Purpose : The purpose of this role is to: Provide a strategic overview of both Primary Healthcare Organisations as well as wider developments within the Primary Care sector Provide contract management and monitoring for all Primary Healthcare Organisation contracts within Waitemata & Auckland DHBs Monitor Primary Healthcare Organisation delivered programmes within the district and to initiate changes in service delivery as a result Understand and actively manage Primary Healthcare Organisation stakeholder relationships Use population approaches, data and other evidence based strategies to initiate and drive Primary Care improvements Lead and work closely with key team members and key stakeholders ensure that within Waitemata & Auckland DHBs, that there is a coordinated and coherent approach to the implementation of the Primary Healthcare Strategy. KEY TASKS Develop and provide strategic overview of Primary Care issues and initiatives within the ADHB & WDHB areas through: Understanding the status of Primary Care in the district Understanding the key determinants of child and whanau health Understanding national and regional child health and other social policies Understanding the range of plans, projects and services currently being provided Advising the Group Funding Manager, the GM Funding and Planning and the Board as to the appropriate strategic direction for Primary Care within the ADHB & WDHB areas. Develop, understand and actively manage DHB Primary Care stakeholder relationships Identify key Primary Care stakeholders within the region, external and internal Identify key national and regional key stakeholders Develop and maintain relationships with these stakeholders Involve key stakeholders in Primary Care initiatives as appropriate Actively work with primary care and engage Maori and Pacific providers and stakeholders as appropriate. Primary Healthcare Organisation Contract Management Understand all ADHB & WDHB Primary Healthcare Organisation contracts Develop relationships with all providers Manage and monitor all contracts Liaise with other members of the Group Funding and Primary Care Team as appropriate with respect to Primary Care services Monitor Primary Care service delivery and initiate service reviews as appropriate Contribute to the Funding Team-based Primary Care team EXPECTED OUTCOMES Able to articulate the range of government policies in this area Able to articulate the overall status of Child, Youth and Oral in the ADHB & WDHB areas Able to articulate the key determinants of Primary Care Provides written advice to ADHB & WDHB on Primary Care strategy regarding their respective providers and populations. KEY TASKS Report writing and oral presentations Reducing inequalities and population health gain Positive and constructive relationships with key stakeholders are established and maintained EXPECTED OUTCOMES Contribute to the goals of the DHBs and the Planning and Funding Team by producing highquality reports (including Board and Community & Public Health Advisory Committee) and oral presentations, as agreed with your manager. Write relevant board reports, HNA, DAP, DSP and Ministry of Health reports as required. Ensure all primary care projects have a focus on reducing health inequalities and lead to measurable health gain. Stakeholders have a known point of contact within respective DHBs for Primary Care issues. The DHBs are responsive to stakeholder issues in the area of Primary Care Administration co ordination Organise regular meetings with your team Arrange agendas and take minutes Elevate matters requiring management sign off to manager Liaise with other teams to ensure integrated approaches are achieved Recognise Individual Responsibility for Workplace Health and Safety under the Health and Safety in Employment Act 1992 Company health and safety policies are read and understood and relevant procedures applied to their own work activities Workplace hazards are identified and reported, including self management of hazards where appropriate Can identify health and safety representative for area Statutory & Treaty obligations Ensures the Auckland and Waitemata DHBs meets all statutory regulatory and contractual obligations, including financial reporting and compliance with occupational health & safety and employment legislation.
