These services are provided in community health centres, residential care facilities and mobile service teams working in the client's home About the Role Community Transition Care provides short term multidisciplinary care for older people after a hospital stay in their own home to complete their recovery process and improve their functioning and level of independence.

Context This role reports to the Community Transition Care Team Leader for operational management and leadership and the CISS Director of Occupational Therapy for professional leadership. Purpose The purpose of this position within the Community Transition Care Team is to apply high level occupational therapy skills and knowledge in the assessment and treatment of patients/clients in the local geographical population in order to promote, maintain and improve their health, functional ability and quality of life. The Community Transition Care Service operates with extended opening hours 0800 hours to 1700 hours seven days per week including public holidays. Only nursing is required on weekends and public holidays. Further extension of operating hours may occur based on client and hospital and health service need. Key Accountabilities Clinical Practice Deliver high level occupational therapy clinical services across a variety of caseloads in a Community Transition Care setting in accordance with prescribed professional and ethical standards. Exercise independent judgement and problem solving skills, in matters beyond routine practice with infrequent clinical practice supervision. Apply professional judgement to a complex caseload, adapting routine clinical practice using valid and reliable evidence. Communication / Team Participation Provide high level advice with regard to occupational therapy service delivery to diverse stakeholders. Actively contribute to service and caseload management and planning within the team. Work cooperatively and effectively within the team and with patients/clients, their families, community groups, other health professionals, supervisors and students. Accurately and concisely record all patient interaction as well as workload measurement statistics, to facilitate the analysis of service trends. Leadership / Service Improvement Monitor and report on clinical practices and outcomes within clinical service area. Initiate quality, research and service improvement activities. Actively participate in initiating, developing and implementing quality improvement activities to enhance the delivery of occupational therapy services and multidisciplinary care. Provide clinical practice supervision, education and mentoring to less experienced occupational therapists. Provide educational and operational direction to assistant and support staff and supervision and education to work experience or observational occupational therapy students. Participate in and contribute to workforce planning to meet service requirements. Contribute to the development of clinical practices, procedures and protocol that support the continuum of care engaging cross-discipline referencing where possible. This position requires the incumbent to operate a C Class motor vehicle, and an appropriate Queensland licence endorsement to operate this type of vehicle is a mandatory requirement. Ability to apply high level, independent professional judgement to assess, plan, deliver and evaluate occupational therapy clinical services, including case management of a complex nature, where established principles, procedures and methods require some expansion, adaptation or modification. High level communication skills that enable development of improved service delivery outcomes and effective relationships with staff, patients and other key stakeholders. Ability to lead, initiate and evaluate local service improvement initiatives and provide clinical supervision to less experienced staff and students.
