THE INNER SOUTH EAST MENTAL HEALTH ALLIANCE (ISEMHA) ISEMHA works to ensure people living with a mental illness in the inner south east of Melbourne receive appropriate clinical and psychosocial support services when and where they need them; and that communities and community services are engaged in creating a sense of connectedness and supporting opportunities for meaningful consumer participation. Our aim through this new initiative is to strengthen the capacity of the specialist service system to provide best practice treatment and support to targeted clients with severe and enduring mental illness and high level psychiatric disability The intent of ISEMHA is to: Provide clear leadership at all levels of their respective organisations to develop and promote a shared vision for a robust, cohesive and consumer centred recovery and rehabilitation service system. 2 Position Description Implement its shared vision by working in a coordinated and collaborative manner to identify common issues and solutions, and build and progress working alliances in the interests of shared consumers. Achieve sustainable outcomes by embedding the agreed protocols and functional arrangements into local work practice and providing ongoing support for the implementation of practice reforms, cost effective innovation in service system delivery and the structural changes necessary for the development and sustainability of an integrated recovery and rehabilitation mental health service system. Active and ongoing participation in the Alliance, including monitoring the success of ISEMHA arrangements and contributing to their ongoing development in response to changing needs of consumers and the evolving nature of the mental health service system. MISSION: The ISEMHA works to ensure people living with a mental illness in the inner south east of Melbourne receive appropriate clinical and psychosocial support services when and where they need them; and that communities and community services are engaged in creating a sense a connectedness and supporting opportunities for meaningful consumer participation. The ISEMHA members endorse the following principles and agree to reflect them within their own organisations and in their contribution to Alliance activities: I. Dignity to ensure the organisation s approach to recovery and care planning for individuals, sharing consumer information, program and service planning and facility design has, at its core, an enduring respect for the rights and personhood of each consumer. Similarly the organisation will ensure that its contribution to Alliance activities is reflective of this core principle. II. Respect to promote an organisational culture that is built on respect for consumers, carers and the diverse roles, approaches and contributions of different organisation. Alliance participation will be based on mutual respect for each others philosophy, priorities, knowledge, skills and experience and on learning from the diversity of its membership and collaborators. III. Leadership to develop and promote a clear and shared vision for enhanced consumer outcomes and service improvement at all levels of the organisation. This includes promoting a spirit of cooperation and collaboration and willingness to explore new solutions and change old mindsets within the organisation and between Alliance members. IV. Authority/Ownership to actively use authority to support the implementation of the required intra and inter organisational changes necessary to achieve improved consumer outcomes and the sustained development of a cohesive recovery and rehabilitation service system. Accountability for quality service provision, continuity of care and optimal recovery and rehabilitation outcomes for consumers (including shared consumers), and for maximising the use of available resources and minimising duplication of effort. VI. Sustainability by embedding change activities and processes (including coordination) within existing organisational structures and service delivery networks and models. VII. Health promotion - to demonstrate a commitment to the broader concepts of health promotion and well-being in relation to the organisation s primary target group as well as in its work in building inclusive communities. VIII. Adaptability/Flexibility to enable the integrated recovery and rehabilitation service system to respond to local needs, evolving service models, emerging evidence based practice change and changing consumer needs. (ISCHS) is a multi-sited community health agency providing a comprehensive range of health and welfare services to the communities of Prahran, St Kilda, South Melbourne and Port Melbourne. Approximately 270 staff work in multidisciplinary teams to deliver programs and services across four key service locations: SouthPort (in Coventry Street, South Melbourne); Inkerman Street, St Kilda; Mitford Street, St Kilda and Malvern Road, Prahran. In addition other staff are located in and provide services in public housing high-rise estates. For more information on our organisation please visit our website 3 POSITION DETAILS Context People suffering severe and chronic mental illness experience multiple co-morbidities that dramatically impact on their capacity to live independently and safely within the community. Our existing service system is at times too fragmented to adequately assist highly complex people. Specialist expertise in primary care, mental health, alcohol and other drug, housing support, legal support or education and training is available through agencies within our region. But given the expertise is spread across multiple agencies, a coordinated response that would stabilise the consumer and establish a pathway to a safe and meaningful life in