2 The Oklahoma Chapter of the Case Management Society of America (CMSA-OK) invites you to participate in a three day celebration of the case management profession: Celebrating Case Management Made in Oklahoma, October 1-3, 2012 at the Embassy Suites Hotel and Conference Center in Norman, OK. This year s theme highlights products, programs, and individuals that are native to the state of Oklahoma. From presenters with roots in our state to cutting edge programs right here in our own back yard, we are focusing on the people and programs that make Oklahoma OK! We will have a CCM Prep Course on Monday, Oct. 1. This will be a full day, pre-conference workshop for those taking the exam or who want a review of the Case Management Standards of Practice along with legal & ethical implications. Attendees will receive 7.0 Contact Hours for RN, Social Workers and CCM for attending this course. On Tuesday, Oct. 2, we will have a one day Annual Educational Conference. Whatever your specialty is, there are topics and speakers to help you as you learn about people and programs that are Made in Oklahoma This will offer 5.25 CEUs for RN, CCM, CRC, CDMS and social work clock hours. Also on Tuesday, Oct. 2, the Exhibit Hall will be open to allow you to network and talk with resources essential to our profession. New this year, the Vendoropoly participation during the tour of the Exhibition Hall will allow the attendees to acquire an additional 1.0 CEU while learning about Continuum of Care: Coordination through Resources poster presentation. On Tuesday evening, Oct. 2, there will be a special reception/ceu presentation with Carol Smith, CMSA National Board of Directors and Public Policy Chair. Carol will be available to network with attendees to discuss the status of the Multistate Compact Licensure, CMSA involvement, and Oklahoma's grassroots efforts. This reception will provide attendees with an additional 1.0 CEU through this poster presentation and discussion. To wrap up the week s activities, on Wednesday, Oct. 3, we will have a one day Leadership Summit. This will be a post-conference workshop with outstanding leaders and representatives from programs in Oklahoma sharing their knowledge and experience on team building and professional development. This will offer 6.25 CEUs for RN, CCM, CRC, CDMS and social work clock hours. Make your reservations early and take advantage of the discounts. 4 EDUCATIONAL PROGRAM AGENDA MONDAY, OCTOBER 1, :30 a.m. 4:30 p.m. A Comprehensive Overview of Case Management; A Tool for Preparing for the Case Management Certification, BK Kizziar, RN-BC, CCM It has been said that case management is the best kept secret in healthcare. Case managers may be found in all areas of healthcare & deliver a service that is critical in facilitating successful client outcomes. This session will provide a comprehensive look at the practice of case management including various practice settings, reimbursement models, the role of the case manager & much more. A review of the Case Management Standards of Practice is included along with legal & ethical implications. TUESDAY, OCTOBER 2, :00 9:30 a.m. Cancer Research and Nurse Navigator Program, Jari Askins, Former Lt. Governor, Director of Marketing & Outreach, Stephenson Cancer Center Of Oklahomans alive today, 1.4 million will be diagnosed with cancer. That s more than 18,000 new cases each year. 7,500 Oklahomans die each year from cancer. Oklahoma s cancer death rates are higher than the national average in most categories. Ms. Askins presentation will highlight the benefits and realities of having a comprehensive cancer center in Oklahoma and what it means to patients diagnosed with cancer along with their Nurse Navigator Program. 10:30 11:45 a.m. CONCURRENT SESSIONS Oklahoma s Adoption of ODG What Can you Expect?, Charles Kennedy, MD Dr. Kennedy will present a walk-through of ODG including the history and methodology of ODG. Highlights of his presentation will include: disability management with evidence based medicine, low back pain diagnosis and treatment, medication crisis and how ODG is working in other states. Developing an Effective Discharge Plan, Brenda Keeling, RN, BSN, CPHQ, CPUR A common thread and ongoing challenge between hospitals, home health, skilled nursing, rehabilitation facilities and insurance case managers is the development of an effective and safe discharge plan. Joint Commission, NCQA, Medicare and Medicaid Services Conditions of Participation, State Department of Health, and others hold providers and payers accountable for the development of an effective discharge plan. The plan must map a course of treatment aimed at minimizing the likelihood of rehospitalization while focusing on clinical, financial, psychosocial and functional status of the patient. In this session learn the requirements for an effective discharge plan, regardless of your employment setting. 11:45 a.m. - 1:30 p.m. Lunch, Continuum of Care: Coordinating Care Through Resources, Elizabeth Teet, LSW, CCM, ABDA, Poster Presentation and Time with Exhibitors Persons with continuous complex care needs frequently require care in multiple settings. During transitions between settings and/or providers, this population is particularly vulnerable to experiencing problems of care fragmentation. Despite how common these transitions have become, the challenges of assuring quality and coordination of care continue for case managers in the face of the changing role of healthcare and healthcare legislation. It is crucial that case managers are aware of the options along the healthcare continuum for appropriate levels of care, options for resources for those care needs, and how to access these resources to ensure quality and efficient care in all settings along the continuum.

