2 FOLDS of HONOR The Folds of Honor Foundation is rallying a nation to ensure no family is left behind in the fight to preserve American freedom. Through scholarships and other assistance, we give back to the spouses and children of soldiers killed or disabled in service to our country. We provide healing, hope and an opportunity for dreams to be realized...with the support of people like you. We feel this is our duty as citizens of the greatest country in the world. The Folds of Honor Foundation was founded in 2007 by Major Dan Rooney, a former F-16 pilot, golf course owner, PGA Professional and USGA member. A Major in the Air National Guard and a decorated military aviator, Rooney has served three combat tours in Iraq. After returning from his second tour, Major Rooney witnessed a profound situation that drove him to create the Folds of Honor Foundation. ABOUT FOLDS OF HONOR FOUNDATION The Folds of Honor Foundation is rallying a nation to ensure no family is left behind in the fight to preserve American freedom. There are currently more than 1 million dependents of military service men and women, 85% of whom do not qualify for educational assistance from the U.S. Department of Veteran Affairs and other governmental programs. Folds of Honor recognizes our government s efforts to support military service families, but also realize that government cannot succeed alone. Folds of Honor seeks to honor military families by providing educational support and opportunities to ensure that no one is left behind. We feel this is our duty as citizens of the greatest country in the world. ABOUT YEWELL INSURANCE/ROCKFORD MUTUAL Yewell Insurance was founded in 1950 by WWII Veteran Virgle Yewell. Current owner, Don Yewell, joined the business in Along with staff and family, they are proud to support the Folds of Honor Foundation and the families of the defenders of this nation. 3 FOLDS OF HONOR CHARITY CLASSIC Two Person Team Event to be flighted following play. Entry is not limited to amateurs or adults. Get your favorite wingman to partner with you for this event. Many of our participants choose to invite a veteran to play with them as a gesture of thanks for their service. This is a day to celebrate that you are Proud to be an American! 12:00 1:00 Lunch provided. (Lunch will be closed during the Proud to Be An American program) 1:00 Proud To Be An American Ceremony, Silent Auction, and other fun activities. 1:30 18 holes scramble Each player will receive a tee gift celebrating this great charity event. There will be special prizes for winning teams. Through the generous sponsorship of Yewell Insurance/Rockford Mutual Insurance, all proceeds from this tournament go directly to the Folds of Honor Foundation.

(Proud to be an American program at 1:00) We appreciate the support of proud Americans such as yourselves and as such are trying to make the entry and donation process as easy as possible while providing the proper documentation for recognition and tax purposes. If you are playing in the event.. Please complete the entry application. All blanks must be filled in to also include partner s information. $120 team entry must be attached to secure place in tournament. It is our hope that you would also make an additional donation to this worthy cause by using the supplemental donation form. If you are playing and will make an additional donation.. Please fill out the entry form AND the supplemental donation form. If you are entering the event and contribute $100 or more in excess of your $60 entry, it will qualify you for recognition on our Honor Board. If you are supporting the Folds of Honor with a donation. Please fill out the supplemental donation form and attach your donation. A donation of $100 or more will qualify you for recognition on the Honor Board and a personalized tee sign. Donations of $50 will qualify you for signage sponsoring a cart. See specifics on form.