the community can be difficult to achieve. This model of care is underpinned by key principles articulated in policy directions that promote area based planning, community based care and integrated service delivery to ensure that consumers receive the right care, in the right place, at the right time and can be safely transitioned out of clinical bed based care systems sooner. Through support from the proposed combined model of care, we will build on existing interagency initiatives to establish efficient and effective mechanisms of providing a coordinated, enhanced, and person-centred approach to caring for Complex Care Need Consumers, to establish clear and accountable mechanisms to improve the safety, quality of life, and social participation of vulnerable consumers in our catchment. This program is funded through new initiatives arising from the report Because Mental Matters. A consortium of agencies are involved through the Inner South East Mental Health Alliance. The major agencies engaged in this initiative are: Prahran Mission Alfred Health Inner South Community Health Service Mental Illness Fellowship MIND Through this new funding, a team of 6 people will form to deliver integrated services including: A Program and Panel Manager (Alfred Health) A Clinical Specialist (Alfred Health) 2 Advanced Mental Health Practitioners (one ISCHS and one Prahran Mission) 2 Care Coordinators (one ISCHS and one Prahran Mission) Inner South Community Health Service Page 4 of 11

5 Key Features of the Role The Advanced Mental Health Practitioner is responsible for implementing a whole of person approach to assessment, person-centered planning and intervention. The role will combine direct service delivery with responsibility for the coordination of care of clients through: o providing a single point of contact and a single care plan for the client o promoting access to a range of clinical and non-clinical services o coordinating the clinical and community supports responsibilities of each agency o reviewing the overall progress and monitoring outcomes. Specific Responsibilities of the Advanced Mental Health Practitioner 1. Take a Recovery approach to all planning and work with clients. 2. Provide expert mental health/housing/aod advice and assistance in decision making regarding the provision of treatment and support to clients. 3. Act as a resource to clinical and other staff on supports/services available in the community. 4. With consent of client, and in collaboration with other key providers, undertake all necessary care and support planning (including treatment, social and community care) and make recommendations for funded supports 5. Work with the client to facilitate self advocacy where possible, and where necessary, advocate on behalf of the client for access to services which best meet their needs; 6. Coordinate and facilitate all aspects of support planning, case conferencing and review, ensure congruence between the ISP and the service delivery. 7. Using current evidence based interventions, implement an individual s support plan. 8. Lead the development, implementation and monitoring of a Crisis Prevention and Management Plan for each client and ensure that services are delivered to the client in a way that maintains the person s dignity and rights. 9. Actively foster the client s engagement in support, planning and review activities. 10. Maintain the Client File incorporating (and identifying) all ISEMHA Care Coordination documents. 11. Ensure the agreed outcome measurement tools are used fostering client engagement and ownership and appropriate levels of involvement from other service providers. Facilitate all necessary communication in relation to the client ensuring both consent and privacy matters are respectfully addressed and support people identified by the client including providers and carers are appropriately engaged. 13. Identify emerging issues and act to resolve them at the soonest possible time in a transparent manner. Authority and Relationships 14. Contribute to the development and maintenance of effective relationships with other providers engaged in delivering services to the client. 15. Liaise with other providers of services, providing direction where necessary. 16. Liaise with family members/key decision makers chosen by the client; 17. The Advanced Mental Health Practitioner is responsible for decision-making in relation to developing and implementing Individual Support Plans with clients within the program. Where there is a difference of view between providers it is expected the Advanced Mental Health Practitioner will carefully consider all views (including that of the client) and evidence as part of his or her decisionmaking process. Skills and Experience Required 1. An experienced mental health practitioner with demonstrated experience of working effectively with mental health consumers with multiple needs 2. Demonstrate a high level understanding of the challenges faced by consumers in developing and maintaining safe and satisfying lives in the community 3. Understanding of clinical and/or PDRS sectors. Knowledge of housing and/or drug and alcohol sectors an advantage. 4. Experienced in the planning and delivery of services in the community 5. Extensive knowledge of the community and healthcare sectors 6. Capacity to resolve problems and conflict constructively 7. Ability to effectively manage formal contract arrangements whilst fostering collaboration. Professional Development To attend and contribute to staff development and supervision programs, and to attend relevant courses and mandatory in-house training when appropriate. 