1 Agreed Job Description and Person Specification Job Title: Line Manager: Professionally accountable to: Job Purpose Registered Nurse Lead Nurse Inpatient Unit Clinical Director Provide specialist palliative nursing care that is patient centred, holistic and quality driven, implementing the Hospice Philosophy to ensure optimum quality of life for all patients across all clinical settings. Provide appropriate support and advice for families and carers working effectively alongside the multidisciplinary team across all clinical settings. Within your area assist in the coordination of the nursing team regarding all aspects of clinical and operational activity. Assist in managing, operating and supporting a 24/7 response by providing appropriate clinical and non-clinical advice and support within the boundaries of the service, for patients with cancer and non-cancer palliative care needs. Educate colleagues and influence practice by talking to other health care professionals and participating in education programmes and research. Clinical responsibilities: Organise patient care, assessing, planning, implementing, evaluating and documenting care in accordance with hospice philosophy and policy. Make initial and maintain ongoing assessments of the physical, emotional, spiritual, psychological and social needs of patients. Undertake the safe administration, recording, resupply and storage of drugs in accordance with hospice policy adhering to the Nursing Midwifery Council (NMC) Guidelines for the Administration of Medicines. Offer advice and support to meet the physical, emotional, spiritual, psychological and social needs of families, carers and bereaved relatives advising on all aspects of care. Enable as much patient independence as possible within the limits of disease progression. Maintain accurate and contemporaneous notes using appropriate Hospice recording systems. Assist with the timely and effective admission and discharge of patients onto the caseload ensuring good communication with the MDT and external health care agencies. Use all equipment safely, competently and appropriately taking responsibility to clean and maintain to keep in good working order. Report any defects to the senior staff and record in the dedicated maintenance book.

2 Be competent to undertake all aspects of nursing activity including pain relief, symptom control, wound dressings, mouth care & nutrition needs, moving and handling, personal care, infection control, bowel management and all aspects of drug administration including syringe pumps. When appropriate advise on the support and benefits that may be available to patients, refer to appropriate organisations. Participate in Continuing Health Care applications. To be aware of, and adhere to, hospice nursing policies and procedures and to ensure that junior and unregistered staff do so as well. Respond effectively to the patient s health needs through liaison with other health professionals, voluntary services and other agencies. In the absence of senior staff, be responsible for the safety of patients within your clinical setting and report any adverse events to the on call manager. Encourage insight amongst staff into the many cultural aspects of life and death and the implications for planning nursing care whilst promoting and valuing equality and diversity. Educational responsibilities Identify personal training and educational requirements in order to maintain an up-to-date level of professional competence. Participate in mandatory training programmes RN CPD days and appropriate internal and external training sessions. Participate in the setting of nursing standards and contribute to the clinical audit programme Attend any educational events such as significant event meetings and facilitate and encourage colleagues to do likewise. Follow good infection control practices at all times and be familiar with infection control policies, procedures and guidance. Undertaking the necessary level of training relevant to their area of work. Act as a proactive link nurse as an effective resource for your area of expertise. Professional responsibilities: To be a safe practitioner and work within the Nursing, Midwifery Council Code of Professional Conduct (NMC). To maintain and develop own knowledge and skills ensuring compliance with revalidation requirements. Maintain continued membership of a professional body indemnity scheme. Participate in the Performance Review process of yourself and others and take responsibility for achieving the objectives identified in the Personal Development Plan. Work in accordance to organisational policies and procedures. Maintain confidentiality Act as a role model for junior colleagues, embracing the ethos of hospice care Participate in induction programmes and provide effective and supportive mentorship for new/junior members of staff

3 Provide mentorship, support and encouragement for student nurses Participate in regular clinical supervision as provided by the organisation Ensure Health and Safety Polices are adhered to and report any accidents, incidents and complaints through the Hospice normal reporting chain. Liaise with all members of the multidisciplinary team in the provision of specialist palliative care for patients, families and carers. Support, attend and make an active contribution to the ward multidisciplinary team meetings. Liaise with external agencies such as the primary care team, hospital team, social services and Somerset Coordination Centre to promote seamless admissions, discharges and transfers. Liaise with specialist nurses for advice and guidance on best practice particularly for patients with complex problems Facilitate accurate and objective team handovers which are SMART (Specific, Measurable, Accurate, Realistic and Timely) Liaise with team members in a constructive and positive manner, adhering to best practice when giving and receiving feedback. Develop and maintain regular opportunity for team discussion regarding service development and decision making. Attend ward meetings on a regular basis. Promote and maintain a culture of multidisciplinary team working. Provide advice for Health Care Professionals via the 24 - hour advice line using effective telephone communication. Communicate with the Doctor on call if necessary. Provide an out of hours communication link for staff working in the community setting Establish and maintain effective communication with patients, families and carers including those with communication difficulties.
