1 Position Description Position title: Population Health Analyst Location: St Leonards Thornleigh Reports To: Population Health and Informatics Manager Direct Reports: Nil Internal: External: Working Relationships: PD Approved: General Manager Commissioning and Partnerships Commissioning and Partnerships business unit Health Service Engagement business unit General Manager Commissioning & Partnerships Local Health Districts Universities Primary health service providers and NGOs Local and State Government agencies Approved: Organisation description The Sydney North Health Network (SNHN) works on behalf of the Commonwealth to improve health outcomes for patients by increasing the capability and capacity of the primary healthcare workforce to ensure patients receive the right care, in the right place at the right time. Position purpose Position purpose: Working closely with the Population Health and Informatics Manager, the role will contribute to the development of regional primary healthcare population health analysis and the design of appropriate commissioned activities. The role will source, interpret and analyse reliable population health-based data, health system performance information and general practice data, to inform population needs assessment, local priority setting and monitoring, subsequent population health planning in order to support the design of appropriate commissioned primary health care service responses and improvement initiatives. The role will support the Population Health and Informatics Manager to

2 champion the development of an information culture that drives continuous clinical and business improvement across the PHN. This includes the development, application and effective use of health management systems and technology. Accountabilities Primary accountabilities: To ensure the organisation works as effectively as possible to achieve its annual business plan, each team member has responsibility for a range of activities and outcomes. These accountabilities and their outcomes are reviewed annually formally and on an ongoing basis informally with team members and managers. Collate, identify, interpret, analyse and prepare population-based and epidemiological data to inform regional population needs assessment, local priority setting, population health planning, service improvement initiatives and program/service evaluation. Work with the Population Health and Informatics Manager to identify appropriate data collection and analysis methodology for performance reporting, organisational service evaluation and research. As required, undertake data collection, collation and analysis for program/servcie evaluation and research, including the innovative use of GP practice-level data for population health planning and service improvement initiatives. Contribute to the development of an information culture that that drives continuous clinical and business improvement across the PHN and its providers, as an enabler for safe and effective care. Enhance the capacity of the PHN to use relevant data and evidence in health service planning and commissioning, through supporting the development, application and effective use of health management systems and technology. Communicate complex population health information persuasively to decision-makers, including SNHN staff, the Executive, Clinical Councils and Community Advisory Committees. Support and assist the development of partnerships with Local Health Districts, universities, government agencies and other key stakeholders with a particular focus on population health, primary health care service development, evaluation and research. Prepare written reports, articles and oral presentations as required. General Contribute towards the SNHN s overall strategic direction including the implementation of the organisations values and mission statement. Page 2 of 5

4 Key selection criteria - qualifications and experience Required experience, capabilities and qualifications Certifications required Essential: Tertiary qualifications in public health, population health, epidemiology, health sciences or equivalent. Previous experience in health-related data management applied to population needs assessment, strategic population health planning and/or program/service evaluation. Demonstrated knowledge of the principles underpinning the population health approach. Demonstrated ability to communicate and liaise effectively with a wide range of stakeholders, including GPs, researchers, service managers and community representatives Demonstrated ability to prepare technical, descriptive and analytical population health-related data, convert / interpret data into meaningful information and written reports within specified timeframes. Demonstrated ability to contribute concise and meaningful data summaries and analyses to presentations and publications. Desirable: Demonstrated knowledge of the primary health system in Australia, including its interaction with the acute care sector, as well as the challenges and opportunities. Experience in working for a not-for-profit-organisation. Certification of required tertiary qualifications and professional memberships Current NSW drivers licence and access to a comprehensively insured motor vehicle. National Police Clearance Check. Working with Children Check (or willing for a check to be performed). Reference Checks (2) from past employers. Special conditions Workplace Health and Safety Some out of hours work on weekends or evenings may be required, for example, attendance at community forums or meetings, for which time off in lieu may be taken. Intrastate, interstate and/ or international travel may be required.
