3 SECTION 1 BACKGROUND The Western Health and Social Care Trust provides services to a 3,800 square miles area of outstanding beauty in the west of Northern Ireland. It delivers acute services across this region through two main acute hospitals Altnagelvin Area Hospital (AAH) in Londonderry, and the new 267 million South West Acute Hospital (just outside Enniskillen) supported by a local hospital the Tyrone County Hospital in Omagh. SOUTHERN SECTOR Acute healthcare for Fermanagh and West Tyrone was previously provided by Western Health and Social Care Trust from two sites the Tyrone County Hospital (TCH), Omagh and the Erne Hospital, Enniskillen. Following a rationalisation of acute services in March 2009 and major capital funding a new 276 million, 312 bedded, Acute Hospital has been commissioned which opened 22nd June 2012 and named the South West Acute Hospital This new 312 bedded acute hospital incorporates full 24 hour accident and emergency, critical care (ICU & HDU), Acute Surgical Services, Acute General Medicine, Cardiology with CCU, Acute Stroke Services, Rehabilitation Medicine and Care of the Elderly services. It is also the base for Obstetrics, Gynaecology and Paediatric Services across the Southern Sector. The new radiology department offers comprehensive services including a new 128 slice spiral CT and new MRI scanner, both with non-invasive cardiac modalities. The new acute hospital incorporates new fully equipped day case, endoscopy / bronchoscopy suites and five new hospital theatres. The hospital has full 24 hour laboratory services. The Tyrone County Hospital now provides non-acute inpatient rehabilitation, orthogeriatric care and stroke rehabilitation. There is an Urgent Care and Treatment Centre incorporating a cardiology assessment service, a Diabetes Day Resource Centre, and a full range of outpatient facilities. Support facilities include a full radiology department with spiral CT scanning, a dedicated day procedure unit and a purpose built laboratory block. The South West Acute hospital anticipates in excess of 5,500 acute medical admissions per year. NORTHERN SECTOR The Western Health and Social Care Trust area hospital Altnagelvin is situated in Derry/Londonderry and provides a full range of acute services. It is the largest district hospital in Northern Ireland and has the widest range of specialties outside Belfast City and Royal Group of Hospitals. There are approximately 450 beds on the site covering all the acute specialities including Trauma and Orthopaedics, Maxillo-Facial surgery and Ophthalmology in addition to the usual DGH specialities. It is designated as a Cancer Unit linked to the Cancer Centre at Belfast.

4 THE POST The Western Health and Social Care Trust is seeking to appoint a Consultant Physician with an interest in Acute Medicine upon the South West Acute Hospital site. The Consultant will have responsibility for providing an Acute Medicine service in conjunction with other consultant colleagues. Specialist interests that complement those of the existing consultants and expand the current services would be welcome. This is a new post, aimed at enhancing senior medical input to the Medical and Surgical Assessment Unit (MSAU), and is in addition to the current 0.5 Acute Physician input to the unit. This provides an excellent opportunity for a suitably qualified specialist to actively contribute to the further development of not only the MSAU, but also policy across the Trust for the management of the acutely unwell individual. An outline of the Medical and Surgical Assessment Unit (MSAU) is provided below.

5 SECTION 2 MEDICAL AND SURGICAL ASSESSMENT UNIT The MSAU is a purpose built unit located on the ground floor, in close proximity to the Emergency Department, Radiology and ITU/Coronary care Units. All medical and surgical patients referred by GPs to assess the need for admission to hospital are assessed here. The unit also receives urgent adult medical and surgical patients requiring further assessment and treatment from other areas such as A&E, outpatients and the mobile Coronary Care Unit. Surgical patients undergoing assessment are the responsibility of the on-call surgical team. The Unit itself can cater for 16 patients in single rooms in addition to having an area to be developed as an ambulatory area for patients requiring admissions for transfusions, infusions, simple medical procedures etc. There is the ability within the Unit to increase capacity down the line should it be required. Annually, in the region of 6000 patients are admitted to the unit, with an average length of stay of hours. With on average 80-85% of patients staying less than 24 hours, and over 30% being discharged directly home, the unit combined with the hospital Bed Co-ordinating team greatly supports the daily flow of patients through the hospital. Presently, the on-call physician provides morning and evening ward round input to the unit, with additional rostered input outside of these provided by Dr. Keegan on the acute medical side. There are currently, in addition to the on-call junior doctors, two medical specialty grade level doctors allocated to the unit, with funding for a further 1.5 doctors of this grade approved. The unit has enhanced access to radiological and laboratory investigations, including CT and MRI scanning in addition to access to a dedicated blood gas analyzer. The PACS system is accessible within MSAU, enabling recall of all existing radiological images for any given patient, with system for bedside availability of such images and patient blood results in each room due to go live in the immediate future. The intranet provides a wide range of guidelines, contact numbers and other useful resources. The MSAU takes a central role in the Trust s Hospital at Night Programme. It is also a very important area for the teaching, training and assessment of Junior Doctors as well as an important area for the teaching and training of Medical Students and other staff.

