3 JOB DESCRIPTION: POINT: 5-7 SALARY: SERVICE/DEPARTMENT JOB TITLE ACCOUNTABLE TO per hur Hmecare Supprt Wrker Service Manager Peripatetic and Sessinal Supprt, Assistant Service Manager Peripatetic and Sessinal Supprt JOB SUMMARY Supprt Wrkers wrking in the Hme Care service prvide care and supprt t peple in their wn hmes. The aim f their wrk is t ensure that peple remain healthy and well and are as independent as pssible. Smetimes this means helping peple wh are returning hme frm hspital care. Usually the emphasis is n helping peple t stay happy and well s that they will nt need care away frm hme. The wrk cmprises f multiple huse calls, fr varying perids f time, during which assistance is prvided in accrdance with the persn s Care Plan and Service Delivery Plan. The rle des invlve persnal care. MAIN DUTIES AND RESPONSIBILITIES: 1. Supprting the persn 1.1. Respect the persn and their hme at all times Prvide prf f identity when accessing the persn s hme, as required Ensure that the service prvided is based n all relevant infrmatin abut the persn, including their Care Plan and Service Delivery Plan and mst recent recrds Get t knw the persn s that their supprt is apprpriate and sensitive t their individual wants and needs Enable the persn t express their preferences abut the care and supprt they wish t receive, and t give verbal r implied cnsent Assist the persn with varius tasks which may include:
4 Washing, changing medical dressings, shwering, bathing, dressing and undressing, ral hygiene, tileting, and getting in and ut f bed. Taking prescribed medicatin and cmpleting the necessary dcumentatin Preparing and/r heating meals, drinks and evening snacks where necessary, and mnitring fd and drink intake. Shpping, cllecting items such as pensins and prescriptins, dealing with crrespndence, and paying bills Using mbility equipment as instructed. Transprting and escrting. Arranging scial cntacts and activities. (NB. This list is typical but nt exhaustive. Activities will vary accrding t the individual) Enable the persn t d things fr themselves as much as pssible, including helping them t re-learn skills r regain their cnfidence Ensure that the persn s dignity, cmfrt and privacy are maintained whilst assistance is being given Recrd imprtant infrmatin such as hw the persn is, whether they have any requests r cncerns, activities undertaken and incidents f nte Obtain the persn s signature n timesheets (when apprpriate). 2. Keeping everyne safe 2.1. Risk assess the care t be prvided, and any related equipment r physical aspects f the hme Safely put n, take ff, and dispse f, prtective clthing r equipment Thrughly wash hands befre and after hygiene-related supprt activities Be alert t any indicatins that the persn may be at serius risk frm such matters as accidents, fd pisning, r abuse Where serius risks are identified, take actin directly r reprt them t management as apprpriate.

Subject t the persn s wishes, encurage and supprt them t keep in tuch with peple that they knw and trust Cmply with Cartrefi Cymru s plicy and prcedures cvering the Abuse f Vulnerable Adults Ensure the persn s hme is secure when leaving. 3. Maintaining cnfidentiality 3.1. Disclse infrmatin relating t Cartrefi Cymru, r service users, r ther wrkrelated cntacts, as fllws: nly when it is essential fr the carrying ut f duties nly t thse wh need t knw. 4. Organisatinal duties 4.1. Liaise with the Line Manager, ther team members and ther rganisatins as requested Participate in and cntributing t staff meetings Attend apprpriate training events and cmpleting apprpriate dcuments related t training Assist, when required, in maintaining apprpriate recrds. 4.5 Cntribute t an nging system f service evaluatin. 4.6 Be aware f Cartrefi Cymru's Equal Opprtunities Plicy and cmplying with it and als staff related plicies Undertake duties apprpriate t the Supprt Wrker rle, as required, in any part f the Cartrefi Cymru service within the lcality 5. 