1 ONS CHEMOTHER APY and BIOTHERAPY COURSE Prcedures Effective July 1, 2012 The Cancer Chemtherapy Curse was develped t help standardize the educatin f nurses regarding administratin f chemtherapy. Trainer s names, telephnee numbers and address are prvided upn request by ONS. A listing f trainers and their addresses are als psted n the ONS website 1. Curse Lgistics Determine prgram hst, date, time, and lcatin f curse. ONS defines an apprpriate hst as a hspital, physician ffice, r ther like clinicall institutin and/r an ONS Chapter. Ntify ONS immediately f curse dates and lcatin. Trainers are respnsible fr setting up curses n their wn. Curse dates and lcatin, Trainer(s) names and cntact inf are regularly psted by ONS n the virtual cmmunity site. Trainers are nt able t pst curses t the virtual cmmunity site Pleasee allw at least ne week fr psting t the website Curse date will nt be psted until ONS has received and apprved a cpy f the annually required brchure 2. Curse Brchure One brchure must be develped and submitted fr apprval prir t advertising the first curse. The apprved brchure may then be used fr the entire year. All curse brchures/flyerss must include the fllwing: Target Audience: The intended learner fr this prgram is a licensed registered nurse with a minimum f 6 mnths emplyment as a nurse, wh pssesses a basic understanding f cancer bilgy. Cntinuing Nursing Educatin (CNE) credit: The credentialing statement must stand alne, meaning that it shuld be n a separate line frm any ther statements, including number f cntact hurs awarded. Separatin frm ther statements with a perid and a new sentence is nt sufficient the accreditatin statement must be n a line by itself and appear as fllws: ONS will award cntact hurs t registered nurses wh attend this prgram in its entirety, earn at least 80% n the exam, and submit the curse evaluatin. ONS is accredited as a prvider f cntinuingg nursing educatin by the American Nurses Credentialing Center s Cmmissin n Accreditatin. ONS is apprved as a prvider f cntinuing nursing educatin by the Califrnia Bard f Registered Nursing, Prvider # This is NOT a cmpetency r certificatin curse. The individual emplyer r institutin shuld determine clinical cmpetency. All bjectives fr the curse

2 Curse Requirements Attend the curse in its entirety Minimum f 80% n the pst test Submissin f the Curse Evaluatin within ne week f the clse f the curse Only registered nurses are eligible t receive the ONS chemtherapy prvider card 3. Registratin Registratin /payment agreement frms MUST be RECEIVED at ONS TWO WEEKS prir t the curse. Registratin frms shuld be ed t educatinchem@ns.rg. This will start the registratin prcess All trainers participating as curse faculty must be listed n the registratin frm. Trainers nt listed n the registratin frm will nt receive credit fr the curse and are nt eligible t serve as faculty. A maximum f 4 trainers may act as faculty fr a curse Any change in curse faculty must be reprted t ONS befre the start f the curse. If ONS is nt cntacted, the individual will nt receive credit fr teaching because they were nt evaluated. All faculty listed n the registratin frm are expected t teach equal prtins f the curse Trainers wh d nt meet this tw week prir t curse deadline will be charged fr express shipping f curse materials. Registratins frms received seven days r less befre a curse will nt be prcessed. The curse will need t be rescheduled. If additinal curse materials are required (additins t the riginal registratin frm) thse materials must be requested first by telephne and then a new registratin frm ed t ONS SEVEN BUSINESS DAYS prir t curse. D nt send additinal registratin frms withut cntacting ONS. The trainer is respnsible fr all registratin materials, serving as the cntact persn fr all curses. This is the case even fr curses cnducted at an utside institutin. If a curse is being cnducted fr an institutin, it is the trainer s respnsibility t prvide the institutin with all registratin materials and deadlines fr submissin. In the event an institutin r department des nt cmply with the deadlines, the trainer will be the respnsible persn f cntact. All participants must have their wn curse materials t participate in the curse. The sharing, cpying r recycling f curse materials is nt permitted. If the trainer desires t accept late registrants/walk ins, the trainer must have adequate materials n hand. Additinal materials will need t be rder in advance. Curse materials will nt be shipped after the curse has begun. D nt accept late registrants int a curse if yu are unable t fully accmmdate them and they have been included in the numbers submitted t ONS Curse participants are nt t be instructed t cntact ONS t purchase their curse materials. Such requests will nt be hnred and the individual will be referred t the curse trainer. ONS prepares and ships curse materials based n the registratin frm received. Materials will be shipped based n the number listed n the frm. 4. ONS will nt sign any letters f agreement between the prgram prvider (speaker), hst and any cmmercial interest supprting the prgram as ONS is nt invlved in the nsite prgram executin r mnitring.

