Welcome Board President On behalf of the Board, I would like to welcome all students, staff, parents, and interested community members to tonight s Board of Education meeting. I would like to remind everyone that this is a meeting of the Board of Education held in public for the purpose of conducting the school district s business and is not to be considered a public community meeting. There is time for public comment during the meeting in the Public Comments section on the agenda. Board members generally do not provide a response to questions during public participation. C. A copy of an unabridged Board agenda is on the table. After the meeting, citizens are welcome to review it. D. Please make sure to sign the board meeting sign-in sheet and silence your cell phones during the meeting.

2 IV. Treasurer s Business A. Minutes 1. Motion to approve the minutes of the regular meetings held August 23, 2016, as found in Attachment #1. B. Financial Transactions 1. Motion to accept the August, 2016 bank reconciliation and financial reports, as prepared and certified by the treasurer as accurate, as found in Attachment #2. Approved Not Approved Other Action 2. Motion to approve purchase orders greater than $5,000, as found in Attachment #3. 3. Motion to approve new vendors to the school district, per W-9 s attached, as found in Attachment #4. 4. Motion to adopt the Permanent Appropriations for fiscal year 2017, as found in Attachment #5. Page 2

Motion to approve a purchase order not to exceed $84,000 for services provided by the Educational Service Center of Cuyahoga County, to be paid through the ODE Innovation Grant awarded to the Cuyahoga Heights Schools, at no cost to CHS. Funds must be obligated by June 30, 2017 and paid out by September 30, 2017 [Attachment #6] 6. Motion to approve a purchase order not to exceed $25,000 to Jack Berckemeyer Consulting, to be paid through the Ohio Department of Education Innovation Grant, at no cost to CHS. Funds must be obligated by June 30, 2017 and paid out by September 30, 2017 [Attachment #7] C. Comments from Treasurer V. District Business A. Personnel 1. Motion to adjust the supplemental pay of Jory Huffman as Assistant Wrestling Coach, for his experience level, adjusting resolution # to a wage of $3, Motion to authorize the treasurer to utilize Neonet as needed, for instructing new staff on the district s accounting and payroll software, at a rate of $40 per hour. 6. Motion to approve employment of Renee Bosso as a consultant, to assist as needed with payroll processing and reports until the position s responsibilities have been transitioned, at a rate of $40 per hour. 7. Motion to approve children of Dr. Brian Friedt (Intervention Specialist) to attend the Cuyahoga Heights Schools tuition-free for the duration of the time he is working at the Cuyahoga Heights Schools, while employed by the ESC of Cuyahoga County. 8. Motion to employ Rebecca DelRosa as the evaluator and coordinator of the Alternative Education Challenge Grant for , in the amount of $7,500, paid from the Alternative Education Grant, Fund 463. B. Overnight Trips 1. Motion to approve an 8 th grade overnight trip and related contracts/agreements, to Washington D.C. on March 21-23, 2017, as found in Attachment #12. Page 5

6 B. Overnight Trips 2. Motion to approve overnight travel for teams or athletes who qualify for regional and/or state championships, if applicable for the school year, contingent upon completion of the overnight trip form upon qualification of the athletes by the date specified on the play-off schedule. C. OHSAA Site Agreements for Tournaments Motion to authorize the Superintendent or designee to sign contracts with OHSAA for the school district to be a home site for OHSAA tournaments, if selected. D. Non-Public Transportation Requests Motion to approve Non-Public Transportation Requests for , declaring transportation to those non-public schools as impractical; and therefore authorizing payment in lieu of transportation, as found in Attachment #13. E. Agreements / Contracts / Memberships 1. Motion to approve an agreement with Re-Ed Services, Inc. for transportation services for a student with special needs, who resides in the Cuyahoga Heights School District, at a per diem rate of $97, for each school day the student requires the transportation service to a Re-Education Services Program, during the school year, as found in Attachment # Motion to approve an agreement with Johnson Dance & Gymnastics, Inc. for the gymnastics season, at a cost of $6,000, as found in Attachment #15. Motion to approve a contract with Luther s Farm Market for a kindergarten field trip on September 30, 2016, as found in Attachment # Motion to approve an agreement with Cleveland Music Group for DJ services for the 2016 Homecoming Dance to be held on October 15, 2016, at Cuyahoga Heights High School, as found in Attachment # Motion to approve an agreement with the HELP Foundation, Inc. for 2016 services for the summer continuing education program (extended school year services) for a student with special needs at a cost of $1,800, as found in Attachment # Motion to approve an agreement with the ESC of Cuyahoga County for special education services provided by the Positive Education Program (PEP), for students receiving services at Day Treatment Centers during the school year, at a daily cost between $231 and $413, as found in Attachment #19.
