1 2014 Care Transitions Forum COLLABORATING TO STRENGTHEN CARE TRANSITIONS November 10-11, 2014 Holiday Inn Harrisburg-Hershey Grantville, PA REGISTER TODAY! 2 Forum Overview: By improving transitions of care, providers can engage patients, share care plans, and coordinate efforts to ensure that patients receive the right care at the right time, helping reduce unnecessary hospital readmissions. This statewide conference will feature national, Pennsylvania, and regional perspectives on innovative strategies to improve transitions in care, with a focus on participation, action, and collaboration. Representatives from across the care continuum will share their insights and expertise on person-centered, effective practices, and methods for overcoming barriers. A combination of plenary speakers, breakout sessions, roundtable discussions and networking time will provide multiple opportunities for translating ideas into action. Six different breakout sessions will allow you to maximize your learning experience and attend the sessions that are of special interest to you! Don t miss the opportunity to explore innovative patient care transitions concepts and dialogue with others about how to make an impact and start the momentum towards collaborating together. Most of all challenge yourself and each other to think about how you can take what you learn back to your own organization and make it applicable, meaningful, and scalable to meet your unique needs and challenges. Participants will be able to: Describe national, state, and regional efforts to improve care transitions Discuss strategies to improve cross continuum partnerships Identify available resources and effective practice to facilitate better care Describe the roles to be played by both acute and post-acute providers in managing patients across a broad continuum Discuss application of medication reconciliation, physician integration into care settings, an enhanced communication among providers to deliver better care Evaluate how care coordination strategies interface with emerging models of care such as medical home and accountable care organizations Who Should Attend: This conference will be of particular interest to nurses, physicians, discharge planners, case managers, emergency department staff, home health executives, nursing home administrators, quality leaders, pharmacists and any health professionals interested in improving care transitions across the continuum including hospitals, home health agencies, skilled nursing facilities, rehab hospitals, hospices, physician practices, and numerous other providers and community organizations that support care delivery.

It s not an easy task, but not impossible either. 6 CONTINUING EDUCATION CREDITS Nurses (CNE): The Hospital & Healthsystem Association of Pennsylvania (HAP) is an approved provider of continuing nursing education by the Pennsylvania State Nurses Association (PSNA), an accredited approver by the American Nurses Credentialing Center s (ANCC) Commission on Accreditation. The Hospital & Healthsystem Association of Pennsylvania (HAP) designates this activity for a maximum of 8.25 contact hours. Credit will be awarded based on self-reported attendance, and completion of a program evaluation form will be required prior to leaving the meeting venue. Nurses should only claim credit commensurate with the extent of their participation in the activity. ACHE Qualified Education Credit (non-ache): HAP is authorized to award 8.25 hours of pre-approved ACHE qualified education credit (non-ache) for this program toward advancement, or recertification in the American College of Healthcare Executives. Participants in this program wishing to have the continuing education hours applied toward ACHE qualified education credit should indicate their attendance when submitting application to the American College of Healthcare Executives for advancement or recertification. General certificates of attendance will be issued to all participants. Some national, state, and local licensing boards and professional organizations will grant continuing education credits for attendance when you submit the course outline (save the brochure) and your certificate of attendance. It is recommended you contact your own board or organization to find out what is required.
