2 Role description Swanswell s Nurse Clinical Lead supports our Service Manager to deliver quality services and go the extra mile for our service users. You ll also contribute to ensuring services are delivered in line with Swanswell s policies and contractual requirements. You ll work alongside the team, helping service users to deal with their substance misuse and physical health so that they can be well and stay well. We re looking for talented and motivated people to work with our managers and workers in West Berkshire based in Newbury and covering West Berkshire area. You ll also have the opportunity to contribute to the strategic aims of the organisation by managing projects across the whole organisation. Under the supervision of the Operations Manager, you will carry out all aspects of the role within Swanswell s policies and procedures, including our Equal Opportunities Policy. Salary This is a full-time post earning up to 35,225 per annum. Hours of work This post is for a 37 hour week. You will be required to work bank holidays and some weekends in order to meet our service delivery requirements. Swanswell operates a flexible working system, so you ll be able to work flexible hours based on service needs. Annual leave Swanswell s holiday entitlement is 36 days per holiday year including bank holidays and statutory public holidays. This will be a pro rata amount for part time and fixed term contract employees. Transport This role will require you to regularly travel to appointments in the community and other Swanswell offices, so a current driving licence and own transport would be an advantage. 2

3 Key activities and responsibilities Take a lead role on all aspects of Swanswells clinical governance and provide evidence of best practice through clinical audit and competency assessment, development and management Work closely with Swanswells clinical governance team Ensure that Swanswell s clinical governance framework underpins all working practices within the team. Work effectively with partner agencies to deliver a quality service, focussed on achieving positive outcomes for service users. Ensure that all incoming work/referrals are dealt with promptly and in line with relevant policies, priorities and quality standards. Carry out comprehensive and detoxification assessments in order to identify the precise needs of clients and formulate an initial care plan. Assess and recommend adults for their suitability for detoxification (community or inpatient) as appropriate. Support individuals through detoxification programmes. Prepare clients for planned discharge from treatment and manage the closure of individuals from detoxification programmes. Undertake brief interventions with service users, providing support through a range of treatment models. Work with service users to draw up their individual care plan and provide support/treatment according to the care plan. You ll regularly review the care plan with the service user alongside recovery workers to make sure it is still meeting their needs. Take all reasonable steps to ensure the safety of service users, relevant others (particularly children and vulnerable adults) and members of the community. Take a lead role in developing clinical services within Swanswell West Berkshire which provide easily accessible, non-judgemental, and confidential services, which support service users in reducing harm, assessing need, planning care and reviewing progress 3

4 Developing and coordinating effective clinical pathways into community healthcare services, supporting the needs of service users Lead and coordinate complex care meetings, providing management and supervision to the staff team. Communicate effectively with service users and carers, actively listening to understand the needs of service users and their carers/families. Carry out health monitoring and administer screening and tests, including taking blood samples (venepuncture) in accordance with agreed protocols. Carry out BBV Testing and Hepatitus B Vaccinations, including onward referrals where appropriate Carry out all aspects of the role cost effectively and in line with Swanswell s policies, the requirements of funders and all relevant clinical standards/frameworks. Work unsupervised within own professional guidelines and training, drawing on support from others where necessary and appropriate. Support the delivery and development of an integrated substance misuse service, working closely with clinicians and a multidisciplined staff team. Contribute to the provision of prescribing services across the region Develop and support staffs clinical abilities alongside the Team Leader and Service Manager Work at a range of locations where service users are found, including other health premises and in their homes. Utilise psychosocial intervention, including ITEP, in both one-to-one and group settings to promote engagement in treatment services and ownership of Recovery Planning 4

5 Maintain high standards of office security, confidentiality and administration. This includes ensuring the safe and secure keeping of files on Swanswell premises and any premises used by Swanswell. Work therapeutically with adults with a mental health issue, joint working when required with other professionals as part of a wider care plan. Offer wound care assessment and basic wound dressing to clients. Organise signposting to other services as required, and ensure clear pathways into other treatment arenas as appropriate. Pro-actively maintain an up to date knowledge of medicine management and both local and national guidelines and agreements. Develop and participate in relevant training, coaching and mentoring sessions, as required. Keep up-to-date with the latest developments in the field and contribute to the continuing development of Swanswell s services. Support and represent the Service Manager, as required. Adhere to Patient Group Directions and ensure requirements on updating are maintained Ensure the smooth transition of service user s clinical needs (including prescribing) into the community shared care Participate in the development, management and review of both existing and new initiatives including projects and posts. Carry out other duties appropriate to the role, as necessary or requested. Nurses who are existing independent non medical prescribers (NMP) will be able to assess, diagnose and prescribe (as per locally agreed protocols) for service users on occassion. Where required ensure you remain up to date with the knowledge and skills that enable you to prescribe competently and safely 5