6 SECTION 3 JOB DESCRIPTION Title Consultant Physician with an Interest in Acute Medicine. Location Based at South West Acute Hospital, Enniskillen. New/Replacement Post New Post. Accountable to Managerially to the Chief Executive of the Trust for the successful fulfilment of their responsibilities and working with the Director of the Emergency Care Division, to ensure delivery of a high quality service. Professionally, through maintenance of standards within the Service Contracts, to the Medical Director. Principle Job Purpose To provide in conjunction with colleagues Acute Medicine services for the Trust. Minimum Qualifications/ Experience Essential Criteria Applicants must be on the General Medical Council (London) Specialist Register for Acute or General Internal Medicine with another Specialist Interest. Doctors in training must be within 6 months from date of interview of obtaining the Certificate of Completion of Training. Doctors who would be deemed eligible within 6 months from date of interview, for inclusion on the Specialist Register prior to taking up appointment will also be considered. Where professional qualifications form part of the criteria it will be necessary to produce original certificates issued by the appropriate authority. Failure to produce evidence of qualifications will result in the rejection of an application.

7 Main Duties 1. Share with consultant colleagues the Acute Assessment workload. 2. Provide with the other Consultants a system of mutual out-of-hours cover and cover for annual, study and professional leave. 3. Give a consultation and advisory service to medical colleagues in other specialties in the Trust. 4. Liaise with medical staff in other specialties and participate in the regular clinical meetings and other post-graduate activities of the hospital. 5. Take part in the Royal College of Physicians continuing medical education programme. Study leave expenses in line with national terms and conditions of service will be provided. 6. Play a full part in the teaching and audit activities of the department, and be involved in providing professional supervision and management of junior medical staff. 7. Participate in the rolling process of clinical governance. 8. Contribute to management and development of the service. This will include development of protocols and care pathways, and involvement in established MSAU interface meetings with Medical, Surgical and Emergency Department staff. 9. Comply with the outline Consultant work programme (under provisions of Circular HC90/16) and the weekly timetable of fixed commitments (i.e. regular scheduled NHS activities in accordance with paragraph 30B of the TCS). 10. Deploy unallocated, flexible professional time on liaison, emergency work, travel, continuing clinical responsibility, teaching, research, medical audit, inter-disciplinary meetings and continuing professional development. Other Responsibilities Subject to the provisions of the Terms and Conditions of Service to observe the Trust s agreed policies and procedures drawn up in consultation with the profession on clinical matters and to follow the standing orders and financial instructions of the Trust. To observe local and national employment and personnel policies and procedures, and to comply with Regional and Trust Health and Safety policies. To assume responsibility for and facilitate contact from members of staff with delegated duties for personally accountable patient care.

The basic contract is for 10 programmed activities. Additional programmed activity may be available for negotiation depending on circumstances e.g. on call arrangements. The Job Plan will be subject to review in the context of annual appraisal and will be subject to modification by mutual consent. PROVISIONAL ASSESSMENT OF PROGRAMMED ACTIVITIES IN JOB PLAN Direct Clinical Care 7.5 PA s on average per week Includes clinical activity, clinically related activity, predictable and unpredictable emergency work. Supporting Professional Activities 2.5 PA s on average per week Includes Medical school teaching and familiarisation with the Trust policies, guidelines and procedure and also includes CPD, audit, other teaching activity and research. The teaching component may vary. Research interests would be accommodated if possible within the job plan.

9 JOB PLAN REVIEW The job plan will then be reviewed annually, following an Appraisal meeting. The document will be a prospective agreement that sets out a consultant s duties, responsibilities and objectives for the coming year. It will cover all aspects of a consultant s professional practice including clinical work, teaching, research, education and managerial responsibilities. It will provide a clear schedule of commitments, both internal and external. In addition, it will include personal objectives, including details of their link to wider service objectives and details of the support required by the consultant to fulfil the job plan and the objectives. ON CALL COMMITMENT The current Acute on call rota for general medicine covering the hospital is 1 in 9. With changes in the admission patterns there may well be in the on call arrangements to incorporate the new post-holder. These will be in agreement with the Consultant body and remuneration for predictable and unpredictable on call PA allowance will be factored into subsequent job-planning. This job description is not meant to be definitive or restrictive and may be modified to meet changing needs with agreement between the post-holder and Director of the Emergency Care Division.

(S)he will be expected to work effectively with consultant colleagues, the Lead Clinician, Assistant Director, Clinical Director, Divisional Managers, senior nursing staff and others in the formulation and updating of policy guidelines and initiatives to develop and enhance services within the Trust. The post-holder will be expected to undertake administrative duties associated with patient care and to ensure arrangements are in place to meet quality standards for appointments and discharge procedures etc. within contracts.

