1 Tree Permit Prcess Intake: 1. Applicants may btain the Tree Permit Applicatins frm the Envirnmental Resurces Divisin in the Planning and Zning Department lcated at 444 SW 2 nd Avenue, 3 rd Flr, Miami, FL Applicatins may als be dwnladed frm the Planning and Zning website at 2. The cmpleted tree permit applicatin, alng with the supprting dcumentatin required, shall be submitted t the Envirnmental Resurces Divisin. A nntechnical cmpleteness review is cnducted f yur applicatin and cnstructin plans/dcuments with the bjective f identifying any missing, critical elements prir t the actual start f the frmal review prcess. If elements are missing, yu will be advised f any missing plans/dcuments. a. Checklists (attached) have been designed t list the required supprting dcumentatin needed fr cmpliance. The fllwing Checklists shall be used fr the belw listed prjects: New cnstructin with the remval, relcatin r rt pruning f trees (A) New cnstructin with n tree remvals, relcatins r rt pruning (B) Tree remval, relcatin, rt pruning, trimming withut cnstructin (C) Tree planting in the City Right-f-Ways (D) (A cmbinatin f checklists may be applicable depending n the scpe f wrk) 3. An applicatin/prcess number is issued and the applicant (n file) is ntified via . Applicants can check n the review status via the internet using the prcess number issued. Review: 4. Upn receipt f the tree permit applicatin, the department shall review said applicatin fr cmpliance. The review shall include a field inspectin f the site and referral f the applicatin t ther departments r agencies, as necessary, such as Planning Envirnmental (PA, PE r PH). Nte: The prpsed scpe f wrk f the tree permit applicatin is reviewed in accrdance with Zning and Public Wrks review and cmments fr the new cnstructin. 5. Once the review is cmpleted, the applicant (n file) is ntified f the review results and the Department shall issue a ntice f Intended Decisin. This ntice is psted nt the subject prperty r adjacent t fr ten (10) calendar days which prvides all interested parties the pprtunity t review and appeal the Intended Decisin. Issuance f Permit: 6. If n appeals are received within the appeal perid, the applicant (n file) is ntified and a receipt is generated fr payment. 7. Once payment is made, the permit is issued accrdingly. Final Inspectin: 8. The final inspectin shall be cmpleted n mre than six (6) mnths frm the apprval date f the tree permit. This inspectin shall be apprved prir t requesting a Temprary Certificate f Occupancy (TCO) r Certificate f Occupancy (CO) frm the Building Department. 9. T request the final inspectin, the applicant shall call the number indicated n the permit card r the Envirnmental Resurces Divisin at The inspectin is cnducted within hurs f the requested date. 11. Written statement - frm prperty wner r authrized representative indicating the reasn(s) fr the wrk Three (3) signed and sealed sets f the fllwing plans: Tree/Bundary survey drawn t scale, identifying the tree species, lcatin, and listing the height, spread and diameter f all existing trees including all trees within the right-f-way. Plan shall illustrate the lcatin f all existing structures and any verhead and/r undergrund utilities. This survey shall be prepared by a prfessinal Land Surveyr, licensed in the state and dated n mre than 12 mnths. The Base Building Line, establishing the prperty bundaries, shall be marked n said Bundary Survey by the Building r Public Wrks Department. Dispsitin Plan / Site Plan- drawn t scale, identifying and listing all existing trees and specifying the cnditin f each tree and whether said trees are t remain, t be remved, r t be relcated. The tree dispsitin plan shall superimpse all prpsed new cnstructin. Plan shall include lt dimensins, Center Line, Mnument Line and adjacent street names Tree Replacement/Landscaping Plan - drawn t scale, illustrating all prpsed new cnstructin, new lcatins f relcated and newly installed trees. Said plan shall include lt dimensins, Center Line, Mnument Line and depict adequate tree spacing between trees, hardscapes and utility lines. Tree requirements fr bth Miami 21 landscape requirements as well as Chapter 17 tree replacement requirements shall be included and depicted n the Landscape Plan t ensure bth requirements are met. If minimum tree requirements are nt prvided n-site, applicant must meet alternative requirements via ff-site replacement r Tree Trust Fund Tree Prtectin / Planting / Maintenance Plan identifying all trees t remain n site with the apprved tree prtectin plan and prtectin barrier detail, as guided by a Certified Arbrist. Said plan shall als include the details n newly installed planting materials and a ne (1) year maintenance guarantee. *All plans shall be prepared by and bear the seal f a registered Landscape Architect currently licensed in the state r by persns authrized by F.S. ch. 481 t prepare landscape plans r drawings. Certified Arbrist Reprt prviding an assessment n all existing trees with the trees specificatins (cnditin, species, size, etc.) and includes clr phtgraphs in supprt f the assessment.
