2 General Information Conference description and goals Complications as a result of diabetes mellitus have serious medical, economic, and social implications for individuals, families, and society. The initial symptoms of diabetes may go unrecognized, yet chronic complications may already be developing at the time of diagnosis. Some individuals may have long-standing diabetes and develop complications later in life. As a health care provider you have a role in empowering individuals in the prevention and treatment of complications to minimize disease burden. The goals of the conference are to: Provide updated strategies to prevent complications of diabetes. Explore treatment options for patients with complications of diabetes. Identify opportunities to enhance quality of life for patients with diabetes. Date and location The conference will be held Thursday, February 26, 2015, in Phillips Hall, first floor, Siebens Building, Mayo Clinic, 100 Second Avenue SW,. Intended audience This conference is designed for certified diabetes educators, nurses in all specialties, and all other healthcare professionals interested in the care of patients with diabetes. Exhibits Nationally known companies will display the latest in medical products, pharmaceuticals, and services. You will have opportunities to visit the exhibits, interact with representatives, enjoy refreshments, and network with colleagues. Continuing education credit Mayo Continuing Nursing Education is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center s Commission on Accreditation. Participants can earn up to 6.75 ANCC nursing contact hours for attendance at the conference and completion of the evaluation. These hours apply toward recertification for certified diabetes educators (CDE). *Mayo Clinic Section of Social Work has been designated by the Minnesota Board of Social Work as an approved continuing education provider. This conference fulfills requirements for 1.0 continuing education hours. Register before Thursday, February 5, 2015 The conference registration fee of $155 includes instructional materials, breakfast, and break refreshments. Lunch will be on your own. A student rate of $85 is available. Please bring proof of current student status to the conference (for example, a copy of your student identification card). Pre-registration is required and preferred by Thursday, February 5, Registrations received after February 5, 2015 will be accepted on a space-available basis. Visit to register. 3 General Information Cancellations If you cancel your participation in this conference, your registration fee, less an administrative fee, will be refunded when notification is received by Mayo Continuing Nursing Education before February 5, 2015 (fax#: or No refunds will be made on or after February 5, Mayo Continuing Nursing Education reserves the right to cancel or postpone any conference due to unforeseen circumstances. In the unlikely event Mayo Continuing Nursing Education must cancel or postpone this conference, Mayo Continuing Nursing Education will refund the registration fee, but is not responsible for any related costs, charges, or expenses to participants, including fees assessed by airline/ travel/lodging agencies. Lodging and parking Rooms have been reserved for conference participants and their guests at the Doubletree by Hilton Hotel Rochester Mayo Clinic Area ( ) at a rate of $139 per night and the Rochester Marriott Mayo Clinic Area ( or ) at a rate of $140 per night. The rates listed may be extended before and after the conference based on availability. To ensure accommodations, please make your reservation by Thursday, February 5, Reservation requests received after this date will be accepted on a space available basis.
