Purpse The purpse f general practitiners and bstetrician Shared Maternity Care Affiliate accreditatin is t: Recgnise the cmmunity-based care prvider f maternity care as an integral member f the health care team Identify health care prviders wh are apprpriately qualified and meet agreed standards in the prvisin f care t mutual patients Outline the criteria and prcess t achieve and maintain accreditatin as a shared maternity care affiliatin as a general practitiner and bstetrician Clarify care and cmmunicatin standards and facilitate cmmunicatin with affiliates Supprt the quality prvisin f shared maternity care, cntinuing prfessinal develpment and quality care imprvements in shared maternity care. 2. Definitins General Practitiner Liaisn Officer and Unit (GPLO and GPLU): prvides a link between the hspital and GPs, and encurages and imprves cmmunicatin and care planning, cperatin, efficiency and quality f care between hspitals and general practice in relatin t patients and t services prvided by the hspital (Lippmann L, 2006). Shared Maternity Care Affiliates (SMCA): a cmmunity based General Practitiner, Obstetrician r Midwife wh is accredited by the hspital t prvide shared maternity care in partnership with the hspital. Shared Maternity Care: the jint prvisin f maternity care by an accredited cmmunity-based Shared Maternity Care Affiliate and hspital staff. The birth and immediate pstnatal care are managed by the hspital. Shared Maternity Care is a maternity care ptin fr wmen attending The Ryal Wmen's Hspital. Suitability fr shared maternity care is addressed in the hspitals Guidelines fr Shared Maternity Care Affiliates. 3. Respnsibilities Shared Maternity Care Affiliate/ The Ryal Wmen's Hspital staff: T prvide high quality care T dcument care n the patient s hand held recrd T undertake apprpriate and timely cmmunicatin and referral between prviders. Respnsibilities f the accredited Shared Maternity Care Affiliate: T agree and abide by the undertakings as per 5.4. Respnsibilities f The Ryal Wmen's Hspital: T prvide apprpriate guidelines t supprt high quality care, cmmunicatin and referral T prvide apprpriate cntinuing prfessinal develpment t supprt shared maternity care T undertake quality assurance prgrams and service develpment. Guideline 4.1 General Practitiners and initial accreditatin Accreditatin as a Shared Maternity Care Affiliate: Is applicable t an individual, nt a practice Is given fr a maximum f 4 years Accreditatin is btained until 31/12/2016 There is jint accreditatin criteria and a jint prcess between The Ryal Wmen's Hspital, Mercy Public Hspitals Incrprated, Western Health and Nrthern Health. Criteria fr accreditatin Fr successful accreditatin as a Shared Maternity Care Affiliate, all the belw criteria headings must be met. Prfessinal requirements MBBS r equivalent; and Current Unrestricted Medical Registratin in Victria; and Current Medical Indemnity/Insurance membership; and In the event f cnditinal registratin: this may be acceptable at the discretin f the Head f the General Practice Liaisn Unit after establishing the cnditins with Australian Health Practitiner Regulatin Agency and liaising with the Clinical Directr f Maternity Services. Practice requirements Fr General Practitiners initially accredited after 1 st January 2014, shared maternity care affiliatin is restricted t the prvisin f shared maternity care frm general practices that meet current Practice Accreditatin against the Ryal Australian Cllege f General practitiners Standards fr General Practice (e.g. by Australian General Practice Accreditatin Limited r General Practice Australia). At initial accreditatin, the general practitiner seeking accreditatin will be required t frward dcumentatin t this effect. Prfessinal referees The applicant is required t prvide the hspital with the cntact details f tw prfessinal medical referees, ne f whm is a current Shared Maternity Care Affiliate r bstetrician at the Ryal Wmen's Hspital, Mercy Public Hspitals Incrprated, Western Health r Nrthern Health r bstetrician affiliated with anther public hspital. If these are nt available, ther medical referees may be acceptable at the discretin f the Head f the General Practice Liaisn Unit. Further qualificatins and experience One f the fllwing must be fulfilled: Fr : Attainment f Fellwship f the Ryal Australian and New Zealand Cllege f and Gynaeclgists (FRANZCOG) r Fellwship f the Ryal Australian Cllege f and Gynaeclgists (FRACOG). Fr General Practitiners: Attainment f Diplma f the Ryal Australian and New Zealand Cllege f Obstetrics and Gynaeclgy (DRANZCOG) r Certificate Satisfactry Cmpetitin Training (CSCT) in Wmen s Health frm RANZCOG in the previus 5 years; OR Attainment f DRANZCOG r Diplma Ryal Australian Cllege f and Gynaeclgists (DRACOG) /r Certificate Satisfactry Cmpetitin Training (CSCT) in Wmen s Health r equivalent Uncntrlled dcument when printed Publicatin date (24/06/2014) Page 2 f 7

