In response to your requests, AHHC has arranged educational opportunities for you with the cooperation of the Division of Medical Assistance. E-1 Creating a Culture of Patient Safety in Home Care Suggested Audience: All Speakers: Loretta Matters, RN, MSN, Director of Patient Services; Mari Pitcher, LCSW, DHCC Safety Team Facilitator, Manager of Duke Hospice Inpatient Facility; Jamie Puckett-Lee, RN, BSN, Home Health Local Quality and Safety Team Facilitator; & Laverne Mullin, RN, MPA, Director of Accreditation and Compliance Description: It has been reported that as many as 180,000 deaths occur in the United States each year due to errors in medical care, many of which are preventable. Homecare staff must recognize and acknowledge that homecare is a high risk industry related to potential patient safety risks. Today s discussion will focus on how to create a culture of safety within a home care agency by engaging all staff in the assessment of risk, the analysis of near misses, and the identification of solutions. The delicate balance of creating a culture of reporting and communicating events that supports the culture of safety while not tolerating negligent behavior will also be explored. We will present lessons learned to guide participants in creating a Culture of Patient Safety within your own agencies. 2

3 E-2 Making Home Care Sausage Inside the Washington Beltway - A Candid and Informative Interview with Ann Howard, Director of Federal Policy for the American Association for Home Care Suggested Audience: All Moderator: Ron Clitherow, MPH, LarsonAllen Description: Using a Charlie Rose type of interview style, attendees will have an enlightening opportunity to hear and dialogue with one of our nation s most experienced home care advocates and lobbyists. Ms. Howard will respond to a broad range of questions ranging from the most recent Deficit Reduction Act and political processes, how home care is perceived on the Hill and by whom and current and emerging issues in Congress and within CMS and MedPac. Attendees will have an opportunity to participate in this provocative discussion and call to action! E-3 PCS Scoring & Plan of Care Development Suggested Audience: Home Care - PCS Directors, Supervisors, Clinical Staff Speakers: Phyllis Steven, RN, DMA PCS Consultant & Kathie Smith, RN, AHHC Special Project s Coordinator/DMA Liaison Description: PCS must be provided under a plan of care based on the RN s assessment and certified as needed by the primary physician. Documentation on the PCS PACT form at recipient assessment times serves as the basis for determining whether the recipient qualifies for PCS. Part of the PACT form completion is scoring the recipient s functioning in each ADL; the scoring determines the type of assistance the recipient will need and the amount of time that may be needed for each task per DMA s time guidance. Through policy review and case studies, this workshop will cover scoring of the recipient s ADL s and developing the PCS Plan of Care based on PACT findings and scores. E 5 Understanding Accounts Receivables for the Agency Director Suggested Audience: Home Health Administrators, Directors, Supervisors, Finance Staff Speakers: Kimber Walters, M.B.A., Executive Director, Gaston Memorial Hospital Home Health Care Description: Most agency Directors understand the importance of cash flow and low DSOs. But what are the tools we need for managing on a day-to-day basis? And what if you're already in trouble? Through an actual case study, attendees will see how a medium sized agency turned around their billing/collections area and are able to maintain DSOs below industry averages. Attendees will leave with actual tools to assist them in their own agencies. E-6 In-Home Aide Special Session Walking the Walk! Suggested Audience Home Care, Home Health, Hospice In-home Aides and/or In-home Aide Supervisors Speakers: Carolyn Cusic, LPT and Leslie Carswell, RN, BSN Description: This session is a highlight of the Convention and is utilized by agencies to recognize and reward their aide staff as well as to provide them with the latest information impacting aides and their work environment. In this session, aides will "walk the walk" and experience what it is like to have a medical condition or disability that causes mobility, communication and/or sensory problems. The aides will walk away with renewed spirits of empathy and patience - as well as a deeper understanding of the importance of encouragement. E-7 The CAPT Grant: A Demonstration to Improve Direct Service Workers Job Satisfaction and Retention Suggested Audience Home Care, Home Health, Hospice Providers of In-home Aide Services Speakers: Linda Kendall-Fields, Grant Coordinator, Roy Burnette, MAEd., Administrator, Pathways for the Future S.A.I.L. Program; Dr. John Sherlock, & Grant Morgan Description: All in-home aide providers desire to recruit and retain quality staff. Faculty from the Pathways for the Future S.A.I.L. Program and research staff from Western Carolina University will discuss in this session the CAPT (Caregivers are Professionals Too!) Grant awarded to Pathways for the Future Center for Independent Living by CMS (Centers on Medicare and Medicaid Services). Four in home care agencies in Western North Carolina, representing over 900 Direct Care Workers, have participated in a variety of research initiatives including paid health insurance, performance bonuses, and longevity awards. The team will explain the results of several surveys and focus groups which have attempted to determine the impact these initiatives have had on job satisfaction and direct care workers willingness to remain in the home care profession. 3