7 CARTREFI CYMRU PROTECTION OF VULNERABLE ADULTS The brad definitin f a vulnerable adult is a persn wh is r may be in need f cmmunity care services by reasn f mental r ther disability, age r illness, and wh is r may be unable t take care f him r herself, r unable t prtect him r herself against significant harm. What is abuse? Abuse is any frm f actin, perpetrated against a persn withut their cnsent and, in a way that ptentially acts against their interests, is a vilatin f an individual s human and civil rights by any ther persn. Abuse can take a number f frms, including physical, verbal, sexual, financial, emtinal r psychlgical, neglect and acts f missin and discriminatin. Cartrefi Cymru will d its utmst t prevent abuse thrugh careful recruitment and selectin prcesses, guidance and training fr care staff and mnitring. PVA List Like all Care Prviders, Cartrefi Cymru, has a legal bligatin t refer all abuse cases fr inclusin n the Prtectin f Vulnerable Adults (PVA) list. The PVA list is a natinal database that cntains the names f peple wh are deemed unsuitable t wrk r have cntact with vulnerable adults. In the case f inclusin n the prvisinal r permanent list, Cartrefi Cymru is barred frm cntinuing t emply an individual in a care psitin and he r she must be mved frm the care psitin immediately if this has nt already been dne. If a mve is nt pssible, dismissal may be the nly ptin. What des this mean fr yu? 1. If yu are nt n the PVA list, either as cnfirmed r prvisinal: we lk frward t receiving yur applicatin frm. 2. If yur name is n the prvisinal list: yu are welcme t apply, hwever Cartrefi Cymru cannt emply yu in a care psitin until yu have been exnerated frm the abuse allegatins and yur name has been remved frm the prvisinal list. (If yu are included n this list yu will have been infrmed and have been given the pprtunity t submit yur cmments as t why yu shuld nt be included n the list.)
8 3. If yur name is n the permanent list: by legislatin yu may NOT seek emplyment within the care sectr. Cartrefi Cymru will nt ffer yu emplyment as a care wrker. (If yu are included n this list yu will have been infrmed and have been given the pprtunity t submit yur cmments as t why yu shuld nt be included n the list.)
9 CARTREFI CYMRU POLICY STATEMENT ON THE RECRUITMENT OF EX-OFFENDERS Because f the nature f Care wrk, all Care Psitins are exempt frm the prvisins f Sectin 4(2) f the Rehabilitatin f Offenders Act 1974 by virtue f the Rehabilitatin f Offenders Act 1974 (Exceptins) (Amendment) Order As such all applicants are required t declare any cnvictin, cautin, warning etc. even if they wuld therwise be regarded as spent under this Act. We will ensure that all thse in Cartrefi Cymru wh are invlved in the recruitment prcess have been suitably trained t identify and assess the relevance and circumstances f ffences. We will als ensure that they have received apprpriate guidance and training in the relevant legislatin relating t the emplyment f exffenders, e.g. The Rehabilitatin f Offenders Act At interview, r in a separate meeting, we will ensure that an pen and measured discussin takes place n the subject f any ffences r ther matter that might be relevant t the psitin. Failure t reveal infrmatin culd lead t withdrawal f an ffer f emplyment. If yu are already emplyed by Cartrefi Cymru then we reserve the right t terminate yur emplyment summarily. We will make every subject f a Criminal Recrds Bureau Disclsure aware f the existence f the Criminal Recrds Bureau Cde f Practice and will make a cpy available n request. We undertake t discuss any matter revealed in a Disclsure with the persn seeking the psitin befre withdrawing an ffer f emplyment.. If yu are already emplyed by Cartrefi Cymru then we reserve the right t terminate yur emplyment summarily. Having a criminal recrd will nt necessarily bar smene frm wrking with us. This will depend n the nature f the psitin and the circumstances and backgrund f their ffences, and n their hnesty in disclsing prir ffences.

It undertakes nt t discriminate unfairly against any persn being the subject f a Disclsure n the basis f cnvictin r ther infrmatin revealed. Cartrefi Cymru is cmmitted t the fair treatment f its staff, ptential staff r users f its services, regardless f race, gender, religin, sexual rientatin, respnsibilities fr dependants, age physical/mental disability r ffending backgrund. We have a written plicy n the recruitment f ex-ffenders, which is made available t all Disclsure applicants at the utset f the recruitment prcess. We actively prmte equality f pprtunity fr all with the right mix f talent, skills, and ptential and welcme applicatins frm a wide range f candidates, including thse with criminal recrds. We select all candidates fr interview based n their skills, qualificatin, and experience.