3 mre than 5 years ag PLUS invlvement in the prvisin f shared maternity care within the previus 5 years; OR Significant Experience as an Antenatal Care Prvider; This is decided n an individual basis and must be agreed upn by the designated registered medical practitiners, at the accrediting hspitals (i.e. The Ryal Wmen's Hspital, Mercy Public Hspitals Incrprated, Western Health and Nrthern Health). At the Ryal Wmen's Hspital, this is the Head f the GP Liaisn Unit; OR Successful cmpletin f Hspital Antenatal Clinic Attendances and ther training. GPs wh d nt meet the abve pstgraduate/experience requirements may apply fr accreditatin after undertaking training determined by the assessing medical practitiner at ne f the fur hspitals. This may cnsist f: Satisfactry cmpletin f pre-attendance questins based n the hspital guidelines fr shared maternity care affiliates (guidelines are available n the hspital websites) Other training (e.g. The Ryal Australian Cllege f General Practitiners CHECK independent learning prgram fr GPs r ther activities, attendance at wrkshps etc) Attendance at antenatal clinics at ne f the hspitals, with at least ne f these sessins undertaken at a primary hspital site Undertakings T agree t: Satisfactry cmpletin f pst-placement and activity questinnaires. Review the hspitals guidelines fr shared maternity care affiliates Observe hspital guidelines in respect f mutual patients, including criteria fr hspital review/referral and sharing investigatin results and management Prvide a readily available cpy f the accreditatin frm at the practice fr administratin and ther staff Participate in apprpriate cntinuing prfessinal develpment fr the prvisin f shared maternity care Ensure the hspitals have up t date preferred cntact infrmatin (telephne, facsimile, pstal address) Ensure the facsimile number given applies t a machine that is in a private lcatin and prcedures fr handling patient infrmatin cmply with privacy principles and legislatin Maintain Medical Registratin as current and withut cnditins and t ntify the hspitals if registratin is suspended, cancelled r has restrictins impsed Maintain Medical Indemnity Insurance at an adequate level f cver fr the duratin f participatin in shared maternity care Keep apprpriate clinical recrds and dcument care in the patient handheld recrd (e.g. Victrian Maternity Recrd) Make apprpriate arrangements fr cntinuing care with an accredited Shared Maternity Care Affiliate r the hspital where the wman is bked fr birth when n leave r ill Acknwledge the hspitals cnduct research activities and quality assurance prgrams and that Shared Maternity Care Affiliate r patient participatin may be requested Authrise the hspitals and their General Practice Liaisn Units/Shared Care teams t discuss details f prvisin f shared maternity care, bth within the hspitals and between hspitals Authrise the hspitals t exchange details abut accreditatin, including cntact details Authrise the hspitals t publicly publish and prvide wmen and their families with practice details, and languages spken Uncntrlled dcument when printed Publicatin date (24/06/2014) Page 3 f 7

4 Nt t prvide intrapartum care fr wmen wh are bked fr maternity care r wh undertake shared maternity care with the hspitals Understand that Shared Maternity Care Affiliates fund nt t be adhering t guidelines and acceptable standards f quality f care may have their accreditatin status reviewed and revked Cnfirm that shared maternity care is undertaken whilst wrking frm a practice accredited against the RACGP Standards fr general practice, and t infrm the hspitals if this is n lnger the case^. ^Fr GPs wh first accredit as Shared Maternity Care Affiliates after 1 January Prcess fr accreditatin There is a jint accreditatin criteria and prcess fr applicatins fr The Ryal Wmen s Hspital, Mercy Public Hspitals Incrprated, Western Health and The Nrthern Health, whereby the applicant indicates which (up t all fur hspitals) he/she wants t apply t. Applicatin frms can be btained