1 TUITION DISCOUNT PROGRAM FOR THE SCHOOL OF HEALTH SCIENCES AND PRACTICE Date: Nvember 5, 2014 Supersedes: Graduate Schl Educatin Benefits, HR.314, dated 5/8/2001 References: Nne. I. PURPOSE T establish guidelines fr tuitin discunts in the Schl f Health Sciences and Practice ("SHSP") f New Yrk Medical Cllege ("NYMC" r the "Cllege") fr certain Eligible Grups as defined in this Plicy. II. POLICY It is the plicy f the Cllege t encurage the cntinuatin and prgressin f educatin and prfessinal perfrmance thrugh varius prgrams ffered by the Cllege. The Cllege may mdify r withdraw this Plicy any time except that any eligible individual matriculated in the SHSP at the time f such a mdificatin r withdrawal and receiving a tuitin discunt under this Plicy will cntinue t be entitled t the tuitin discunt as prvided herein until ne f the fllwing ccurs, whichever ccurs first: (1) cmpletin f the then current curse f study, (2) the withdrawal frm the SHSP, (3) dismissal fr academic r disciplinary reasns, r ( 4) the individual ceases t be eligible fr the tuitin discunt. As an alternative, any eligible individual matriculated in the SHSP at the time f such a mdificatin r withdrawal and receiving a discunt under this Plicy may elect fr the new mdified tuitin plicy if they are eligible fr the newly mdified tuitin plicy. The Cllege des nt prvide tax advice r cunsel, s any individual receiving a tuitin discunt under this Plicy shuld cnsult with their persnal tax advisr. Infrmatin n "qualified educatinal expenses" can be fund n the Internal Revenue Service website, see IRS Publicatin 970 n Tax Benefits fr Educatin at III. SCOPE This Plicy applies t members f the Eligible Grups as defined belw in Sectin IV.A f this Plicy. IV. DEFINITIONS A. This Plicy shall apply t the members f the fllwing Eligible Grups: Grup 1. Full-Time Faculty fthe Cllege wh are nt emplyed by the Cllege and wh have been emplyed full-time fr at least tw years by, and are currently emplyed full-time by, faculty practice entities cmpliant with the requirements f the Cllege;

2 Grup 2. Nn-physician, full-time emplyees wh are nt emplyed by the Cllege and wh have been emplyed full-time fr at least tw years by, and are currently emplyed full -time by, faculty practice entities cmpliant with the requirements f the Cllege; Grup 3. Full-time emplyees wh are nt emplyed by the Cllege but wh are currently emplyed full-time by lcal, state r federal health departments in the United States; Grup 4. Nn-physician, full -time emplyees wh are nt emplyed by the Cllege but wh are currently emplyed full-time by Affiliated Hspitals; Grup 5. Primary Care Preceptrs fr the Schl f Medicine and Clinical Site Supervisrs fr the Physical Therapy and Speech-Language Pathlgy prgrams f the SHSP; Grup 6. Residents and fellws currently emplyed and in graduate medical educatin (GME) prgrams - directed r spnsred by NYMC; Grup 7. Active Duty members f the U.S. military r U.S. Public Health Service Cmmissined Crps, r civilians currently emplyed in federal agencies fthe U.S. Public Health Service; Grup 8. Students enrlled in the NYMC Schl f Medicine pursuing an MD/MPH degree frm the Cllege; Grup 9. Students enrlled in the NYMC Dctral in Physical Therapy prgram pursuing a jint DPT/MPH degree. B. Tuitin-The fee charged fr educatinal instructins r maintenance f matriculatin fee. Excluded frm this definitin are fees fr admissin, graduatin, testing, husing, meal plans, labratry, and ther purpses. C. Cllege-Cmpliant Faculty Practice Entity A faculty practice entity shall be deemed Cllege-cmpliant under this Plicy if, in the sle discretin f the Cllege, it meets the requirements f the Cllege including financial supprt t the Cllege determined as f the date fthe applicatin fr educatin benefits in the SHSP, and thereafter in each subsequent semester in the SHSP. D. Affiliated Hspitals A Hspital is an Affiliated Hspital under this Plicy prvided that all f the fllwing cnditins are met: 2

