1 Academy f Geriatric Physical Therapy Outstanding PT & PTA Student Awards Purpse T identify a student physical therapist (SPT) and student physical therapist assistant (SPTA) with exceptinal schlastic ability and ptential fr cntributin t geriatric physical therapy. T prvide the means fr ne exceptinal student PT and ne exceptinal student PTA t attend and participate in a natinal meeting, with the intentin that this expsure will encurage future invlvement in Academy f Geriatric Physical Therapy (AGPT) activities. T encurage develpment f prgrams and activities that prmte physical therapy fr geriatric ppulatins at the lcal, state and natinal levels. T prmte the Academy f Geriatric Physical Therapy and encurage active student and new graduate participatin in the Sectin. Eligibility The nminee must be enrlled in an accredited r develping PT r PTA prgram at the time f nminatin. Fr student PTA candidates, the nminee must be enrlled in a PTA Prgram and must be btaining their first degree within the prfessin. Fr student PT candidates, the nminee must be enrlled in a DPT Prgram. Individuals wh have btained a prir degree as a PTA but wh are enrlled in a PT Prgram are nt eligible fr the Outstanding PTA Student Award. These individuals are, hwever, eligible fr the Outstanding PT Student Award. Students btaining a transitinal Dctr f Physical Therapy Degree (t-dpt) are eligible fr the Outstanding PT Student Award. The nminee must be a member f the APTA and the Academy f Geriatric Physical Therapy. Criteria fr Selectin The student shall excel in academic perfrmance in bth the prfessinal and prerequisite phases f their educatinal prgram, as evidenced by submitted transcripts. The student shall demnstrate exceptinal nnacademic achievements, representing initiative, leadership, and creativity, bth n campus and in their cmmunity. The student shall be invlved in prfessinal rganizatins and activities that prvide the ptential grwth and cntributins t the prfessin and geriatric physical therapy. Prcedure fr Nminatin Any member f the Academy f Geriatric Physical Therapy may nminate candidates fr this award. 1 f 6

2 The applicatin must be submitted electrnically t the award crdinatr at the Sectin ffice by Nvember 15, fr cnsideratin fr the award in the fllwing year. The materials submitted fr each nminatin shall include n mre than fur (4) letters f supprt as indicated belw in the fllwing rder: A supprt statement frm the nminatr, highlighting reasns fr the nminatin and clarifying the relatinship between the nminatr and nminee. The nminatr may be a clinician, clinical educatr r member f the PT / PTA Prgram s faculty, but must be a member f the Academy f Geriatric Physical Therapy. A supprt statement frm ne (1) faculty member in the educatinal prgram in which the nminee is enrlled. Preference may be given t letters f faculty members wh are current members f the Academy f Geriatric Physical Therapy. Supprt statements frm tw (2) student clleagues wh are members f the APTA Student Assembly. Preference may be given t letters frm students wh are current members f the APTA and the Academy f Geriatric Physical Therapy. In additin t the abve mentined letters f supprt, please include: A cmpleted applicatin frm. A cpy f the APTA Membership Card, indicating current APTA and AGPT membership. The student s mst current fficial transcript. A resume and cver letter frm the nminee detailing previus health care experiences, hnrs and awards, evidence f service activities and participatin in prfessinal activities, including APTA and AGPT-spnsred activities. The nminatin materials shuld dcument examples f hw the nminee fulfills the criteria fr this award. Prcedure fr Review and Selectin Nminatin materials shall be submitted electrnically t the AGPT ffice. The ffice will distribute the materials t the Selectin Cmmittee. The Selectin Cmmittee will review the nminatins and recmmend the mst qualified candidate(s). The awards will be presented nly if there are qualified candidates, and nly if ne is selected in either r bth categries. Nminatin materials are cnsidered the prperty f the Selectin Cmmittee, wh will maintain their cnfidentiality. If any individual is nt selected fr the award in a given year, that individual may be nminated in a subsequent year. New nminatin materials must be submitted in subsequent years. 3 The Sectin Executive will ntify the recipient and btain written cnfirmatin f acceptance. The nminatrs f individuals nt selected will receive a letter thanking them fr their participatin. The cnfidentiality f the Outstanding Student Award(s) will be maintained until the recipient(s) has been ntified. The Award and its Presentatin The Academy f Geriatric Physical Therapy will recgnize the student at its Business Meeting / Awards Ceremny during the Cmbined Sectins Meeting. The student will receive a certificate, cnference registratin, up t $250 fr expense reimbursement and will be recgnized in GeriNtes. 3 f 6
