The medical center ffers extensive inpatient and utpatient services including a 120-bed skilled nursing and rehabilitatin center, fur primary care physician practices (Niagara Falls, Nrth Tnawanda, Grand Island and Wheatfield), a suburban utpatient service center (Summit Healthplex/Wheatfield) with labratry, radilgy, rehabilitatin, sprts medicine and specialty physician services, and a health clinic fr members f the nearby Tuscarra Indian Reservatin. Memrial ffers several services unique t Niagara Cunty including a diabetes and endcrinlgy center, inpatient adult behaviral health unit (54 beds), dedicated psychiatric emergency department and the Child Advcacy Center f Niagara, a multi-disciplinary prgram respnding t reprts f physical and sexual child abuse. Prject Runway. Funded by the Peter and Elizabeth C. Twer Fundatin, Prject Runway aims t increase cmmunity awareness f the issues surrunding substance use and abuse by girls and yung wmen and t train healthcare prfessinals, including physicians, t effectively evaluate and treat thse wh use alchl and drugs. The wrk plan fr this prgram is based n input received at public sessins held in three different Niagara Falls neighbrhds. Niagara Falls Memrial Medical Center has been named a prvider fr NY State f Health, the state's In-Persn Assistr and Navigatr (IPA/Navigatr) Prgram t help peple shp fr and enrll in health insurance cverage. In that capacity, Memrial has cncluded memranda f understanding with numerus cmmunity rganizatins and prvides IPA/Navigatr Assistance at 10 sites acrss Niagara Cunty. 3 Definitin and Descriptin f Cmmunity Served Niagara Falls Memrial defines its service area as western Niagara Cunty and nrthern Erie Cunty. The zip cdes included in this area are 14072, 14301, 14303, 14304, 14305, 14092, 14107, 14109, 14120, 14131, 14132, 14144, and The service area is a study in cntrasts. Niagara Falls (zips 14301, 14303, 14304, 14305), with an estimated 49,722 residents, has a median husehld incme f $32,617 with 21.7% f its residents living belw the pverty level (Surce: U.S. Census Bureau). Mre than 50 percent f Niagara Falls residents receive sme frm f gvernment assistance and 66 percent f the city schl district's ppulatin qualified fr either free r reduced-price meals during the schl year. Black/African American and Latin/Hispanic residents accunt fr 24.6 percent f the city s ppulatin. The next largest city, Nrth Tnawanda (zip 14120) has 31,269 residents and a median husehld incme f $46,203, with 10.5% f its residents living belw the pverty level. Mre than 96 percent f the city residents are white. The surrunding twns f Niagara, Wheatfield, Grand Island, Lewistn, Prter and Wilsn are predminantly suburban and rural with a significant agricultural presence. Public Participatin Public participatin in the Cmmunity Service Plan prcess was perfrmed cllabratively by a wrk grup cmprising the Niagara Cunty Health Department, Niagara Falls Memrial Medical Center, Munt St. Mary s Hspital & Health Center, DeGraff Memrial Hspital/Kaleida Health, Eastern Niagara Health System, Niagara Cunty Department f Mental Health and University at Buffal Preventive Medicine Residency Prgram. Thse effrts were facilitated by the P 2 Cllabrative f Western New Yrk, a nt-fr-prfit rganizatin that wrks with individuals and rganizatins in Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans and Wyming cunties t imprve the health f Western New Yrkers. The wrk grup met seven times in an eight mnth perid (March 7, April 9, May 16, July 2, Aug. 27, Sept. 11 and Oct. 3). Early in the prcess the wrk grup cnducted a cuntywide Cmmunity Health Survey that was widely publicized thrugh the use f news releases, website pstings and in-persn cmmunity distributin. Survey respnses were gathered bth nline and n paper (hard cpy) with 1,455 surveys cmpleted and submitted. The five mst ften identified issues f cncern and the assciated Preventin Agenda fcus areas were: Cancer (50.24%) - Prevent Chrnic Disease Heart-Related Issues (37.23%) - Prevent Chrnic Disease Overweight/Obesity (31.93%) - Prevent Chrnic Disease Alchl/Drugs (27.94%) - Prmte Mental Health/Prevent Substance Abuse Nutritin/Healthy Diet (27.32%) - Prevent Chrnic Disease/Healthy & Safe Envirnment. 