3 V. PROCEDURES 1. There is a current written affiliatin agreement with the Cllege pursuant t which it is designated by the Cllege as an Affiliate f the Cllege in accrdance with its standards and requirements; 2. The Hspital prvides clinical and educatinal experiences fr the Cllege's undergraduate medical educatin prgram and/r speechlanguage pathlgy and physical therapy internship prgrams as specified in the respective affiliatin agreement; and 3. The Hspital is at all times herein in full cmpliance with the Cllege's standards and requirements, and as such may be amended, fr the designatin specified in the respective affiliatin agreement. A. Eligibility and Tuitin Discunts by Grup Grup 1. Full-time Faculty f Cllege wh are nt emplyed by the Cllege and wh are currently emplyed full-time in Cllege-cmpliant faculty practice entities shall be eligible t receive a tuitin discunt ftwenty-five percent (25%) fr up t six (6) credits per SHSP semester and up t a maximum f eighteen (18) per year prvided they meet all f the fllwing criteria in each SHSP semester: Be a full-time emplyee f the Cllege-cmpliant faculty practice entity fr a minimum f tw (2) years and currently emplyed full time by such faculty practice entity ; Obtain a written recmmendatin frm the Cllege-cmpliant faculty practice entity's leadership; Meet the requirements fr admissin int ne f the Master f Public Health Prgrams; and Cmplete all degree requirements within five (5) years f matriculatin. Grup 2. Nn-physician, full-time emplyees f Cllege-cmpliant faculty practice entities wh are nt emplyed by the Cllege shall be eligible t receive a tuitin discunt f ten percent (1 0%) per SHSP semester prvided they meet all f the fllwing criteria in each SHSP semester: Be a full-time emplyee f the Cllege-cmpliant faculty practice entity fr a minimum f tw (2) years and currently emplyed full time by such faculty practice entity; Obtain a written recmmendatin frm the Cllege-cmpliant faculty practice entity's leadership; Meet the requirements fr admissin int ne f the Master f Public Health Prgrams; and 3

4 Cmplete all degree requirements within five (5) years f matriculatin. Grup 3. Full-time emplyees wh are nt emplyed by the Cllege but wh are currently emplyed full-time by lcal, state, r federal health departments in the United States, shall be eligible t receive a tuitin discunt ften percent (10%) per SHSP semester, prvided they meet all f the fllwing criteria in each SHSP semester. Written dcumentatin is btained by such full-time emplyees frm their supervisr r department head cnfirming his/her current fulltime emplyment status and jb title with such health department; Meet the requirements fr admissin int ne fthe Master f Public Health Prgrams; and, Cmplete all degree requirements within five (5) years f matriculatin. Grup 4. Full-time emplyees wh are nt emplyed by the Cllege but wh are currently emplyed full-time by Affiliated Hspitals shall be eligible t receive a tuitin discunt in each SHSP semester as prvided in the respective hspital's Affiliatin Agreement, prvided they meet the criteria in each SHSP semester specified in the respective Affiliatin Agreement. Grup 5. Primary Care Preceptrs fr the Schl f Medicine and Clinical Site Supervisrs fr the Physical Therapy and Speech-Language Pathlgy prgrams f the SHSP shall be eligible t receive a ten percent (1 0%) tuitin discunt per SHSP semester prvided they meet all f the fllwing criteria in each SHSP semester: Written cnfirmatin is btained by Primary Care Preceptrs and Clinical Supervisr respectively, frm the Cllege's Office f Undergraduate Medical Educatin and Primary Care f the Cllege's Schl f Medicine r the Department Chair f, as applicable, the Department f Physical Therapy r the Department f Speech Language Pathlgy f the SHSP f their current pmiicipatin in the respective educatinal prgrams; Meet the requirements fr admissin int ne f the Master f Public Health Prgrams; and, Cmplete all degree requirements within five (5) years f matriculatin. Grup 6. Residents and fellws while in Cllege directed r spnsred graduate medical educatin prgrams shall be eligible t receive a tuitin discunt f twenty-five percent (25%) per SHSP semester prvided they meet all f the fllwing criteria in each SHSP semester: 4

