1 CHESTERFIELD ROYAL HOSPITAL NHS FOUNDATION TRUST JOB DESCRIPTION POST: BASE: Information Governance Assistant Chesterfield Royal Hospital NHS Foundation Trust SALARY RANGE 16,271-19,268 per annum pro rata (Band 3) REPORTS TO: ACCOUNTABLE TO: Information Governance Officer Head of Health Informatics JOB PURPOSE: Information Assistants are responsible for the production and verification of clinical and non-clinical information to professional standards, under the direction of senior departmental staff. The post has specific responsibility for providing information to defined areas as well as providing a range of other activity information. This includes other routine assigned work, cover for colleagues in their absence and supporting the quality assurance programme. KEY RESULT AREAS: 1. Contributes to providing data for internal and national monitoring. It is crucial for this information not only to accurately reflect the actual position, but also to conform to national data definition standards and guidance. Failure to do this may unnecessarily penalise the Trust in being assessed against those targets. 2. The post supports the production and verification of detailed reports in a specified clinical or non-clinical discipline, under minimal supervision. 3. To ensure the highest possible quality of completeness and accuracy is achieved at all times in delivering personal work, while continuing to meet demanding deadlines 4. Ensure all information produced adheres to all relevant local and national standards, particularly with regard to collection of data, coding definitions, quality assurance, presentation, etc 5. Ensure that departmental quality assurance processes are applied to work, as appropriate

To produce information for performance monitoring and commissioning purposes, to CCG s / DH and other NHS organisations via SUS and other routes Routine information flows with NHS partner organisations 7. Maintains a detailed knowledge of processes outside normal area of specialisation, supporting colleagues when covering absence and providing genuine quality assurance 8. Provides detailed support to senior managers on the interpretation of the trusts activity needed to achieve key targets and monitor performance GENERAL RESPONSIBILITIES 1. Ensure that proper documentation is available for all designated processes and procedures 2. Analysis of ad hoc queries and, using SQL and other query tools, generate appropriate reports in response Data is principally derived from the following systems. Patient Administration System (PAS) (includes inpatients, outpatients, waiting list, A&E, clinical modules etc. HIPPO Data Warehouse Agreed departmental systems 3. Maintenance of up to date knowledge of information resource and national requirements 4. Provide all necessary information reports and analysis for designated activity areas, ensuring that reports are accurate, complete and produced within preagreed deadlines 5. The post holder should have knowledge and experience in technical data handling and manipulation skills including: Relevant experience in information management within the NHS, underpinned by a theoretical understanding of relevant issues Detailed knowledge of and skill in using software query tools including SQL, AQUIP, TM1 and standard MS Office (Word, Excel & Access) products Intermediate knowledge of the NHS Data Manual and Data Set Change Notice (DSCN) process Intermediate knowledge of Trust data collection systems, particularly PAS and the HIPPO data warehouse system Intermediate knowledge of Trust operational processes, particularly relating to data collection and usage

3 GENERAL RESPONSIBILITIES 1. To maintain patient confidentiality at all times. 2. To actively participate in the staff appraisal process. 3. To be familiar with relevant Operational, Personnel, Health and Safety Policies and procedures, including Fire, COSHH, No Smoking and Alcohol. 4. It is the responsibility of every employee to actively help the Trust prevent and control infection by washing their hands effectively, carrying out their duties in a way which minimises infection, attending appropriate training and maintaining up to date knowledge and skills in infection control and prevention as relevant to their post. This job description acts as a guideline to the responsibilities of the post holder and will be reviewed in consultation with the post holder and amended to meet changing professional and service needs.

Verification of I.D. documents Applicants must provide documents that satisfy one of the following options: Two forms of photo I.D. + one document confirming current address (dated within last 3 months) One form of photo I.D. + two documents confirming current address (dated within last 3 months) Two forms of non-photo I.D. + two documents confirming current address (dated within last 3 months) + a passport sized photo of self with a letter from a person of standing who can confirm they have known you for at least 3 years and in what capacity. Full UK birth certificate Full UK (old style) paper driving licence Adoption certificate Marriage/civil partnership certificate Divorce/annulment/civil partnership dissolution papers Deed poll certificate HM Revenue and Customs correspondence (dated within last 12 months) P45 or P60 are not acceptable Grant letter or student loan agreement from a Local Education Authority Benefit statement, book or card (dated within last 12 months) Address Utility bill (dated within last 3 months) Council Tax bill (dated within last 12 months) Old style non-photo driving licence HM Revenue and Customs correspondence (dated within last 12 months) P45 or P60 are not acceptable Financial statement e.g. bank/credit/store card statement (dated within last 3 months) - Online printouts are not acceptable.