3 5. Curse Cst and Payment Cst: The curse participant fee is $90 per participant. This fee is nt a text fee but cvers all curse materials, prcessing cntinuing educatin credit, prvider card and recrd keeping. ONS des nt discunt the curse fee. Trainers are permitted t charge abve this amunt t cver csts assciated with ffering the curse (e.g., fd, rm rental, AV equipment rental). Trainers are respnsible fr all shipping csts. All curses must be paid in full PRIOR t being permitted t hld additinal curses. Payment: All curses will be inviced t the trainer (nt an institutin). The trainer is the nly authrized purchaser f ONS Chemtherapy & Bitherapy curse materials, regardless f wh may be paying fr curse ONS will nt invice an institutin nr will invices be mailed t an institutin The trainer will be the individual inviced fr all curse materials and the invice will be mailed directly t the trainer by the ONS Finance Department. Third party checks (i.e., lcal ONS Chapter, institutins) cvering 100% f the cst f the curse will be accepted. Individual checks frm participants will nt be accepted. Purchase rder frm r request fr payment must be submitted with the registratin frm. Purchase rder infrmatin received after the registratin frm will nt be included n the invice. The trainer is financially respnsible fr all curse expenses, including shipping. If an institutins will be paying fr the curse materials and specific infrmatin shuld be included n the invice make sure that infrmatin is nted n the registratin frm. 6. Distributin f Curse Materials Upn receipt f curse registratin frm and fees, curse materials will be shipped t the trainer t arrive at least 2 days prir t the curse. If the trainer des nt receive the shipment at least 2 days befre the scheduled curse date she/he must ntify ONS immediately fr tracking f the materials. The ONS Natinal Office clses at 5:00PM EST. The ONS Business Center must have all shipments by 3:30PM EST. Please remain aware f time differences t ensure shipments can be tracked in a timely manner. Failure t ntify ONS that materials have nt arrived may result in materials nt being available fr the curse and the need t reschedule. Curse materials (guidelines, tests, etc.) MAY NOT be cpied r reprduced in any manner. 7. Curse Prcedure T fully execute the curse, the Trainer must: Cnduct the curse: Trainers may present curse ver tw days, r split int smaller blcks (tw eight hur days, fur fur hur blcks, etc.) Curse cntent cannt be abbreviated. Administer pst test The curse utline includes apprximate times fr cntent delivery. The trainer is t insert apprpriate breaks t address adult learning needs. Additinal slides which enhance the curse cntent (e.g., case examples) may be inserted as lng as the trainer is able t cver the bjectives and stay within the cntent delivery time.

4 Distribute evaluatin cde infrmatin t each eligible curse participant n the last day f the curse. Eligible curse participants are thse that have attended the entire curse and earned at least an 80% n the curse exam. OPTIONAL CASE STUDIES: Case studies have been integrated thrughut the curse. Additinal case studies and a matching game are prvided t be used at the discretin f the trainer. Participatin is based n grup preference r experience. Optins include: Small grup interactin with case study presentatin t entire grup. (Example: small grups each receive a different case study and present the case t all curse participants). Small grup interactin with each grup reviewing all cases fr discussin within the individual grup. Trainer presents each case t participants fr grup discussin. Trainer presents each case t participants fr them t answer n their wn in written frmat. Participant receives all case studies fr essay cmpletin n their wn. 8. Examinatin The trainer is t maintain the integrity f this examinatin by keeping it in a secure area at all times. N ne may duplicate the exam. Review the examinatin prir t the curse t highlight cntent during yur lecture. Participants must cmplete the pen bk examinatin n their wn within the scheduled time frame (75 minutes). Testing as grup is nt permitted. The examinatin is intended t be administered in the English language. Participants will return exam fr trainer scring. The participant s scre must appear n the scan sheets when returning t ONS. Make sure the participant has cmpleted the address infrmatin n the scan sheet. Hme address is required. After the curse, all exams are t be shredded. Exams are nt t be held and stred after a curse. 9. Cntinuing Nursing Educatin Credit Required curse cmpnents t receive cntinuing nursing credit are: Attending the entire curse Earning a passing scre f 80% r better n the pst test and Cmpleting the curse evaluatin via ONS nline evaluatin system. Only registered nurses are eligible t receive the ONS chemtherapy prvider card Upn cmpletin f the curse, all participants must cmplete and submit the evaluatin within 1 week, even if retesting is required. Neither CNE credit nr a prvider card will be issued if the participant des nt cmplete an evaluatin. ONS will nt fllw up with individuals wh d nt cmplete the evaluatin. Participants will nt be able t print their CNE certificate and prvider card immediately frm the Evaluatin Center CNE certificates and prvider cards remain in Pending mde until ONS has received and prcessed the curse.