5 Written cnfirmatin is btained by Resident and fellw frm the Cllege's Office f Graduate Medical Educatin f the Cllege's Schl f Medicine f his/her current participatin in the respective graduate medical educatin prgram; Meet the requirements fr admissin int ne f the Master f Public Health Prgrams; and, Cmplete all degree requirements within five (5) years f matriculatin Grup 7. Active Duty members f the U.S. military r U.S. Public Health Service Cmmissined Crps, r civilians currently emplyed in federal agencies f the U.S. Public Health Service shall be eligible t receive a tuitin discunt f fifty percent (50%) per SHSP semester fr n-line Master f Public Health (MPH) and graduate certificates educatinal prgrams prvided they meet all f the fllwing criteria in each SHSP semester: Written dcumentatin is btained cnfirming his/her active duty military, U.S. Public Health Service r full-time emplyee status with any fthe abve mentined federal agencies f the U.S. Public Health Service; Meet the requirements fr admissin int the n-line Master f Public Health Prgram (MPH) r ne f graduate certificate prgrams; and, Cmplete all degree requirements within five (5) years f matriculatin. Grup 8. Students in the NYMC Schl f Medicine pursuing an MD/MPH degree frm the Cllege shall be eligible t receive a tuitin discunt f thirty percent (30%) n the MPH tuitin, prvided they meet all f the fllwing criteria in each SHSP semester: Be in gd academic standing in the Schl f Medicine; Meet the requirements fr admissin int the n-line Master f Public Health Prgram (MPH); and, Cmplete all degree requirements within five (5) years f matriculatin Grup 9. Students in the NYMC Dctr f Physical Therapy (DPT) prgram pursuing a jint DPT/MPH degree shall be eligible t receive a tuitin discunt f fifty percent (50%) n the additinal MPH credits which they are required t take fr the jint degree, prvided they meet all f the fllwing criteria in each SHSP semester: Apprval f Chair f the Physical Therapy Department Meet the requirements fr admissin int the n-line Master f Public Health Prgram (MPH) and, 5

6 Cmplete all degree requirements within five (5) years f matriculatin B. General Requirements 1. Ntwithstanding the prvisins f this Plicy, the SHSP reserves, in its sle discretin, the right t limit the number f discunts awarded in any given semester withut any prir ntice. 2. All applicants fr admissin must pssess and be able t demnstrate the academic prerequisites required fr admissin t study and upn admissin, will be subject t all regulatins, plicies, requirements and standards, as adpted frm time t time. 3. Upn admissin, the student must maintain, and remain in, gd academic standing t cntinue in the prgram as determined by the SHSP and shall pay all fees withut discunt, ther than tuitin. C. Applying fr tuitin discunt 1. Interested members f the Eligible Grups shuld cntact the SHSP fr infrmatin brchures and enrllment infrmatin. 2. Prir t each SHSP semester' s registratin, the member f the Eligible Grup shall submit the required dcumentatin t the NYMC Registrar' s Office and the NYMC Bursar's Office at the time f registratin. VI. EFFECTIVE DATE This Plicy is effective as fthe date signed belw except fr members f the Eligible Grups wh are currently enrlled in the SHSP and wh receive educatin benefits as f the date f the signing f this Plicy. Such individuals shall cntinue t receive the tuitin discunt benefit at the level prvided prir t this Plicy until cmpletin f the educatinal prgram he/she is matriculated in, withdrawal frm the schls, dismissal fr academic r disciplinary reasns, r in the event f the terminatin f, as applicable, emplyment r eligibility status f such individual. VII. POLICY RESPONSIBILITIES A. Member f Eligible Grup- cmplete and submit the required frms. B. NYMC Bursar 1. Verify infrmatin n the applicatin frm. 2. Keep a recrd f the applicatin frm in the student's file. 3. Determine allwable credits per semester. 6
