1 JOB DESCRIPTION Title of Post: Grade/ Band: Directorate: Reports to: Accountable to: Location: Hours: Clinical Nurse Specialist in Attention Deficit Hyperactivity Disorder (ADHD) Band 8A Specialist Hospitals, Women & Child Health Directorate Assistant Service Manager for Community Paediatrics, Royal Belfast Hospital for Sick Children (RBHSC) Service Manager, RBHSC The Carlisle Health and Wellbeing Centre The Bradbury care and Treatment Centre The Arches Health and Wellbeing Centre Beech Hall Wellbeing and Treatment Centre 37.5 hours per week The service Belfast Trust is seeking to ensure that the Attention Deficit Hyperactivity Disorder (ADHD) service is delivered in line with NICE Clinical Guideline 72, the Diagnosis and Management of ADHD for children and young people. The Trust is seeking to offer consultation, assessment, intervention and therapeutic support to children and young people. The service will link with and/or signpost users and carers to other relevant voluntary/community sector organisations, whether locally or nationally based, for additional information and support. The ADHD service falls under the remit of community paediatrics and the postholder will work in a multidisciplinary team environment. With community paediatrics there are currently 5.55 wte Consultant Paediatricians, 1.70 wte Associate Specialists and 1.7 wte Specialty Doctors. Externally the post holder should be able to represent the ADHD service and the Trust whilst engaging with external stakeholders from clinical or non-clinical backgrounds. Job Summary The Clinical Nurse Specialist, who is an independent/ supplementary prescriber, will work within the community paediatrics team to provide a specialist clinical assessment and intervention ADHD service to children, young people and their families. This will include writing prescriptions for controlled drugs and other medications where appropriate. The post holder will also be responsible for contributing to the strategy to enable the Belfast Trust to be compliant with NICE
2 Clinical Guideline 72, the Diagnosis and Management of ADHD for children and young people. Post holder will provide a leading role in the further development of the ADHD service for children, young people and their families. This will include engagement with external organisations such as HSCB, schools and voluntary organisations and may involve contracting and business case development. Post holder will be clinically autonomous from the assessment through to the diagnosis and long term management of these children Post holder will contribute to the planning and implementation of outcome monitoring of clinical services, as required. Job will involve complex formulation of needs, co-constructing and agreeing treatment plans / therapeutic interventions with children, young people and their families and the implementation and evaluation of these with minimal supervision Post holder will undertake research and lead audit in own specialist area Post holder will independently and supplementary prescribe medications for this condition as required following local and NICE guidelines The role will include providing specialist training to other professionals and other agencies as appropriate The role will involve leading on consultation to other staff, internal and external to the organisation in their specialist field. Key result areas/ Main Responsibilities Clinical The postholder will undertake clinical assessments of children and young people referred to the ADHD service. The clinical workload will be agreed with the line manager. The clinical work is specialised, and is based upon the appropriate use, interpretation and integration of complex data from a variety of sources, including schools, family members and others involved in the child or young person s care. The clinical assessment and management of risk related to medication is integral to the work. To ensure agreed standardised assessment questionnaires are completed with children, young people and their families referred to the ADHD service as per the ADHD protocol and to systematically collect data on caseload as required by the Assistant Services Manager and Clinical Lead.
3 The postholder will be expected to arrange reviews as required and communicate effectively with the child, young person and their family and all others involved; and to monitor progress during the course of assessment and intervention. To plan and undertake specialist clinical assessments for children and young people referred for suspected ADHD To offer a variety of complex treatment plans, for children, 0-16 years, referred with ADHD. To autonomously make clinical decisions within the context of assessments and work with families to identify appropriate treatment interventions that best meets the need identified. To take a leading role in the assessment and management of ADHD; to develop this area of clinical expertise and interest, and maintain a resource library on the subject. To take on specific strategic tasks within the role. Utilise highly developed communication skills and interpersonal skills in engaging all members of the family in the assessment process. Act as their advocate when their needs are not being met. To use a range of nursing and therapeutic interventions including family therapy, direct work with the child, liaison and joint work with other agencies. To take a lead in devising, developing and facilitating parenting programmes and group work where the main problem is ADHD. Help facilitate and mentor the use of expert patients/users of the service to participate in the delivery of care provided for children and their families with a diagnosis of ADHD Co-ordinate and run children s groups such as to improve management of ADHD To develop and manage own Nurse-led ADHD clinic s. To monitor, review progress and stop treatment in accordance to NICE guidelines, local policies or individual s clinical condition. Continue to give expert advice to families outside of appointments, managing a very unpredictable caseload. Independently or Supplementary prescribe medications for ADHD, under agreed service protocols and /or under supervision from Paediatrician.
