2 About the Program The Empira Fall Prevention program is a combination of nationally recognized evidence-based, fallprevention practices and practical applications from numerous research studies. NCHCFA is pleased to once again partner with Empira to present a full-day seminar for all long-term care providers on RESTORING QUAILITY SLEEP-IMPROVING RESIDENT CARE AND QUALITY OF LIFE. Growing evidence from medical research points to the importance of sleep and the devastating impact inadequate sleep and rest has on the lives of all human beings. The National Institute of Health notes, Seniors who get fewer than six hours of quality sleep at night have a 50% increased risk of viral infections and increased risk of heart disease and stroke. Poor sleep is also associated with a poorer quality of life. CMS and senior service providers have never considered sleep hygiene as an integral part of the plan of care and services provided to the resident. The MDS 3.0, with over 500 inquiries, only mentions the word sleep three times in the entire document and it contains no reference to a sleep assessment. Empira s consortium of 23 skilled nursing homes has come to the forefront in recognizing that a good night s sleep is one of the most important health benefits we can offer our residents. Empira s, Restorative Sleep Vitality Program (RSVP) has two primary goals: provide uninterrupted sleep at night and increase wakeful daytime activities for our residents. They are challenging and changing some of the long held standards of care practices in order to achieve these goals. This Restorative Sleep Vitality Program is a three year quality improvement project funded through the MN Department of Human Services Performance Incentive Payment Program (PIPP). This session will cover the following: Restate the key components of the Empira Fall Prevention program. Explain the process that leads to the Empira Restorative Sleep Vitality Program. Discuss the etiology of sleep and wake in human beings. Identify the key components of the Empira Restorative Sleep Vitality Program. The top 10 disturbances to sleep. Five operational actions a long-term care provider can do to prevent sleep disturbance. About the Presenter - Sue Ann Guildermann Sue Ann Guildermann has over thirty-five years of experience providing education, leadership and consultation to non-profit and for-profit Long Term Care organizations. Sue Ann is a Registered Nurse with BA and MA degrees in communication and adult education. She currently produces educational seminars and conferences for Empira, a consortium of 27 skilled nursing facilities in Minnesota. Sue Ann directs quality improvement projects based upon resource analyses, evidence based practices and data assessment and interpretation. For the last four years, she has been responsible for the education of the leadership, management and direct-line staff of 23 skilled nursing facilities for a national and state pay-forperformance improvement project (PIPP). The purpose of this grant was to reduce resident falls in skilled nursing facilities. She has overseen the implementation of this program that has succeeded in reducing the prevalence of residents who have fallen by an average of 31%. In January 2012, Empira s Fall Prevention program was awarded the Minnesota Commissioner of Human Services, Circle of Excellence Award for their contributions to the wellbeing of human services clients. This fall reduction program was the foundation and precursor for Empira's current three-year government funded project; the investigation and elimination of sleep disturbances of residents in skilled nursing facilities. Sue Ann has edited, designed and written educational manuals, printed materials and videotapes. She also currently teaches at the University of Minnesota in the School of Public Health. Sue Ann is both a nationally and internationally recognized speaker.
