2 TITLE: SPECIALTY: BASE: Specialty Doctor Orthopaedics and Trauma Surgery Altnagelvin Area Hospital RESPONSIBLE TO: Lead Clinician ( Mr K Lappin ) JOB SUMMARY The successful applicant will help to provide a high quality general orthopaedic service for a rural population of 320,000 plus at present and, with the potential inclusion of Coleraine orthopaedics this population will increase to approx The successful applicant will also help to provide a high quality trauma service for a population of at least as the WHSCT already looks after Enniskillen and Coleraine Trauma. The trauma workload increases seasonally peaking in the summer with the inclusion of the Lakeland, North Coast and Donegal holiday makers. PROFILE OF THE TRUST AND HOSPITAL THE TRUST: The Western HSC Trust came into being on the first of April 2007, following the rationalisation of hospital services in Northern Ireland. The Trust is a result of the amalgamation of Altnagelvin hospital Trust, Sperrin- Lakeland Trust and Foyle Trusts. THE HOSPITAL: Altnagelvin Area Hospital is the base hospital for this orthopaedic and trauma post and is situated on the outskirts of Londonderry in an area of outstanding natural beauty. It was opened in February 1960 and was the first new hospital to be built in the United Kingdom after the Second World War. It is the largest district hospital in Northern Ireland and has the widest range of specialties outside the Belfast Trust. The WHSCT includes satellite facilities in the nearby town of Limavady and on the Gransha Hospital site some three miles from Altnagelvin.
3 WORK OF DEPARTMENT The Trauma and Orthopaedic department provides an effective and efficient Orthopaedic and Trauma service within the contract resources available to the Western HSC Trust. We provide a trauma and elective service to Londonderry, Enniskillen, Omagh, Limavady and Coleraine. It is necessary therefore to undertake outpatient clinics at the Erne Hospital, Tyrone County Hospital, Roe Valley and the Causeway Hospital. SUPERVISION ARRANGEMENTS This post will function at ST3 level with the expectation of advancement within the post to the highest level of speciality doctor. Supervision will be geared to the successful candidates needs but it is envisaged that with time the post will become a senior stand alone post. Appraisal and audit of results will be an essential part of this job. KEY DUTIES / RESPONSIBILITIES The postholder will be required to run stand alone trauma lists as well as stand alone fracture clinics. They will be responsible for the ward management of patients they have operated upon. The postholder will also be required to work not only within Altnagelvin hospital but within the other hospitals for outreach clinics both orthopaedic and fracture. The postholder will be required to participate in the middle tier trauma rota. So far as is consistent with the proper discharge of the above duties, the postholder must undertake to deputise from time to time for absent colleagues any request to provide cover will be reasonable, safe and practible to do so. The postholder will be expected to participate in initiatives to improve clinical effectiveness and maximise the use of trust facilities The postholder will be expected in the development of educational programmes for medical and nursing staff within their area The postholder will be expected to participate in CPD within the guidelines set by the GMC and the BOA. The postholder will be expected to participate in audit
4 KEY ORGANISATIONAL RELATIONSHIPS The postholder will 1. be professionally and clinically responsible to the clinical lead for Trauma & Orthopaedic Surgery 2. be responsible to the Medical Director for the maintainence of professional standards 3. undergo Annual appraisal by one of the orthopaedic consultants CONTINUING PROFESSIONAL DEVELOPMENT The post holder will: Play a significant role in medical education. The successful candidate will be expected to participate fully in undergraduate and postgraduate teaching and to promote knowledge of Orthopaedic and Trauma surgery amongst colleagues. Altnagelvin Hospital participates in the teaching of Orthopaedic and Trauma surgery to medical students. The successful candidate will be expected to participate fully in the programme. Seek to develop research-based practice within statutory services and in combination with outside academic institutions. The post holder will maintain his or her own personal development through: 1. Maintaining a high level of specialist knowledge in Orthopaedic and Trauma surgery extending his/her professional and management knowledge and skills. 2. Fulfilling the relevant commitment to continuing professional development/continuing medical education 3. Actively participate in clinical audit. 4. Encourage research within the specialty