4 Monitoring the health of children on medication for ADHD and make clinical decisions accordingly. To evaluate and monitor clinical practice, reassessing needs and interventions, as part of the therapeutic process. Communicate complex information appropriately with referrers and other involved agencies regarding assessment and ongoing treatment. Work within the community paediatric team for children with ADHD to offer highly specialised supervision groups to other colleagues and professionals alike, ensuring that the quality of ADHD assessment and interventions are maintained to high levels The post holder will also work closely in partnership with individuals, groups and agencies in developing a range of joined up community based interventions and provision for children, young people and their families across the tiers. To work jointly as appropriate with other professionals in the support of children and young people with ADHD needs, offering a nursing perspective and maintaining a positive multi agency culture. To participate in all team structures and forums within the team to help shape the future of the ADHD service within community paediatrics. Participate in, and use National Prescribing centre competencies framework, to undertake audit and review, of prescribing practice and medicines management. Maintain relevant clinical and pharmaceutical knowledge, critically appraise and apply information in practice. Prescribe Controlled drugs and other medications, safety and effectively, acknowledging own limitations and scope of practice adhering to the latest legislation. To act as a resource utilising expert advice for advice / consultancy for other professionals regarding the service and children experiencing mental health difficulties through participation in the telephone duty system. Professional Work to expand the role of the ADHD nurse in connection with new ways of working in community paediatrics, to include nurse prescribing and other nurse led initiatives in line with agreed service developments. Attend Team meetings, and other professional meetings as arranged.
5 Working positively with colleagues to maintain clinically effective relationships. Responsibility for ensuring they comply with Trust clinical governance requirements. Participating in clinical supervision/mentoring To work with the Trust to manage demands placed on the service. Education To plan and lead in the implementation of specialist training programmes related to ADHD to Social Services, Education, Health and Voluntary agencies. The post holder will work towards promoting ADHD awareness offering specific training, education and consultation to primary care colleagues to increase their awareness, skill and knowledge in relation to ADHD and its overlapping disorders. To participate in clinical/nursing teaching to other professionals, and to offer consultation to tier 1 in the community working with children and families. Take responsibility in updating knowledge of developments around the treatment and diagnosis of ADHD. To support and mentor more junior and student nurses and others within the team Continually update and consolidate nursing knowledge in line with NMC statutory requirements and the nurse prescribing role specifically. Compliance with Trust arrangements for continuing professional development. Management To receive clinical and case load supervision Be able to create audit trails, specialist surveys and participate in research related to nurse prescribing and in the new ways of working for ADHD service within community paediatrics. To participate and work closely with families and staff who are completing clinical drug trails (if applicable). To participate in and contribute to service development and take a lead role in action plans addressing areas of team /service development To contribute to the implementation of services and quality improvement.
6 To ensure appropriate medical records are maintained ensuring compliance with the agreed record keeping systems of the Trust. Informed consent to treatment is obtained and documented in accordance with Trust procedures. Appropriate risk and other assessments are completed and documented in a timely manner in accordance with Trust policy. To participate in the selection and recruitment of new staff as appropriate. To work collaboratively within the teams administrative support structures and adheres to requirements relating to the recording of statistical and clinical data on the clinics database. Environmental Cleaning Strategy The Trusts Environmental Cleaning Strategy recognizes the key principle that Cleanliness matters is everyone s responsibility, not just the cleaners. Whilst there are staff employed who are responsible for cleaning services, all Trust staff have a responsibility to ensure a clean, comfortable, safe environment for patients, clients, residents, visitors, staff and members of the general public Infection Prevention and Control The Belfast Trust is committed to reducing Healthcare associated infections (HCAIs) and all staff have a part to play in making this happen. Staff must comply with all policies in relation to Infection Prevention and Control and with ongoing reduction strategies. Standard Infection Prevention and Control Precautions must be used at all times to ensure the safety of patients and staff. This includes: Cleaning hands either with soap and water or a hand sanitiser at the appropriate times (WHO 5 moments ); Using the correct 7 step hand hygiene technique; Being bare below the elbows when in a clinical environment; Following Trust policies and the Regional Infection Control Manual (found on intranet); Wearing the correct Personal Protective Equipment (PPE); Ensuring correct handling and disposal of waste (including sharps) and laundry; Ensuring all medical devices (equipment) are decontaminated appropriately i.e. cleaned, disinfected and/or sterilised; Ensuring compliance with High Impact Interventions Clause: This job description is not meant to be definitive and may be amended to meet the changing needs of the Belfast Health and Social Care Trust.

Applicant must be a registered nurse on the live NMC register Part 1 children s or Part 1 mental health Or hold the specialist Community Public Health Nursing Health Visitor qualification Or hold the Specialist Community Public Health Nursing School nurse qualification of the NMC register 2. Applicant must have a minimum of 2 years post registration experience including 1 year at band 6 or equivalent 3. Applicant must have a minimum 2 years post registration experience within a service working with children and young people with ADHD and its overlapping disorders. 4. Applicant must have extensive knowledge of the ADHD diagnostic procedures and treatment packages offered. 5. Post registration qualification in independent and supplementary prescribing recordable with the NMC 6. Applicant must have a full driving licence valid in the UK and have access to a car, the criteria will be waived in the case of an applicant whose disability prohibits driving but who is able to organise suitable alternative transport.
9 Where educational/professional qualifications form part of the criteria you will be required, if shortlisted for interview, to produce original certificates, and one photocopy of same, issued by the appropriate authority. Only those certificates relevant to the shortlisting criteria should be produced. If educational certificates are not available an original letter and photocopy of same detailing examination results from your school or college will be accepted as an alternative. If successful you will be required to produce documentary evidence that you are legally entitled to live and work in the United Kingdom. This documentation can be a P45, payslip, National Insurance Card or a birth certificate confirming birth in the United Kingdom or the Republic of Ireland. Failure to produce evidence will result in a non appointment.