5 Once curse materials have been received and verified, participants and trainer(s) will receive an frm ONS that the certificates and cards can be printed. Prvider cards will nly be issued t RNs wh have successfully earned a scre f 80% r better n the pst test and submitted the nline curse evaluatin. Passing the pst test des nt eliminate the need t submit the curse evaluatin ONS will nt prvide hard cpy evaluatins t participants. Participants wh audit the curse are nt eligible fr CNE credit r the prvider card Individuals wh submit an evaluatin and have nt met all curse requirements will nt receive CNE. 10. Participant Wh Fails Examinatin If the participant des nt pass the examinatin, they can make arrangements with the trainer t retake the exam within TWO WEEKS f the curse. The curse evaluatin must be cmpleted within ne week f the end f the curse by all curse participants even if a retest is necessary. All curse materials shuld be held by the trainer until the participant has retested. Prctrs are permitted t mnitr the re taking f the exam. Cmplete the prctr agreement frm n the trainer disk prvided by ONS. The prctr agreement shuld be returned t ONS with the curse materials If additinal tests are needed, the trainer must cntact the Natinal Office. When retaking the pen bk exam, nly the riginal curse materials and a calculatr may be used t assist the student. Participant has 75 minutes t cmplete the exam. Trainer will check exam and return all curse materials t ONS. If the participant des nt pass the secnd exam (minimum scre 80%) and still desires t receive the ONS Cancer Chemtherapy Prvider Card, the trainer may permit the participant t sit thrugh a future curse. Participants are nt permitted t take the exam a third time withut sitting thrugh anther curse. Trainers will be charged $20.00 fr new test materials and prcessing fr participants sitting thrugh a secnd curse new guidelines will nt be supplied. Charges (abve the $20.00 paid t ONS Natinal) t the participant sitting thrugh a future curse are at the discretin f the trainer. 11. Participant Cancellatins Trainers determine their wn cancellatin plicies. If a participant cancels r des nt shw fr the curse, and the trainer wishes t return the curse materials, a refund f $72 (less 20%) will be refunded upn return f curse materials t the ONS Natinal Office. A refund request frm must accmpany the returned materials. ONS will nly refund a maximum f 5 guidelines annually per trainer. 12. If a Participant Must Leave Early N refund will be made by ONS N cntinuing nursing educatin credits will be prvided t the participant (partial credit cannt be awarded). Include the individual n the rster, nting the participant did nt attend the entire curse Evaluatin cdes are nt t be distributed The trainer can determine if the participant can attend a future curse.

