2 seventh annual sc heart & stroke care alliance general information who should attend Cardiologists Emergency department physicians & all physicians involved in cardiac, stroke, sudden cardiac arrest and heart failure care overall program objectives Plan and implement clinical and operational elements to build and sustain a statewide cardio- and cerebrovascular system of care involving all acute care facilities in South Carolina. Identify and implement validated standards and processes for timely access to treatment for heart and stroke care patients. Identify South Carolina facility specific and statewide process and outcome measures for improvement. Plan and implement processes to reduce facility specific and statewide variability in clinical processes related to interventional STEMI care, heart failure, stroke and sudden cardiac arrest in South Carolina through shared learning using the collaborative model. location William L. Participants who attend the entire activity and complete an evaluation will earn 4.5 contact hours. AMA PRA Category 1 Credits for Physicians: This activity has been planned and implemented in accordance with the Essential Areas and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the University of South Carolina School of Medicine Palmetto Health CME Organization and the South Carolina Hospital Association.