4 CAP/DA CONFERENCE EVENTS Tuesday, May 2, 2006 General Session Dinner & Keynote 6:45 p.m. to 8:45 p.m. Denise Ryan, Fire Star - Caring for others is deeply rewarding, but can be also challenging and stressful. This keynote will not only fire you up about your life and your work, it will give you six steps to help you stay that way. You'll learn how to deal with enthusiasm vampires, how to conquer fear, and how to live your best life. This will be one dinner you won't want to miss! Coming highly recommended Denise is a magna cum laude graduate of the University of South Carolina with a master s degree in business. What she really specializes in, however, is lighting fires. Denise has re-energized thousands of people; waking them up to their full potential and helping them get everything they can out of their lives and their work. One of her audience members said She makes me feel like I can do anything! Ms. Ryan has spoken for Fortune 500 companies as well as small not-for-profit agencies. She has addressed audiences ranging from senior military officers to school secretaries. Her infectious enthusiasm, her big smile, and her candor are her trademarks. Audiences love her humor and the tools she gives them to bring the fire into their lives - both personally and professionally. CAP/DA EDUCATIONAL EVENTS Wednesday, May 3, Educational Activities 9:15 a.m.-11:30 a.m. F-1 Fundamentals of Plan of Care Development Speaker: Sandra Stanley, SW II, Albemarle Regional Health Services/Albemarle Home Care Description: This session will cover one of the most confusing elements of CAP, the Plan of Care. We'll cover general Plan of Care preparation, and get into the "nitty gritty" of the cost summary (effective date, formulas used in computing costs, DME, etc.). We will also discuss guidelines for revisions, CNRs, and review critical time limits. Time will be given to practice with a few case studies. This session would be great for new case managers of less than a year. F-2 AQUIP What You Need to Know! Speakers: The Carolinas Center for Medical Excellence (CCME) Staff Description: This session will focus on completing the CAP/DA data set and its effect on RUG scores as well as a system overview. This session will also serve as a refresher for current system users as well as an orientation for new AQUIP users. F-3 CAP Review and Ways to Improve Documentation Speaker: Tracy Colvard, PCS and CAP/DA Manager, Division of Medical Assistance Description: This session will focus on what a CAP/DA Consultant from the Division of Medical Assistance looks at when conducting an on-site program review. Minimum monitoring requirements that are expected from CAP/DA case managers will be covered along with documentation myths and realities. F4 Tips for Conducting an Accurate Functional Assessment of the CAP Client Speaker: Cheryle Atwater, PT, MPH Description: Functional assessments are always challenging; and to be valid, must be thorough and accurately document the client s functional abilities and limitations. This session will provide guidance on the hows of conducting a functional assessment covering such information as how to accurately measure a patient s ambulation and transfer capabilities; self-feeding abilities; toileting abilities; dressing abilities, etc. Faculty will also address intrinsic and extrinsic factors that influence the client s functional status. Accurately assessing the patient s functional status allows the case manager to more clearly paint a picture of the client s true needs. 4

Contact Hours: The Association for Home and Hospice Care of North Carolina is an Approved Provider of continuing nursing education by the North Carolina Nurses Association, an accredited approver by the American Nurses Credentialing Center s Commission on Accreditation. Cancellation and Substitution Policy: Fees will be refunded only if written notice of cancellation is received by AHHC on or before April 1, In the event of cancellation, AHHC will retain $45 of the initial fee, per registrant, to cover administrative overhead. While it is not permissible for several individuals to share a registration, AHHC will be happy to accept substitutions if notified of the change in writing. Hotel Information: The Sheraton Imperial Hotel & Convention Center in Durham, North Carolina (located just off I-40 in the Research Triangle Park between Raleigh and Durham) will serve as the host hotel and headquarters for the CAP/DA Casemanager Conference. The Association has secured a block of rooms at the Sheraton Imperial Hotel & Convention Center for the night of Tuesday May 2 nd at a special rate of $99 per night, single or double occupancy. The hotel offers free parking to hotel guests. Room reservations may be made by calling the hotel directly at (919) , or their national toll free number at Please state that you are with the Association for Home & Hospice Care (AHHC) when making your reservations to receive these special rates. Reservations must be made by April 07, 2006 to take advantage of the special rates, based on availability. 5