7 Occupational Health & Safety Position Description Ensure that work and services are provided in a safe manner at all times by regularly reviewing practices and environment and by participating in OH&S training where required. ISCHS has a Smoking Free Workplace Policy which prohibits smoking within all all managed or controlled buildings, vehicles and grounds. CONTRIBUTING TO STRATEGIC ORGANISATIONAL INITIATIVES 1) Continuous Quality Improvement Identify continuous quality improvement opportunities Participate in the development of quality procedures and contribute to internal and external program reviews as required. 2) Community Participation To undertake active and meaningful community participation and engagement 3) Partners for Better Health Participate in the development, implementation and evaluation of chronic conditions care at ISCHS 4) Health Promotion Contribute to the development, implementation and evaluation of organisational Health Promotion strategies as identified in the Health Promotion Plan; and undertake health promotion training as required. Performance Management Framework Actively participate in an annual individual performance plan, which determines key responsibilities, strategies and performance indicates in line with the organisation s Strategic Plan and Program Business Plan. Intellectual Property Policy ISCHS has an Intellectual Property Policy with which staff are required to understand and comply Administrative To attend all relevant Centre meetings. 8 To accurately complete timesheets and statistics on a routine basis. General To participate in the ongoing planning, evaluation and development of the Primary and Mental Health Program. To adhere to ISCHS policies and procedures. To participate in quality assurance activities as required. To receive appropriate training about the Needle & Syringe Program (NSP) and participate in the NSP as required. To undertake active and meaningful community participation and engagement Other duties as required by the Manager of the Program. Key Selection Criteria: Must be submitted with your Application 1. Qualification in nursing, allied health or social work or other relevant disciplines 2. Extensive understanding of the philosophy and provision of recovery focused services to persons with psychiatric disability or functional limitation as a result of serious mental illness. 3. Demonstrated commitment to the principles and practice of consumer empowerment 4. Demonstrated knowledge and ability to collaborate with consumers and other relevant individuals and agencies across the community, acute and sub-acute services to achieve planned outcomes 5. Demonstrated ability to use a client centered approach to engage and support clients with multiple needs to achieve whole of person outcomes. 6. Ability to work effectively under limited supervision. 7. Highly developed interpersonal, communication and negotiation skills. 8. A high level of skill in the use of Microsoft Office Programs. 9. A current Victorian Driver s Licence Desirable Knowledge of the Inner South Region and its networks. Experience in working with, and sensitivity to, a variety of cultures, including the use of interpreters. Ability to speak a language relevant to the Inner South community. 3.4 REPORTING AND SUPERVISION RELATIONSHIPS The Advanced Mental Health Practitioner will report to the Program and Panel Inner South Community Health Service Page 8 of 11

9 Manager. Supervision will be provided by a Mental Health coordinator within the respective employing agency. 5. CONDITIONS OF EMPLOYMENT Award Based on relevant Psychiatric Nurses Award, Health Professionals Award or Social & Community Services Award (SACS) or other as negotiated. Salary Level Dependant on qualifications and experience. Salary range $65,000 to $73, Superannuation and salary packaging Salary packaging For further information please refer to Salary Packaging information below Hours per week Full time To meet Client, Program and Service needs, some out-of-hours and/or weekend work may be required. Any time worked in excess of the agreed hours per week is expected to be negotiated with the coordinator and taken as time-in-lieu. Duration Contract Position: to Nov 30 th 2013, Full Time. Probationary Period Location Accountability In accordance with our usual practice, there will be a probationary period of three months, during which time a review process will be undertaken. At the expiration of the probationary period, employment may be confirmed as an ongoing employee, employment may be terminated or the probationary period may be extended up to a maximum of 6 months with further review. During the probationary period either party can terminate for any reason. The team base for this initiative is expected to be Alma Rd, St Kilda. Staff will also have a worksite at their employer base. Salary Packaging Information: Salary packaging is a legitimate method of restructuring existing salary into a combination of salary and expense payment benefits in order to provide a higher net remuneration for the employee. This offer applies while Inner South Community Health Service continues to be granted "public benevolent institution" status and enjoys exemption from the provisions of the Fringe Benefits Tax (FBT). Should our status change salary packaging will no longer be available. We offer 3 types of Salary Packaging:- o Standard tax free salary packaging amount $16,050 [per annum o Meal Entertainment Expenses $12,000 per annum o Entertainment Facility Leasing Expenses - no threshold per annum It is not compulsory for employees to take up the offer to package their salary; however the benefits from packaging are considerable. There is a small cost to the employee. Salary packaging is not available for casual or for locum employees who are on a contract less than 3 months in duration. 6.
