1 FROM MICAL PRACTIC JOB SCRIPTION Job Title: Normal Place of work: Accountable to: Level: Job Purpose: Nurse Practitioner Frome Medical Practice Operations Managers VIII To offer a high standard of evidence based care to patients, to include health promotion, treatment, screening and disease management. MAIN UTIS AN RSPONSIBILITIS 1. Main duties and responsibilities To undertake surgeries working in an autonomous role including the management of minor ailments, chronic disease, family planning To undertake home visits to include both acute visits and the management of chronic medical problems in the house-bound To undertake the supervision of triage staff and taking responsibility for quality and training. To liaise with and advise the uty Manager to ensure most appropriate use of resources To be a training resource. To be responsible for the collection and presentation of data that will lead to development of the service To promote the Triage System within the Practice, working towards a nurse delivered same day service supported by a octor for guidance and consultation. 2. Pathological Specimens and Investigatory Procedures. i. Undertake the collection of pathological specimens including intravenous blood samples (when required), swabs, cervical smears and skin scrapings. ii.all procedures requiring collection of pathological specimens are performed within agreed NHS Somerset protocols and in
2 accordance with the Health & Safety xecutive. 3. Administrative and Professional Responsibilities. Participate in the administrative and professional responsibilities of the Practice team. i. nsure that all patient information following consultations, registration, and/or including telephone encounters are recorded accurately on the computer, using MIS and RA CO as appropriate. ii.very effort should be made to attend and participate in practice meetings, including practice nurse meetings. 4. Research. iii. Assist in the continual review of practice philosophy, strategy and policy. iv. To participate in the formulation of practice protocols in conjunction with NHS Somerset. Co-operate and support research undertaken by the Practice including the referral of suitable patients who may benefit as participants in research programmes. 5. ducation and Training of Students and Practice Staff. Participate in the induction, education and training of all new members of practice staff including students of all disciplines where appropriate. 6. Personal and Profession evelopment. i. Maintain lifelong learning by attending formal and informal educational courses and study days. Undertake a personal commitment to support lifelong learning by updating through relevant books and journals and the retainment of a reflective diary/professional portfolio. ii. To have either successfully complete the xtended Nurse Prescribing course or have the intention of doing so. 8. Liaison. Maintain effective liaison with other health care teams concerned with patient care as appropriate and with all other disciplines within the practice, exercising regard for patient confidentiality at all time. quality and diversity Identify patterns of discrimination and take action to overcome this and promote diversity and equality of opportunity

There may be other duties required of the post holder commensurate with the position. This description will be open to regular review and may be amended to take into account development within the Practice. All members of staff should be prepared to take on additional duties or relinquish existing duties in order to maintain the efficient running of the Practice. 4 Qualification s & Training xperience Current Registration RGN BSc (Hons) Membership of a professional body e.g. RCN. To work in accordance with the NMC Code of Conduct Nurse Practitioner egree (2year programme) or BSc in mergency Care Have successfully completed a minor ailment and injury module at BSc/ Msc level vidence of PRP A commitment to undertake a Primary Care Independent Prescriber Qualification Qualified as a Nurse Practitioner within the last two years Independent Nurse Prescriber NB 998 Teaching and assessing in clinical practice (or equivalent) Leadership experience 1 year minimum triage/minor illness work, A& or as an mergency Care Practitioner 2 years minimum triage work or A& xperience of telephone triage Previous Primary Care experience Family Planning xperience
5 Skills & Abilities Personal Qualities Other Customer focused Autonomous working skills emonstrate leadership skills Ability to work both independently and as part of a team Good communications skills and computer literacy. Hold a current driving licence and have the use of a car MIS xcellent Communication Skills Flexibility Team Player Ability to self-motivate, organise and prioritise workloads. An understanding, adherence to the need for strict confidentiality. Ability to use own judgement, resourcefulness, common sense and local knowledge to respond to the needs of patients.