4 Fcus grup participants at the Jhn Duke Senir Center in Niagara Falls identified the mst significant health issues they face as: Obesity and the way it s cnnected t ther health issues fr bth adults and children Heart disease Diabetes (especially when uncntrlled in senirs) Mental health (particularly dementia) Arthritis/jint prblems Cancer Dental care and eye care These fall under the Preventin Agenda fcus areas Prevent Chrnic Disease, Prmte a Healthy and Safe Envirnment, Prmte Mental Health and Prevent Substance Abuse. The cuntywide wrk grup hsted a cmmunity stakehlder meeting (Aug. 14) that was attended by representatives frm Opprtunities Unlimited f Niagara (a nt-fr-prfit rganizatin prviding prgrams and services t peple with mental retardatin r develpmental disabilities), Niagara Cunty Cmmunity Cllege (a tw-year cllege with mre than 7,000 enrlled students) and The Dale Assciatin (a nt-fr-prfit human service rganizatin with a cmprehensive list f prgrams and services fr adults f all ages) alng with the cunty s hspitals, Health Department and P 2 Cllabrative. Disparity: Peple with mental health issues In cnnectin with this shared pririty, Niagara Falls Memrial Medical Center has chsen as its bjective: By December 31, 2017, increase the percentage f adult health hme members diagnsed with bth schizphrenia and diabetes whse bld glucse is in gd cntrl (hemglbin A1C less than 8%) by 20%. 5 Ratinale: The rate f hspitalizatins fr shrt-term cmplicatins f diabetes (ages 18+ years fr the years ) is 9.2 per 10,000 in Niagara Cunty cmpared t 5.6 per 10,000 in New Yrk State. The New Yrk State 2017 bjective is 4.8 per 10,000. The Niagara Cunty Health Hme has a grwing number f clients frm acrss Niagara Cunty wh have bth an acute care (medical) and a mental health diagnsis. A significant percentage f thse clients have been diagnsed with schizphrenia. Peple with schizphrenia are at a greater risk f metablic syndrme due t their serius mental illness. Diabetes screening is imprtant fr anyne with schizphrenia r biplar disrder, and the added risk assciated with antipsychtic medicatins cntributes t the need t screen peple with schizphrenia fr diabetes. (Surce: Agency fr Healthcare Research (AHRQ) / Natinal Cmmittee fr Quality Assurance (NCQA). On August 27, the wrk grup reached a general cnsensus that Prmte a Healthy and Safe Envirnment wuld be a gd fit fr the secnd shared pririty area. Mst members, including Niagara Falls Memrial Medical Center, agreed that wrking tward preventing falls amng frail elders wuld be the best way t apprach this fcus area. Accrdingly, Niagara Falls Memrial will participate in the fllwing shared pririty: Pririty: Prmte a Healthy and Safe Envirnment Fcus Area: Injuries, Vilence and Occupatinal Health Gal: Reduce fall risks amng vulnerable ppulatins In cnnectin with this shared pririty, Niagara Falls Memrial Medical Center has chsen as its shared bjective: By December 31, 2017, reduce the rate f fall-related hspitalizatins in the ppulatin aged 65+ by 10 percent t achieve a Niagara Cunty rate f per 10,000 residents. The current rate is per 10,000 residents. Ratinale Falls in lder adults are the leading cause f injury related deaths, hspitalizatins and emergency department visits. Falls may have serius cnsequences affecting mbility, mental health and independence. Every day in New Yrk State tw lder adults die and 140 lder adults are hspitalized due t falls. 60% f adults wh are hspitalized due t a fall end up in a nursing hme r rehabilitatin center. 27% f thse wh fall suffer a hip fracture. Falls accunt fr apprximately $1.7 billin in annual hspitalizatin cst and $145 millin in utpatient csts in New Yrk State. (Surce: New Yrk State Department f Health CHAI) In calendar year 2012, there were 128 inpatient admissins t Niagara Falls Memrial Medical Center that were directly attributed t falls. During the same perid, 406 patients presented t the hspital s emergency department with a fall. Of thse, 144 (35.5%) were age 65 r lder. DeGraff Memrial Hspital and the Niagara Cunty Health Department are using the NYSDOH Stay Well prgram, an evidence-based initiative, t prevent falls. The Health Fundatin f Western and Central New Yrk has expressed interest in supprting this spread f falls preventin initiatives thrughut Niagara Cunty, and a multi-stakehlder wrk grup wuld be a strng applicant t seek this funding. Pririty 1: Prevent Chrnic Disease Increase access t high-quality chrnic disease preventive care and management in clinical and cmmunity settings.
