2 Message from the President Dear Members, On behalf of the Indiana Association for Healthcare Quality (InAHQ), I am pleased and honored to invite you to the 36 th Annual Conference. This event will be held in the beautiful city of Indianapolis, IN from May 14-15, This year s theme Quality in Harmony Across the Healthcare Continuum provides members the opportunity not only to learn more about the complexity and the o p port uni ties f or transformational change in healthcare, but to take advantage of additional topics and workshops led by experts in their respective professions. Registrants will apply this knowledge to an existing project and leave with a plan in place for their practice setting. (2 hour presentation with workbook) Hazel Crews: Show me the Money Data: Pay for Performance in the Acute Care Setting -Presentation will discuss the structure of Pay-for-Performance programs that exist in the acute care setting including the Value Based Purchasing Program, the Hospital Readmission Reduction Program, and the Hospital Acquired Conditions Reduction Program. The initiatives for each program will be listed with discussion of program measure details that impact both performance and reimbursement. 5
6 Breakout Session Presentations Continued: Susan McAlister: Sepsis-A Model of Continuous Improvement -Presentation will describe the evolution of Sepsis and the component parts which make a Sepsis program successful. Sepsis recognition, treatment, and measures of success for a continuous journey in process improvement. Karin Kennedy: Hospital Engagement Network 2.0: Next Steps -During this session, participants will learn the impact of the IHA HEN and learn about anticipated next steps. Additionally, this session will focus on incorporating the importance of engaging patients and families and embracing a culture of patient safety. David Jones: Legal Issues in Partnering Relationships & Care Coordination - Presentation will explore the changing relationships along the reimbursement and delivery system landscape. Discussion will additionally review the legal and regulatory issues faced by providers in these new settings. Carolyn Townsend: Charts for Quality Professionals -Presentation will demonstrate graphs including run and control charts with discussion related to when and how to utilize each. Each participant will be provided with a workbook to assist them with practicing the techniques when they return to their workplace. (1 hour presentation with workbook) Linda Gaul: Care Coordination with Patient-Centered Medical Home, Meaningful Use, and the Accountable Care Organization -Presentation will show how a physician network is implementing a population health and care coordination strategy to assist the organization to meet both payer requirements and the needs of complex patients. Tara Hatfield: PQRS (Physician Quality Reporting System): Leveraging Technology to Improve Quality and Ease Reporting Burdens -Presentation will discuss how PQRS is becoming mainstreamed among ambulatory clinics as federal requirements change. The session will highlight how a practice can leverage their technology to not only improve quality, but additionally ease the burden of annual reporting. 6
7 Breakout Session Presentations Continued: Dr. Jose Azar & Dr. Boustani: Implementation Science -Presentation will discuss the definition of Implementation Science with discussion of theoretical and applied knowledge in this field. Natalie Stewart: PCMH (Patient-Centered Medical Home): Tips for Incorporating Common Tenets -Presentation will provide an introduction to the program and discuss the overlap with other program domains. This program will additionally highlight strategies for implementing the tenets of PCMH. Jan Buland: Avoiding Adversarial Peer Review -Presentation will define/review state and federal regulatory guidelines that outline the peer review process with focus on the formation of a multi-specialty peer review committee and restricted access of peer review outcomes. Don Gettinger & Sharon Barclay: CMS QIO Current Initiatives Overview and Collective Impact for Care Coordination -Presentation will discuss the community care coordination efforts (in relation to implementation of evidence-based tools with quality measurement) and will provide information about the benefits and opportunities to participate in these initiatives. Carolyn Townsend: Overcoming Your Fear of Basic Statistics for Quality Professionals -Presentation will provide an overview of basic statistics, including mean, median, modes, and t-tests, as well as when it is appropriate to utilize each statistic. The Data Analysis package in Excel will be demonstrated. Each participant will be provided with a workbook to assist them with practicing the techniques when they return to their workplace. (1 hour presentation with workbook) 7

Substitutions are permitted and these must be requested in writing ( ). CONTINUING EDUCATION CREDITS: An application has been submitted to HQCB for 14 CEUs towards CPHQ Certification. One hour of attendance corresponds to 1 CEU. Approval is pending.