via the Shared Maternity Care Crdinatr at any f the fur hspitals r via the hspital websites. Applicatins can be submitted t the Shared Maternity Care Crdinatr at any f these sites. The applicatin will be prcessed at the hspital site receiving the applicatin. Prcessing will include: Checking all requirements are met (refer t sectin 3.1 Initial Accreditatin), and Cntacting at least ne suitable referee t determine suitability f the applicant fr shared maternity care affiliatin and nting this n the applicatin. At the Ryal Wmen s hspitals, this will be undertaken by the Head f the GP Liaisn Unit, Dr Ines Ri. If a recmmendatin fr apprval is made, the applicatin will be frwarded t the Clinical Directr f Maternity Services, A/Prf Mark Umstad fr ultimate decisin. If it is deemed the applicatin des nt fulfil the required criteria, the applicant will be infrmed. If an applicant is successful, dcumentatin will be kept at the hspital site apprving the applicatin. Other hspital sites that the applicant has als been apprved fr will be ntified f the utcme and fr will have access t that infrmatin as requested. At The Ryal Wmen's Hspital, the Clinical Directr f Maternity Services, A/Prf Mark Umstad, is respnsible fr accreditatin f general practitiners and bstetricians as shared maternity care affiliates. Appeals may be made t the Executive Directr Clinical, Dr Christine Bessell. 4.2 General Practitiners and reaccreditatin Accreditatin as a shared maternity care affiliate is fr a maximum f 4 years, and there are criteria and prcesses fr reaccreditatin each triennium. These criteria and prcesses differ frm initial accreditatin. Alternative activities equivalent t ten Grup 2 RACGP pints may be applied fr - this is decided n an individual basis by the registered medical practitiner at ne f the fur hspitals. All GP and bstetrician shared maternity care affiliates will be psted an applicatin frm t their nminated address. If they d nt return a cmpleted frm that fulfils the reaccreditatin criteria, they will receive at least ne reminder letter by pst. If they have nt returned an applicatin that fulfils the reaccreditatin criteria by 31 st December f the triennium, they will receive a letter that their shared maternity care affiliatin with the hspitals will be ceased and their name is t be remved frm the shared maternity care affiliate lists f all fur hspitals. Shared maternity care affiliates wh have nt fulfilled requirements by 1 st March the fllwing year will then lse their accreditatin as a shared maternity care affiliate. Individual GPs wh are awaiting the fulfilment f specific criteria may have this extended by the Head f the GP Liaisn Unit fr a limited time, at the discretin f the Head f the GP Liaisn Unit. Once shared maternity care affiliate affiliatin has ceased, applicants will need t apply fr accreditatin (refer t sectin 3.1 Initial Accreditatin). Applicatins will need t be submitted at the hspital designated fr the receipt f reaccreditatin applicatins, but may be prcessed at any f the fur hspital sites. Prcessing will include checking all requirements are met by the designated medical practitiner f the hspital (refer t sectin 3.2 Reaccreditatin). At the Ryal Wmen s hspitals, this will be undertaken by the Head f the GP Liaisn Unit, Dr Ines Ri, wh is then respnsible fr apprving r declining the applicatin, with the applicant being infrmed f the utcme. If an applicant is successful, dcumentatin will be kept at the hspital site apprving the applicatin. Other hspital sites that the applicant has als been apprved fr will be ntified f the utcme and fr will have access t that infrmatin as requested. Appeals may be made t the Clinical Directr f Maternity Services, A/Prf Mark Umstad. 5. Evaluatin, mnitring and reprting f cmpliance t this guideline At the time f reaccreditatin a survey is psted t shared maternity care affiliates that cvers the prcess and criteria f reaccreditatin, reasns why shared maternity care affialiates d nt seek reaccreditatin. Every three years medical practitiners frm the 4 General Practice Liaisn Units, the Obstetric heads f the fur hspitals and GP representatives cme tgether t review and decide n accreditatin and reaccreditatin criteria fr the next triennium. 7 The medical practitiners frm the fur hspitals meet at least twice a year, as the Shared Maternity Care Cllabrative with standing agenda items f accreditatin and reaccreditatin f shared maternity care affiliate GPs and bstetricians. 6.