11 SUBSPECIALISATION The post holder is expected to have a sub-specialisation the skills of which should either enhance current service or develop new service supported by the Trust. In particular specialties such as Respiratory Medicine, Diabetes and Endocrinology or Acute Stroke Medicine would be considered. However other new areas such as formal Educational or Managerial skills would also be considered. ADMINISTRATIVE DUTIES The post-holder will be expected, to undertake administrative duties associated with the care of his/her patients and to contribute to the smooth running of the department. The Consultant will have a continuing responsibility for the care of patients in his/her department. The post-holder would be involved in the review and development of policies and process for the delivery of acute care. This would also include the development of pathways for ambulatory care. Secretarial support for this post at the moment is 20 hours per week ie 0.5 WTE. INDUCTION AND MENTORING The Western Trust has an established programme of induction for supporting consultants newly appointed, and early in career. There are also development programmes offered, and arrangements in place to offer mentoring to new appointees. APPRAISAL / REVALIDATION The post-holder, as with all career grade staff within the Western Trust, will undergo annual appraisal. The Clinical Director on behalf of the Chief Executive will normally perform this appraisal. The post-holder will be expected to conform to current and any future GMC guidelines on revalidation. ACCOMMODATION The appointee will have office accommodation fully equipped with /internet enabled PC provided on the South West Acute Hospital site. Secretarial support will be provided to the post-holder by the Medical Specialty Team. (Approximately 0.5 WTE per consultant). Full automated digital dictation systems are available to all consultants. The systems used to support administration and information are largely computerised and all clerical support staff are trained in diagnostic coding, audio typing and all other office automation and clerical tasks.

12 TEACHING DUTIES / EDUCATION The South West Acute Hospital incorporates a new state of the art multidisciplinary education centre with full videoconferencing facilities to support clinical practice, teaching, learning and research. The Trust is fully committed to supporting CPD/CME activity allowing appropriate leave and funding. The education centre supports education for a range of medical students and other disciplines including practical skills training with a broad range of the latest medical simulators. The library in the new South West Acute Hospital is linked through the MDEC Library at AAH as part of the N.I. Health and Social Services Library system and provides services to Altnagelvin, and the South West Acute Hospital, as well as to anyone eligible to use the Queen s University Library. Both Altnagelvin and the South West Acute Hospital support education for a range of medical students and other disciplines including practical skills training. The hospital delivers undergraduate education to third through final years of Queen s University Belfast Medical School. The post-holder would be expected to support the final year students in Medicine, though other modules similarly look to the MSAU to be able to support teaching. The hospital also has a range of post-graduate junior doctors, where again the unit provides a fertile source for education for these individuals. The post-holder may be involved in clinical or educational supervision of doctors in training, and be expected to undertake one session per week in teaching/training/appraisal of these doctors and to carry out the responsibilities of clinical / educational supervisors as indicated in RCP guidelines. The successful candidate will be encouraged to contribute to, and participate in, original and on-going clinical research, and to participate in in-service training for hospital staff. They would also be expected to remain updated with ideas and innovations within the profession, and to follow the RCP recommendations regarding Continuing Medical Education, with study leave being granted whenever possible. The South West Acute Hospital has established links to the Royal Colleges of Physicians (UK) and the Royal College of Physicians of Ireland providing live, interactive video-conferenced linked CPD approved training. There are also well established image links to neuroradiology/neurosurgery at the Regional Unit in Belfast and all three hospitals support active involvement in cancer care networks through use of videoconferencing linked multidisciplinary team meetings. ONLINE RESOURCES HONNI (Health on the Net Northern Ireland) HONNI is the website designed specifically for Health and Personal Social Services (HPSS) staff in Northern Ireland. It provides a single access point to relevant resources on Medicine, Dentistry, Nursing & Midwifery, Pharmacy, Psychology, Social Work, Allied Health and Health Management. HONNI also provides HPSS staff with easy access from a single location to a wealth of relevant information, including clinical guidelines and healthcare subject gateways. 13 SECTION 6 TERMS AND CONDITIONS NHS Terms and Conditions of Service for Hospital Staff will apply. Contributions will be payable under the Health and Social Service Superannuation Scheme. There is reciprocity between the Northern Ireland scheme and that in operation in Britain. Remuneration will be governed by the New Contract Framework for Northern Ireland. ANNUAL LEAVE Annual leave will be six weeks and 3 days per annum plus statutory and public holidays. START DATE Appointment to the post will be as soon as possible, subject to the period of notice required by the successful candidate. RESIDENCE The post holder must live within 10 miles of the "On Call" Hospital and must maintain contact through the public telephone service. REGISTRATION AND DEFENCE Registration with the GMC must be maintained. License to practice arrangements and the new revalidation procedures must be complied with. The Trust bears responsibility for claims of Dental/Medical negligence against practitioners occurring in the course of their Health Service employment. Practitioners are advised to maintain their defence body membership in order to ensure that they are covered for any work that does not fall within the scope of the indemnity scheme. PRIVATE PRACTICE Participation in private practice will be governed by the terms of the post holder s individual contract. MEDICAL EXAMINATION Any offer of appointment will be subject to the completion of a satisfactory general health assessment. This will be arranged by the Trust through its Occupational Health department.
