietetic care plan. 8. Under direction from the Medicines Management Team and with the agreement of General Practitioners, assist practices to develop and improve prescribing management systems. 9. Provide/undertake training to fulfil the duties of the post as necessary. 10. Maintain, collate and present information and reports as necessary. 11. Maintenance of high standards of confidentiality in respect of all matters. GENERAL RESPONSIBILITIES The post holder will be required to: 1. Travel regularly between HSC and other sites with limited time between meetings. 2. At all times provide a caring service and to treat those with whom they come into contact in a courteous and respectful manner; 3. Ensure the ongoing confidence of the public in service provision; 4. Demonstrate commitment by regular attendance and the efficient completion of all tasks allocated; 5. Carry out their duties and responsibilities in compliance with health and safety policy and statutory regulations; 6. Promote the policy on equality of opportunity through his/her own actions and ensure that this policy is adhered to by staff for whom he/she has responsibility; 7. Comply with the No Smoking Policy; 8. Comply with the HPSS code of conduct.

Only information contained in the application form will be considered at the shortlisting stage. ESSENTIAL CRITERIA Qualifications/Education and Experience 1. A minimum of 5 GCSEs Grade C or above, to include English Language and a numerical subject AND 2.a) 2 A Levels, equivalent or higher qualification AND a minimum of 18months relevant experience* OR (b) 3 years relevant experience*. (*Relevant experience is defined as experience working in a Health Care setting, carrying out an administrative role with contact with patients and clients) 2. Hold a full current driving license (valid in the UK) and have access to a car at the closing date or have *access to a form of transport which will permit the applicant to carry out the duties of the post in full. * This relates only to any person who has declared that they have a disability, which debars them from driving 3. Previous working experience and knowledge of computer systems used within a general practice environment (for example; EMIS, VISION, Torex, etc)

6 4. Experience of using Microsoft office including Word, Powerpoint, Outlook. 5. Experience of creating, operating and analysing excel spreadsheets. 6. Effective oral and written communication, interpersonal and team working skills to meet the needs of the post in full. 7. Ability to organise, and plan work for self and others as required. 8. Demonstrate an ability to use initiative and work independently and as part of a Multidisciplinary team. 9. Demonstrate an ability to prioritise work to meet challenging deadlines. 10. Demonstrate an ability to maintain confidentiality. 11. Experience of development and implementation of protocols/procedures. SHORTLISTING A shortlist for candidates for interview will be prepared on the basis of the information contained in the application form.