If it is not possible to agree a Job Plan, either initially or at an annual review, there are agreed procedures for facilitation and appeal with the final decision being binding on both the doctor and the Trust. EMPLOYING AUTHORITY Western Health and Social Care Trust. TERMS AND CONDITIONS The National Terms and Conditions of Service for Specialty Doctors will apply. As per the current salary scale under National Terms and Conditions of Service for Hospital staff as updated via DHSSPS circular on rates of pay,
7 the most recent HSS (TC8) 3/2011, with effect from 1 April to 31 March 2012 and all uplifts thereafter. The full-time appointment will be for 12 PAs per week. The successful applicant will normally be appointed to the grade for a one year probationary period which, if confirmed, will be extended without term and held until retirement under the terms and conditions of service. The appointee will maintain registration with the General Medical Council (London). Annual leave entitlement is 5 weeks (25 days) per year until 2 years service have been completed in the grade and 6 weeks (30 days) thereafter for practitioners remaining in the grade. Where the appointee in his/her immediately previous regular appointment attracted the 6 weeks annual leave per annum, this entitlement would remain. You are required to live within a distance of 30 minutes or 10 miles by road from your base place of work unless the employing organisation agrees that they may reside at a greater distance. Your private residence shall be maintained in contact with the public telephone service He/she will be expected to be flexible and to co-operate with reasonable requests to cover for their colleagues absences where they are safe and competent and where it is practicable to do so. Where doctors undertake duties in accordance with this paragraph and such duties take place outside of their contracted hours, they will receive either an equivalent off duty period or remuneration. The post will be subject to termination at any time, by three months notice in writing given on either side. GENERAL REQUIREMENTS The post holder must: Carry out his/her duties with full regard to the Trust's Equal Opportunities Policy. Co-operate fully with the implementation of the Trust's Health and Safety arrangements and report any accidents/incidents, defects with work equipment or inadequate safety arrangements. Co-operate fully with all policies and procedures relating to Infection Control. Accept individual responsibility for ensuring a suitable, clean, uncluttered and safe environment for members of the public, staff, patients/clients and their relatives. Comply with the Trust's Smoke Free Policy. Treat those whom he/she comes into contact with in the course of work, in a courteous manner. ADDITIONAL POINTS From 1 January 1990, medical staff have not been required to subscribe to a Medical Defence organisation. It should be noted, however, that the Trust's indemnity only covers the Trust's responsibilities and therefore, the appointee is advised to maintain a membership of a recognised professional defence organisation for any work which does not fall within the scope of the Indemnity Scheme. 11 EMPLOYEE PROFILE FACTOR QUALIFICATIONS & EXPERIENCE ESSENTIAL POST: SPECIALTY DOCTOR - MINIMUM CRITERIA Candidates must: Hold Full Registration and License to Practice with the GMC (London). Have at least 4 years full-time postgraduate training (or its equivalent gained on a part-time or flexible basis) at least one of which will be in a specialty training programme in a relevant specialty or as a Fixed Term Specialty Trainee in a relevant specialty (Trauma & Orthopaedics) or shall have equivalent experience and competencies. MRCS or equivalent A full current driving license and access to a car is essential. Consideration will be given to alternative travelling proposals in respect of applicants with a disability who cannot hold a license DESIRABLE It is desirable that candidates: Have basic courses completed (ATLS etc) Orthopaedic Courses ELIGIBILITY FITNESS TO PRACTICE LANGUAGE SKILLS Communication skills Eligibility to work in the UK. Is up to date and fit to practice safely. All applicants to have demonstrable skills in written and spoken English that are adequate to enable effective communication about medical topics with patients and colleagues which could be demonstrated by one of the following: That applicants have undertaken undergraduate medical training in English; OR Have the following scores in the academic International English Language Testing System (IELTS) Overall 7, Speaking 7, Listening 6, Reading 6, Writing 6. However, if applicants believe that they have adequate communication skills but do not fit into one of the examples they need to provide evidence. HEALTH CLINICAL SKILLS Meets professional health requirements (in line with GMC Standards/Good Medical Practice).
