Ermha is a community based, not- for- profit organisation delivering a range of innovative and responsive mental health and disability support services. Ermha employs staff from a variety of backgrounds to deliver services across Melbourne. Ermha has operational bases located in Clayton, Springvale, Dandenong, Beaconsfield and Geelong. VISION- A future in which all people can access a world of opportunity to thrive in their community PURPOSE- Directly providing and advocating for innovative and responsive mental health and disability services of the highest quality Position Overview TAC Support Packages delivered by Ermha 360 South East Services devise and deliver Outreach Services to people that are extremely isolated and marginalized within the community. Participants referred to the TAC program will have an ABI and/or mental illness as a direct result of a transport accident or work related incident. Often the clients referred experience accommodation breakdown, addiction, social dysfunction and moderate to severe behaviors of concern and/or inappropriate behaviors that seriously impairs their ability to: Obtain or maintain secure accommodation of an adequate standard Gain access to specialist services Gain access to mainstream or specialist general health and social support services Participate in community life due to challenging behavior, isolation and exclusion from both formal and informal supportive networks such as family and friends Maintain basic levels of self or home care 1 P age

2 Service Delivery Context: The people who will be targeted by this service will require intensive support packages tailored to: Promoting the capacity of people to maximize their control over their environment. Enable people to live optimally within a supportive community. Facilitate the development of supportive and responsive communities. Facilitate constructive links with the range of community services and resources. Key Tasks and Responsibilities Client Support To undertake and maintain a direct service caseload as determined by Program Management To offer professional support to targeted client/s within a case management framework including assessment and referral, establishment and implementation of community support and development and implementation of recovery focused case plans To provide support to promote the capacity of the client to maximize their control over their environment Facilitate constructive links with the range of community services and resources. Contribute to the delivery of services which facilitate the development and/or rehabilitation of people and their connection into the community Assist with independent living skills including meal planning, recreation and vocational opportunities. Assist with the planning and implementation of a range of structured daily and weekly programs and activities to assist with connecting back into the community. Act as an advocate and mediator with other relevant services and professionals in regard to client needs Develop case plans, prepare risk management plans, organise and facilitate with the support of the Program Team Leader complex care team meetings and provide written reports on a three monthly basis to relevant funding bodies Collaboration with others involved in the participant s life, including relevant service providers; and provision of information for advocacy and referral Completion of accurate and timely client case notes reflecting on client achievements, challenges, exceptions and actions. Attend regular client and staff meetings, program evaluations and reviews and contribute to the strategic planning process of the program To ensure that all services are delivered in accordance with relevant legislation and Ermha s operational policies and procedures, along with accurate record keeping and data collection Team Work To be a participating and respectful member of the team contributing to a harmonious workplace environment 2 P age

3 Provide a team approach to support the work, co- operate closely with team members in order to ensure continuity of care and provision of a high quality service to consumers Actively participate in reflective practice through team meetings, decision making processes, service evaluation, planning and development activities, supervision and staff activities Follow and adhere to incident reporting and quality of care guidelines and ensure that internal incident reports are completed and submitted to the relevant contacts within the stipulated time frames Assist the team to develop best practice, review and development of policies and project submissions To undertake additional duties during other staff absences OH&S Policies and Procedures Understand the importance of the Support Worker s role with regard to Ermha s OH&S policies and procedures, and OH&S hazard and incident reporting Maintain a clean and safe work environment complying with Ermha s OH&S policies and procedures. o Ensure that there are no obstructions blocking escape routes. o Report faulty or missing fire extinguishers. Ensure that safe working processes are undertaken at all times. Identify and report all hazards to your supervisor and/or OH&S Representative. Authority and Accountability Work outcomes will be collaboratively determined with, and be monitored by the Manager South East Specialist Services and the Ermha 360 TAC Team Leader. This position is accountable to the Ermha Board through the Chief Executive Officer and reports to the Ermha 360 TAC Team Leader, who will provide supervision on a fortnightly basis and as required. Autonomy will be exercised within established guidelines. The person employed in this position will make decisions regarding daily operations within Ermha s established policies and procedures. To follow directions given by line management for tasks not otherwise specified in this position description but are consistent to the program brief and role expectations. Key Selection Criteria A tertiary qualification in Social Work, Psychology, Occupational Therapy, Community Development and or relevant work experience, along with excellent written and verbal communication skills. Experience in the field of dual disability support, and/or working with those who are disadvantaged in the community with particular reference to people with complex care needs. 3 P age

4 Demonstrated experience in supporting people with multiple complex needs including psychiatric disability, mental Illness, homelessness, substance abuse, dual diagnosis, ABI, and financial disadvantages along with the ability to work with clients with challenging behaviours and being able to manage risk. An understanding of the Trauma Informed Care framework and experience in broad case management practices working from a recovery focused and person centered approach Demonstrated ability to build rapport and engage with clients at different stages on the change continuum a well as clients with multiple experiences of trauma Demonstrated ability to develop strong collaborative relationships with community agencies including health, legal, income support, drug and alcohol, employment, recreation and government based service providers Demonstrated cultural sensitivity to a variety of cultures and commitment to the principles and practices of consumer empowerment Strong written and verbal communication skills, with a demonstrated capacity for accurate file notes, case management plans, data requirements and preparation of other relevant documents Intermediate skills, knowledge and experience in using the Microsoft Suite of software programs.
