2 Role Statement Position Title and Number Purpose Program Consultant Carer Peer Support (2197) The Consultant Carer Peer Support works in partnership with the Carer Support Team, Customer Service Team and external stakeholders to deliver flexible support services that meet the individual needs of carers and their families, including: Coordinating and facilitating peer support groups and activities; Providing education, information and building capacity Networking with external organisations to provide information to carers in the wider community. Mental Health Respite Carers Support Options (MHRCSO) The objective of the Program is to assist carers of people with mental illness to sustain their caring roles and maintain connection with their communities, by increasing access to flexible, innovative carer support services. Carer Support is short term one-one work with a Carer that assists the carer to achieve balance between their caring role and other aspects of their life, with a focus on carer wellbeing. My Clients Carers and families of people with severe & persistent mental illness (a diagnosable disorder that significantly interferes with an individual s cognitive emotional or social abilities). In this region, individuals with a diagnosis of Autism are only eligible where there are no other services available to them. The program focuses on carers of those with a mental illness in the following target groups: Indigenous carers; Carers with cultural and linguistically diverse backgrounds; Older parent carers; Young carers; Lesbian, gay, bisexual, transgender and intersex carers; Regions serviced include the Southern Metropolitan Region (SMR) - Greater Dandenong, Casey, Cardinia. My Stakeholders Reports To External: Carers, Care recipients, Department of Social services (DSS), service providers and organisations relevant to families, carers and mental illness. Internal: All lifeassist including Work Colleagues, Supervisor and Executive Manager, Finance, Quality, ICT, Customer Service Team, People & Quality Team. 3 Responsible for Working as part of a team to achieve the overall program goal of supporting primary carers and individuals identified as secondary carers in the family or caring relationship annually. Responsibilities Delegations Received 1. Comply with the lifeassist Delegations Policy. 2. Assist with the delivery of the Strategy Implementation Plan (Reference to Strategy Implementation Plan Obligations for responsible strategies) and Carer Support Disability Team Plan. My Responsibilities 1. Manage the day-to-day affairs of the responsibility areas in accordance with policies and procedures including. 1.1 Develop and Coordinate Peer Support Program. Coordinate administrative functions of the carer peer support groups including program content, engaging providers or guest speakers, registration records, minutes and undertaking evaluation. Maintain accurate and up to date case notes on the client information system. Document, monitor, and review peer support needs and budget according to specific internal program guidelines. Provide monthly report to program manager regarding support group attendance; achievements; any operational issues and broader service sector gaps. In consultation with the Program Manager and the team, coordinate and facilitate regular opportunities to raise awareness and educate about mental health and carer issues; promote the program and identify eligible carers to both relevant external organisations and Meet compliance with funding and organisational requirements. 1.2 Coordinate and administer Carer Peer Initiatives, Events / Activities. Design, plan and implement peer program initiatives including educational opportunities for carers relevant to mental health. Organise cost effective resources according to budget. Promotion and Advertising. Registrations. Plan, facilitate and evaluate activities and events. Report and make recommendations regarding activities and events to relevant 1.3 Using lived experience, apply a strengths based, person-centred approach to provide Carer Peer Support including: Meet with carers as part of peer support program activities and / or individually. Support carers to explore issues and formulate strategies to sustain them in their current role. Foster development of effective communication strategies and selfcare skills. 4 Provide information about mental health, caring for a loved one and available services. Encourage the carer to access appropriate carer supports including the LifeAssist MHRSCO Carer Support Program where applicable. 2. Deliver high quality service and positive outcomes for my clients and 3. Develop and maintain effective collaborative relationships with my clients and all stakeholders with a focus on integrity, respect and accountability. 4. Implement best practice principles in my work through professional and personal development, including keeping abreast of legislative and community sector changes. 5. Capabilities Knowledge Experience Skills Experience A lived experience of caring for an individual living with a mental illness is an essential requirement of this role. Experience in coordinating and facilitating effective support groups. Experience in one or more of the following target groups: Indigenous carers. Carers with cultural and linguistically diverse backgrounds. Older parent carers. Young carers. Lesbian, gay, bisexual, transgender and intersex carers. Experience in working with young people in schools and / or community services. Demonstrated experience working with external service organisations in the community sector. Knowledge Demonstrated high level of understanding and knowledge of: The relevant issues facing carers, families and professionals for whom they care. The issues relevant to families and professionals caring for those with complex needs. Relevant health and community based service support systems in the Dandenong, Casey and Cardinia region of Melbourne. Current trends, practices, policies and legislation in relation to supporting carers of people with mental illness; in their communities.
