Our Golf Tournament We are hosting the Second Annual Saint Elizabeth Foundation Golf Tournament! Golfers have the opportunity to play at the Loyalist Golf and Country Club, one of our region s most prestigious golf courses. Our tournament s media exposure will help to increase the value of your investment in this important fundraising event. Participants will be comprised of corporate and community leaders from the South East. The Need Saint Elizabeth has recognized the need and impact of improving end-of-life care for people and families. For this reason we are partnering with Hospice Kingston to host this tournament and raise funds for their new residential hospice beds. For 30 years Hospice Kingston has provided comfort, support and companionship to individuals and families living with a life-limiting illness or coping with grief and loss. We are proud of our past and determined about our future. We are realizing our long-term goal to establish Kingston s first residential hospice. The choice to live one s last days in a residential hospice has been missing from the continuum of care in our community. With a homelike environment individuals in their last few weeks of life will have many of the comforts of home combined with access to specialized care and care will be holistic and patient and family-centered. Spaces will allow for privacy and intimacy while incorporating family and group activities, spiritual and faith based-traditions and quiet reflection. It will encourage multi-generational activities such as cooking, dining, visiting, reading, music, child and youth engagement of daily living. Building a new home for Hospice Kingston to incorporate a seven bed residential care program to its current programs and services will ensure a seamless transition in support and care to individuals and their families. The Time is Now to invest in quality hospice palliative care. The Time is Now to transform the end-of-life care experience in our community.

We support patients and families throughout their most challenging times, ranging from care following an acute incident such as a stroke or heart attack, ongoing care during cancer or dementia, and helping people through their most difficult times at end of life. Saint Elizabeth has helped to create advances in our system, to make healthcare work for patients including being the first to deliver chemotherapy and dialysis in the home. map not to scale From Kingston Take Bath Road/County Road 33 west to Bath. Hole Sponsor.............................................................. $650 Sponsor will receive their company name displayed on a Tee, in our tournament program and will have the opportunity to supply promotional items for the golfer gift bags and includes one foursome. Contest Prizes and Auction Table Prize Sponsorship Please include a business card to be included with your prize. Esther Warnock (1950-2014) a long service Personal Support Worker of Saint Elizabeth It is with great pride and admiration that we acknowledge the service that Esther provided to her patients as a Personal Support Worker at Saint Elizabeth for the past 10 years. Esther was always hard working and kind hearted. She exemplified the definition of honoring the human face of health care. She consistently showed commitment to Saint Elizabeth and her patients by generously going out of her way to make the needs of others important. Esther was always an example for her fellow peers supporting them as they needed it and she always had the best interest of the patients at heart.