5 EDUCATIONAL PROGRAM AGENDA TUESDAY, OCTOBER 2, :30-2:45 p.m. CONCURRENT SESSIONS Panel: ODG's - What does that mean for Oklahoma?, Charles Kennedy, MD, Jeanne Edmondson, RN, BSN, CCM, Laurie Westlake, RN, CCM, Jerry Whorton & Glenn L. Smith, DO, FAOAO Effective March 1, 2012, Work Loss Data Institute s Official Disability Guidelines (ODG) became the standard for evidence-based medical treatment provided for workers compensation injuries in Oklahoma. ODG provides both treatment and duration guidelines for conditions commonly associated with the workplace. The panel will explore how ODG is being utilized within the state of Oklahoma; as well as, how our state and our individual practices can benefit from these guidelines. Each member will share how ODG affects their practice. A representative from Work Loss Data Institute will participate on the panel so to better understand the benefits and use of these guidelines in our effort to provide quality health care to our clients. Effective Transition of Care Strategies (It s Gonna Take a Village), BK Kizziar, RN-BC, CCM Regardless what shape healthcare reform takes public and private payors have indicated that many of the measures contained within the reform bill will be enacted and become regulation. Value based purchasing, bundled payments, penalties for hospital re-admissions and other quality measure make it imperative that all providers work together to ensure success outcomes for the patients they represent. This session will detail specific strategies that will successfully move the patient from the ED through transition beyond the hospital walls. 3:15 4:30 p.m. CONCURRENT SESSIONS Guidelines for Chronic Pain Management, John Munneke, MD Pain, what is a chronic pain patient? This session highlights chronic and acute pain, treatments available, the types of chronic pain, medications used for chronic pain and the problems/issues with dependence and addition. Department of Rehabilitation Services, Eligibility and Services, Sarah Tebow, CRC & Amy Martin, LPC Vocational Rehabilitation Services prepare qualified applicants to achieve a lifestyle of independence and integration within their workplace, family, and local community. Most of our services are free to the public however many practitioners still struggle to connect clients with these services in a meaningful way. Learn more about the OKDRS is, who is eligible, what services are available, and how to most successfully access them. 6:00 p.m. 7:30 p.m. Networking Reception & Poster Presentation, Is Your Job Putting You At Risk? Why Oklahoma Nurses Need The Nurse Licensure Compact, Carol Smith Healthcare is increasingly managed and delivered on regional and national levels yet state based nursing licenses put nurses at risk. The Nurse Licensure Compact (NLC) protects nurse licenses for nurse case managers working across state lines. Learn more about the NLC and what you can do to advocate for all Oklahoma nurses. WEDNESDAY, OCTOBER 3, :30 10:15 a.m. Celebrating Change, Frieda Smith, RN CELEBRATING CHANGE shows how to accept and adapt to organizational or life changes with professionalism, determination, and optimism. People are masters of adaptability. Embrace the fact that change is never boring; in fact, it can be a time of great creativity. Learn ways to deal with the stress and imbalance often caused by change. This presentation is humorous, motivational, inspirational, and emotional. Laugh and be inspired to deal with change in a fresh, new way.

6 EDUCATIONAL PROGRAM AGENDA 10:30 a.m. 11:45 a.m.maintaining Moral and Ethical Values in the Face of Diversity, Brenda Keeling, RN, BSN, CPHQ, CPUR Healthcare reform and financial status of the Medicare Trust Fund are driving massive and fast paced changes in today s healthcare. Hospitals and payers are challenged to meet regulatory compliance while constantly balancing the budget. Understanding that without a margin there is no mission, facility leaders are constantly struggling with monitoring financial impact to the bottom line. The case manager often finds her/himself struggling with moral and ethical values while surviving in the financial climate. In this session, learn strategies for surviving these challenges. 11:45 a.m. 1:30 p.m. Special Olympics in Oklahoma, Heather Hancock & Cathi Morris Special Olympics was founded by Eunice Kennedy Shriver. The first International Special Olympics Games were held in 1968 at Soldier Field in Chicago. Oklahoma's first Special Olympics event was an athletics competition at the University of Tulsa in Special Olympics Oklahoma is authorized and accredited by Special Olympics, Inc., for the benefit of citizens with intellectual disabilities. Special Olympics provides sports training and athletic competition in a variety of Olympic-type sports for children and adults with intellectual disabilities. Attendees will learn about Special Olympics, advocacy, teamwork, and overcoming challenges from Cathi Morris, Program Services Director for Special Olympics Oklahoma and Heather Hancock, Special Olympics Athlete and Global Messenger. 1:30 2:45 p.m. Team Dynamics and Action Planning, Rebecca Fincher-Wilber This session will discuss how to build and engage a team of individuals to work together for common goals. Review of how to handle sensitive and often emotionally charged meetings, that are often the result of miscommunication will be discussed. Other highlights include: examining an action planning process that allows the team to stay focused on the outcomes and responsibilities of the team members and techniques to encourage the team and elicit compliance for action plans. The processes identified will have universal standards to be utilized for all levels of leadership. 2:45 4:00 p.m. Bridge and Boundaries: Building High Performance Intergenerational Teams, Carol Mannahan, EdD, RN, NEA-BC Dramatic changes in society, personal lives, and the workplace present unique challenges to modern health care leaders. One of the most puzzling changes is the generational make-up of the workforce. For the first time in history, four generations, each with differing mindsets and communication styles, find themselves working side by side. Friction can be aggravated by the complexity of the health care system, new technologies, and work patterns that mix workers of different ages in ever-changing teams. Mapping, bridging, and integrating will be discussed as strategies to assist leaders to reap the benefits that diversity offers. Rather than suppressing differences, leaders will be encouraged to engage the differences through a systematic process designed to build team effectiveness and enhance performance.
