Service/Department Area/Division/State Family and Carers Services - Austin COPES Organisational Initiatives/VIC Effective Date August 2015 Position Purpose: To provide services to clients, families and carers in line with Mind s Model of Recovery Oriented Practice and according to the values of Mind. The service type is COPES (Carers Offering Peers Early Support), which provides recovery oriented services to families and carers within an acute inpatient and community clinical setting. The COPES worker provides support, information and referral to families and carers to assist early in the journey of care. Service/Area/Division/State Information: The Carers Offering Peers Early Support (COPES) program in the North East catchment area is a collaborative partnership project between Austin Health Adult Mental Health Program and Mind that aims to deliver peer support, information and referral to carers and families whose relative or friend is in receipt of mental health treatment or support within the North Eastern mental health catchment area. The Peer Support Workers provide telephone and face-to-face support to carers to reduce isolation and improve knowledge of the mental health system. They are also be responsible for working with staff across the clinical mental health service to increase awareness of family and carer needs and raise the profile of the Austin COPES program. COPES support is provided primarily at Austin Acute Psychiatric Unit at Austin Hospital and may also be provided from time to time at other settings within the Austin service structure based within Heidelberg. The worker also provides carer co-facilitation to an evening carer support group in liaison with NEAMHS. PROJECT PARTNER Project partner, Austin Health is a leading provider of health care in the North East region of Melbourne. Austin Health s Mental Health Clinical Service Unit provides a range of inpatient and community based services including: Crisis Assessment & Treatment Service Mobile Support & Treatment Service Continuing Care Service Primary Mental Health Team North Youth Early Psychosis Service Acute Psychiatry Unit (APU) Secure Extended Care Unit (SECU) Community Recovery Program Prevention and Recovery Care (PARC) A joint Governance Group for the Austin COPES, is responsible for the overall direction and outcome of this service. 3 SECTION B: KEY RESPONSIBILITY AREAS The key responsibility areas (KRAs) are the major outputs for which the position is responsible and are not a comprehensive statement of the position activities. Key Responsibility Areas 1. SERVICE In line with Mind s Model of Recovery Oriented Practice: DELIVERY Support personal recovery and well being through the provision of personalised support services to clients and their families and: The development of trusting relationship; The development of strengths-based assessments and plans, with the client at the centre; Collaboration with others in the client s life, including relevant service providers; and Advocacy and referral. Participate in case conferencing and case reviews. Provide group support services; and/or Provide family and carer services; and/or Provide information and advice services. 2. SPECIFIC Provide peer support, information and referral to carers and families whose relative or friend is in receipt of services from NEAMHS or Mind. SERVICE Engage in phone or face-to-face contact with individual carers or families to provide REQUIREMENTS support, reassurance, information and referral. Assist families and carers to overcome isolation in their experience of dealing with a relative or friend who has a mental illness. Demonstrate appropriate knowledge of referral services, including Mind services, external agencies, carer support groups and education. Equip families and carers with some basic coping strategies Liaise with Mind and NEAMHS staff in organising and responding to COPES program referrals. Uphold the dignity of carers, families and consumers; and respect confidentiality. Co-facilitate NEAMHS Family/Carer support group Maintain accurate records on carer contact, for the purposes of monitoring and evaluation of the program. Assist in the general review and evaluation of the program. Work with staff to advocate and educate them about carer needs and issues Build regional relationships and works collaboratively with carer organisations & clinical services to enhance service reputation & referral rates Represent the COPES program as required 3. TEAM WORK Work effectively and cooperatively as a member of the team, in accordance with the values of Mind. Support the Service Manager and all other staff to provide a consistent approach to services to clients and their families. Actively participate in team meetings. 4. DATA Ensure that all Client File information is entered in a timely manner. MANAGEMENT Meet all data entry requirements for MindLink. Meet all other administrative and reporting requirements to ensure a high level of efficiency. 5. PROFESSIONAL Undertake relevant training and professional development, including regular DEVELOPMENT supervision, appropriate to the primary work of the service and Mind. Participate in reflective practice. 6. WORKPLACE HEALTH AND SAFETY Contribute actively to the maintenance of a safe workplace. Proactively support Mind s vision of supporting people facing mental health challenges to live well and be socially included, in accordance with the Mind values. 8. OTHER DUTIES As delegated by the Service Manager. SECTION C: CORE REQUIREMENTS Technical 1. Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind. 2. Must have experience as an unpaid carer / family member for an adult with a mental illness who has been treated within the public mental health system. 3. Current valid Driver s Licence. 4. Current National Police Record Check. 5. Current Working with Children Check. 6. Not listed on the Disability Worker Exclusion Scheme list. Experience / Knowledge / Attributes / Values 1. Knowledge and understanding of the impact of serious mental ill-health and recovery and well being and capacity to work with people with multiple support needs. 2. Commitment to proactively support Mind s vision, purpose, values and goals. 3. Demonstrated commitment to work within the parameters, values and principles of Mind s Model of Recovery Oriented Practice 4. Demonstrated understanding of the principles of peer support and counselling. 5. Experience and ability to utilise own experience constructively for the benefit of others 6. Demonstrated ability to engage individuals and or families through telephone or 1:1 support in an empowering and ethical manner 7. Ability to work with individuals, groups and families and carers of people with mental health issues. 8. Strong interpersonal and team skills and behaviours, incorporating verbal and written communication, flexibility and resilience. 9. Skills and experience in advocacy, collaboration, facilitation, evaluation and problem solving. 10. Computer literacy skills to enhance and maximise service reporting and measurement. SECTION D: CONDITIONS OF EMPLOYMENT This position is a part-time fixed term contract working 15.2 hours per week until 30 June Working days will be negotiated with Mind and NEAMHS worked between the hours of 8.00 am and 6.00 pm as required. Please note that the work pattern may be subject to change to meet the needs of the service. Salary classification range for this position is from CMHP level 2/1 CMHP Level 2/4 depending on experience and qualifications. You are able to salary package a tax free amount, which is currently up to $15,899 per FBT year, plus other items including meal and entertainment and accommodation. The amount available for salary packaging will be subject to Commonwealth legislation as it applies from time to time and on the basis that Mind will not incur any liabilities for FBT. Mind offers salary packaging through an outsourced provider (currently EPAC). Other conditions are as per Mind s industrial agreement. Employer superannuation contribution will be paid as legislative required.
