In rder t submit nline the applicant wuld require the fllwing infrmatin t retrieve Old Passprt Number n OCI card / U-Visa N / OCI Regn number / OCI file number / Date f Birth / Place f Birth / Mther s name. * Please ensure that all infrmatin is filled in carefully and tallies with the supprting dcuments (e.g. Indian passprt, Landing Certificate and Canadian passprt). II. The signature f the applicant shuld be inside the bx left t the pht and at ther Places requiring signature. * In case f minrs wh cannt sign please use left thumb impressin fr bys and right fr girls. Please Use nly blue r black ink fr the thumb impressin. Fr the minr applicant bth parent meet sign the Part B sectin even if nt applying tgether. III. ONE phtgraph f size 51mm X 51mm fr each applicant t be pasted n Part A f the applicatin. Phts shuld be f gd quality high reslutin with 80% cverage f the face. If parents have never landed r applied f Permanent Residence status in Canada need t prvide Ntarised Affidavit frm Canada. In case applicant is unmarried / Adult: Self signed declaratin letter t be prvided. * In case f minr applicant: Parents recrd f landing and parents landed Indian passprts are required if the applicant is belw the age f 18. *Bth parents Canadian citizenship certificate mentining the exact date f acquisitin (i.e., mnth, day and year must all be included) f citizenship.

The applicant shuld als submit bth parents Indian and Canadian passprts alng with their recrd f landing and citizenship certificate. * If the applicant is brn utside f India but held an Indian passprt Statement f Live Birth is nt required. In case the name f the applicant mentined in the Indian passprt, recrd f landing and the Canadian (r Freign) passprts differ (i.e. whle r part f the name/middle name r surname has been added/drpped r changed) a duly ntarized affidavit explaining the difference in the name and the dcument reference shuld be prvided. In case the name has nly been changed n the Canadian passprt and nt n ther relevant dcuments name change certificate wuld be required. * In case f change f surname f wmen after marriage, a detailed affidavit (mentining the details f spuse, date f marriage, place f marriage and name befre marriage and after marriage) alng with riginal marriage certificate shuld be prvided. * Fr Vancuver Jurisdictin the ntarized affidavit shuld als state that bth the names are f the same persn and that the applicant shall be respnsible fr this statement. Ensure that the Date and Place f Birth shuld be same in all the dcuments (Indian passprt, Recrd f landing and Canadian passprt). * Only Fr Vancuver Jurisdictin in case the details differ n these dcuments a duly ntarized affidavit explaining the difference in the details is required.