6 If the participant cmpleted day ne in its entirety day tw must be cmpleted within 60 days r the individual will need t attend bth days f a future curse. If the participant attends a future curse and new curse materials are required, the full participant fee will be charged. 13. Materials Returned t ONS: All f the fllwing materials must be returned t ONS within ONE WEEK after curse unless there is a retest then materials shuld be held and sent in at ne time. Individuals must retest within tw weeks Final typed curse rster including name, hme address, phne number, , RN license number f all participants and final test scre. (This frm is prvided as an Excel file n yur trainer disk) Check in apprpriate clumn any participant that is nt an RN r wh has failed the exam and nt retested. These individuals are nt eligible t receive a Prvider Card. All cmpleted scan sheets (including failed and retest scan sheets). Exams that have nt been scred will be returned delaying the release f CNE and cards t participants Trainer evaluatin f ONS and the curse prcess. Disclsure frm All used and unused exams shuld be shredded. String testing materials is nt permitted. All participants must be included n the curse rster (even thse wh have nt attended the curse in its entirety If a participant fails, hld all curse materials and return them after the participant has retested. Participant has tw weeks t retest. Please ntify the Educatin Team at educatinchem@ns.rg when this ccurs. The trainer may keep unused guidelines frm his r her curse. When registering a subsequent curse, reference the riginal curse date and indicate the number r guidelines n hand, the number f guidelines being rdered and the ttal number f participants expected. ONS will send enugh materials t accmmdate the expected number f participants. This allws trainers t prvide materials fr late registrants and avid the 20% restcking fee. ONS will nly refund a maximum f 5 guidelines annually per trainer. Refunds f guidelines/curse materials are issued at $ A refund request frm must accmpany all guidelines being returned. 14. A summary f the curse evaluatins will be ed t the trainer(s). The summary will include any/all participant cmments. If there is mre than ne Trainer fr the curse, each individual Trainer will receive the evaluatin cmments specific t them. ONS will nt prvide Trainers with ther Trainers evaluatin infrmatin. 15. ONS Cancer Chemtherapy Prvider Card Renewal The prvider card is valid fr tw years frm the date f successfully cmpleting the riginal curse. All individuals wh are prvider cardhlders can renew their card by an independent study/nline curse thrugh ONS University. Tw curse ptins are available each mnth. ONS will attempt t ntify, by mail, all individuals wh are prvider cardhlders six t eight weeks prir t card expiratin n hw t renew their ONS chemtherapy prvider cards. Fr this reasn, it is imprtant t encurage students t ntify ONS f any address changes. Changes t cntact infrmatin can als be made nline thrugh an individual s ONS prfile. Delivery f renewal ntificatins cannt be guaranteed. It is the respnsibility f the prvider t renew their card befre its expiratin. Renewal infrmatin is prvided n the ONS website,

7 ONS cnsiders the expiratin date the last day f the mnth in which the card expires. Any individual requesting t renew after the expiratin date f the prvider card and within the 30 day grace perid will be charged a late fee f $25. The nurse must register t cmplete ne f the tw curses available during their grace perid. Online registratin clses ne week befre the curse start date. The nline renewal curse is 15 days in length. The nurse has 24 hur access t the curse while they are actively enrlled. If the renewal request is made after the 30 days grace perid, the participant must attend and successfully cmplete the ONS Chemtherapy and Bitherapy Curse t earn CNE credit and a new ONS Prvider Card the $90.00 curse fee will be required. Successful cmpletin f the chemtherapy renewal curse includes cmpletin f all curse lessns, passing exam scre f 80% r better and cmpletin f the curse evaluatin. ONS will nt issue prvider cards r CNE credit t individuals wh d nt cmplete all cmpnents. 16. T Maintain Trainer Status Adhere t all prcedures utlined abve Cnduct a minimum f 1 chemtherapy curse per year Maintain an average scre f 3.0 r greater n curse evaluatins Maintain current ONS membership Maintain a current ONS Cancer Chemtherapy Card by cmpleting the renewal prcess every 2 years. Maintain nclgy specialty certificatin (OCN, AOCN, AOCNP, AOCNS, CPHON) Maintain cmpliance with ONS Cancer Chemtherapy Curse plicies and prcedures Maintaining current status is the respnsibility f the trainer. ONS/ONCC sends ntificatin prir t the fllwing requirements lapsing ONS membership Chemtherapy prvider card Onclgy certificatin ONS Educatin will nt prvide additin reminders. 17. Cnsequence f Failure t Cmply with ONS Chemtherapy Plicies ONS reserves the right t revke an individual s trainer status if the trainer fails t cmply with plicies and prcedures gverning the ONS Chemtherapy and Bitherapy Curse, Trainer Curse, Trainer Prgram, and Renewal Prcess. An individual wh has failed t cmply with the abve plicies and whse status has been revked can apply fr reinstatement f trainer status within ne year f being remved. A $ reinstatement fee will be required. The fee will be refunded if a decisin nt t reinstate is made
