Sme wrk frm ther sites may be required Branch / Team: Allied Health and Cmmunity Services Reprts t: Direct Reprts: Team Leader Nne Effective date: January 2016 Psitin Purpse The purpse f the Hume Schl Fcused Yuth Service is t ensure that at risk yung peple (aged 10 t 18 years) are well supprted in their cmmunities, that risk factrs are addressed r mitigated and that they cntinue t be engaged and cnnected t educatin. This will be achieved by: Crdinating the prvisin f cmmunity based services in partnership with schls and cmmunity agencies t supprt schls t address the needs f at risk yung peple Fstering an integrated service delivery mdel thrugh develping shared prtcls and systems Enhancing existing partnerships and creating new partnerships and pathways between schls and cmmunity rganisatins t supprt at risk yung peple T assist all schls t this endeavr including Gvernment, Independent and Cathlic schls in their supprt fr at risk yung peple, and in particular, by wrking clsely with and cmplementing the Student Supprt Services. Achievement f perfrmance, financial and individual Key Perfrmance Indicatrs as utlined in Perfrmance Appraisal prcess Duties and Respnsibilities Hume Schl Fcus Yuth Service: Plan, implement, review and evaluate the Hume SFYS wrk plan and cllect data as apprpriate. Adhere t the SFYS prgram guidelines. Crdinate care t vulnerable yung peple and their families by fstering an integrated service mdel between schls and lcal service prviders. Chair and crdinate the SFYS Gvernance and Advisry Grup and t ensure active membership by key stakehlders such as SSS Netwrk Executive Chairs and C-rdinatrs, Cathlic Educatin and Independent Schls representatins, Hume Whittlesea Lcal Learning and Emplyment Netwrk (HWLLEN), Hume Cuncil, Mental Health, Child First. Identify the psychscial needs and emerging issues f the cmmunity and f individuals and identify at risk/pririty grups and wrk pr-actively with schls and cmmunity agencies t address the needs and reduce risk. Ensure the Hume SFYS is respnsive t lcal issues, emerging issues and trends by accessing and utilising lcal and brader data as well as utilising utcmes/measuring tls such as the Student Online Case System (SOCS), the Australian Early Develpment Index (AEDI), and the Victrian Child and Adlescent Mnitring System (VCAMS). 2 Psitin Descriptin Wrk in partnership with service prviders, cmmunity agencies and grups and health prfessinals within and external t Dianella t develp implement and evaluate ther integrated health prmtin prgrams. Utilise a range f health prmtin interventins and be prepared t wrk in settings utside the centre t prmte scial and emtinal wellbeing. T crdinate the use f flexible funding as per the SFYS wrk plan and ensure funds are accunted fr by wrking cllabratively with Dianella Accunts Payable/Finance. Prfessinal Develpment: Participate in individual and grup supervisin. Plan, implement and evaluate a prfessinal develpment wrk plan. Utilise and cntribute t the evidence based research n at risk yuth and disseminate this infrmatin accrdingly in apprpriate internal and external frums. Cntribute twards scial wrk fieldwrk educatin by maintaining links with prfessinal bdies and educatinal institutins, and thrugh the supprt f undergraduate student placements. Ensure a high quality scial wrk service by updating wn prfessinal skills and knwledge thrugh attendance at apprpriate curses/wrkshps/seminars. General respnsibilities Maintain Duty f Care. Maintain cnfidentiality and client privacy. Assist in the planning, develpment, implementatin and evaluatin f prgrams that respnd t the needs f the cmmunity. Perfrm duties within the scpe f practice f the rle and the standards and guidelines set ut by the funding and accreditatin bdies. Undertake apprpriate activities as directed by management t ensure the quality and safety f services. This includes participating in quality develpment activities, clinical audits, cnduct f client satisfactin surveys, and prgram evaluatin as relevant. Supervise vlunteers r students as required. Participate in the annual perfrmance appraisal prcess. Prmte distribute and share knwledge and infrmatin within the rganisatin and relevant parties. Develp and maintain wrking relatinships with apprpriate lcal and reginal service prviders. Participate in the develpment and maintenance f effective team prcesses. Prvide a best practice mdel f service delivery. Be prfessinally accuntable. Identify wrk prcesses that culd be imprved and present pssible strategies fr service. Additinal Infrmatin Travel between sites may be required including utreach wrk within the Dianella catchment. Occasinal after hurs and weekend wrk may be required. Minimum f 2 years field experience. Experience wrking as a Scial Wrker in a cmmunity setting including wrk with CALD, Aged and clients with a disability Experiencing in the prvisin f care crdinatin f services fr yung peple in a schl setting and/r within an integrated service delivery mdel. Experience in develping partnerships with key stakehlders in the prvisin f cllabrative and integrated services. Experience in develping, implementing and evaluating evidence based psych-scial prgrams. Well develped verbal and written cmmunicatin skills. Sensitivity t issues related t the prvisin f services in a cmmunity f high disadvantage and cultural diversity. Ability t wrk as a member f a team. Demnstrated ability t participate in the develpment f new and innvative prgrams. Cmputer literacy.
