1 JOB TITLE: Occupational Therapist - Rotational 1. PURPOSE OF POSITION This appointee to this position is responsible for providing an Occupational Therapy service to clients in specified wards and the community within Taranaki. The occupational therapy service (screening, assessment, programme planning, intervention and discharge) is provided for clients who meet the referral criteria. These rotations include home visits. Inpatient rotations are 6 months in duration and are based at Taranaki Base Hospital. The community rotation is 12 month duration and there is an expectation this is completed after the inpatient rotation or has relevant experience. The Occupational therapist is expected to follow the Occupational Therapy process to identify the client s occupational performance issues and provide appropriate intervention, recommendations and referral to other agencies, as required. 2. ACCOUNTABILITIES Key area of responsibility 1. Clinical Duties and Technical Functions Effectively provides an Occupational Therapy Service to designated clients and team Expected outcomes Follows the occupational Therapy process: a) Accept appropriate referrals. ie: when a client/tangata whaiora has, or is at risk of developing, an occupational performance issue. b) Screen the referral to determine appropriateness and prioritise accordingly. c) Select theoretical approach appropriate to the occupational performance issues. d) Conduct assessments to identify the occupational performance components and environmental conditions. e) Negotiate goals with the client/tangata whaiora, and where appropriate family/whanau and other involved parties. f) Develop and implement action plans to achieve identified goals. g) Evaluate outcomes throughout the process and discontinue intervention as appropriate. Carries out specialised assessments for ACC and Ministry of Health clients and makes recommendations/applications for people with long term needs including housing modifications, equipment provision, wheelchairs and seating. Carries out Functional Independence Measure (FIM). Carries out specialised assessment and makes recommendations/applications for people with shortterm needs including housing solutions, equipment provision, wheelchairs and seating. Provides education to client/ family/carers/ tangata whaiora with the goal of the client adapting safely to his/her current/future level of function. 3 2. Professional Practice Acts in compliance with ethical, legal, professional and safety requirements 3. Team Work and Communication Works effectively with clients/tangata whaiora, family/whanau and other involved parties within the organisation and community to maintain effective therapeutic and working relationships of equipment/housing modifications. Develops individual rehabilitation programmes within the community to facilitate and maintain occupational performance. In conjunction with other Occupational Therapists, work is allocated to Occupational Therapy Assistants and Wheelchair Technician. Cover is provided for other services as requested by the Occupational Therapy Clinical Leader or Professional Lead. Cover may include any area within the service. Occupational Therapy practices are maintained at all times in accordance with the Code of Ethics for the profession, Occupational Therapy Core competencies, Occupational Therapy Process Standards and other legislative requirements. Actions are in accordance with Taranaki District Health Board and Occupational Therapy service policies, protocols and procedures. Client clinical records are kept and reports completed in accordance with Occupational Therapy department policy. Accredited equipment assessor status is achieved and maintained in the relevant areas. Credentialing in Housing Registration is achieved and maintained, prior to commencing the community rotation or within the first 6 months of starting the rotation. This is achieved via the Enable New Zealand credentialing process. Occupational Therapy interventions are client-centred. Services are provided that respect client/tangata whaiora cultural beliefs, attitudes and practices. Liaison and communication occurs with other health professionals and relevant services and community agencies. The multi disciplinary team concept is utilised to ensure effective management of the client/tangata whaiora occupational performance issues. Contribution is made to the development of Occupational Therapy services by identifying quality improvements and assisting with the implementation of service changes. Recommendation to enable New Zealand, ACC, NASC agencies and others are provided. Statistics are recorded daily as per department procedure. Information is provided to generate invoices for any contract work. ie: ACC. Equipment and resources are used and tracked according to organisation procedures. 4 4. Professional Development Uses opportunities to continually develop professional knowledge and practice. Occupational Therapy staff meetings, clinical supervision sessions, in-service programmes and relevant seminars and courses are attended. Participates in Performance and Training Reviews. Actively seeks feedback on performance. 6 8. EDUCATION Degree/Diploma in Occupational Therapy. 9. EXPERIENCE/SKILLS NZ registered Occupational Therapist and holds a current practicing certificate. Relevant clinical expertise in acute rehabilitation, acute inpatients, neurology/stroke. Knowledge and experience of Enable New Zealand, including accredited and registered equipment/ basic housing assessor. Knowledge of ACC process particularly NAIR contract. Experience of using FIM (functional independence measure). Experience of using InterRAI assessment. Experience in developing and implementing quality initiatives.
