2 TARGET AUDIENCE The 2014 Diabetes Summit is designed for specialists, primary care physicians, pharmacists, dietitians, nurses, certified diabetes educators, and other healthcare professionals with an interest in diabetes. LEARNING OBJECTIVES Identify diabetes prevention and management programs within the Greater Rochester Region. Identify cardiometabolic risk (CMR) factors in patients with or at risk for prediabetes or type 2 diabetes. Implement screening strategies to promote early detection and prevention of prediabetes or type 2 diabetes. Discuss with patients how preventing risk factors can positively impact health. List risk factor treatment options and define goals of treatment. Identify medication adherence strategies to appropriately manage type 2 diabetes ACCREDITATION STATEMENTS Physicians The American Diabetes Association is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. The American Diabetes Association designates this live activity for a maximum of 4.0 AMA PRA Category 1 credits. Physicians should claim on the credit commensurate with the extent of their participation in the activity. Doctors of Osteopathy Category 2 credit will be awarded for formal educational programs that are ACCME-accredited or AAFPapproved Physician Assistants AAPA accepts certificates of participation for educational activities certified for AMA PRA Category 1 Credit from organizations accredited by ACCME or a recognized state medical society. Physician assistants may receive a maximum of 4.0 of Category 1 credit for completing this program. Nurses The American Diabetes Association is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center s COA. California Board of Registered Nursing The American Diabetes Association is also a provider approved by the California Board of Registered Nursing, Provider No. CEP This activity is approved for 4.0 contact hours.

3 Dietitians The American Diabetes Association is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR). Registered dietitians (RDs) and dietetic technicians, registered (DTRs) will receive 4.0 continuing professional education units (CPEUs) for completion of this program/material. Pharmacists The American Diabetes Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. ACPE Provider number The American Diabetes Association designates these sessions as knowledge-based per ACPE. Upon completion of the online evaluation, the American Diabetes Association will upload the needed pharmacy-related continuing education information to the CPE Monitor within 60 days. Certified Diabetes Educators To satisfy the requirement for renewal of certification by continuing education for the National Certification Board for Diabetes Educators (NCBDE), continuing education activities must be diabetes related and approved by a provider on the NCBDE List of Approved Providers ( NCBDE does not approve continuing education. The American Diabetes Association is on the NCBDE List of Approved Providers. Successful Completion Statement Certificates of Completion/Attendance are provided to registered attendees based upon completion of the online evaluation. 7 2014 ADVOCACY EVENTS AT THE DIABETES SUMMIT SAFE AT SCHOOL ADVANCED SCHOOL ADVOCACY TRAINING November14, :30-5pm Rochester Academy of Medicine as part of the 2014 Diabetes Summit Ensuring that children with diabetes are safe at school and have the opportunity to excel requires more than just wishing and hoping. It requires advocacy people focused on making sure that students with diabetes are treated fairly in school and that their diabetes needs are safely met. As part of our mission, the Association, through our Safe at School campaign, works to give a voice to students denied their rights because of diabetes. With expert staff support, our volunteers who complete the Advanced School Advocacy Training Program (ASAT) program are able to enhance the Association s ability to expand its capacity to help parents/guardians of children with diabetes across the country. Program Goals: ASAT s main goal is to expand the Association s ability to ensure that students with diabetes are medically safe at school and have the same educational opportunities as their peers. In this small, one-day workshop, advanced school advocates will be trained to: Facilitate Safe at School Workshops for parents/guardians of school aged children with diabetes Educate others to stand up for and prevent the unfair treatment of affected children Interact with school personnel to begin to forge relationships that will help achieve goals Prepare for Section 504 and IEP Team Meetings Impact other Association advocacy efforts LEGAL ADVOCACY ATTORNEY NETWORK BREAKFAST Friday, November 14, :30-8:30 a.m. as part of the 2014 Diabetes Summit Hosted by Richard Yates and Tony Lee Presenters: Stephen Habbe, Advocacy Director, American Diabetes Association Crystal Jackson, Director of Safe at School, Government Affairs & Legal Advocacy, American Diabetes Association When a child is told he must transfer to another school to receive diabetes care, his parents call the American Diabetes Association. And when an employee is fired for taking a break to treat low blood glucose, she calls the American Diabetes Association. We are here to help our community understand how to support people with diabetes and to help people with diabetes advocate for that support. As a volunteer organization, we rely on local advocates to educate, negotiate, litigate, and legislate for change. Our 2014 Diabetes Summit will welcome members of our legal community to our Advocacy Breakfast Advocates will not be obligated to work on any particular task or case, and we will not ask them to waive their fee. We just ask advocates to be at the other end of the line when their neighbors are in need.
