1 JOB DESCRIPTION Job Title: Registered Nurse Palliative Care Salary: NHS Band 5 equivalent ( 21,388-27,901) Reports to: Responsible to: Hours /Annual Leave: Base: Lead Nurse for Outreach Services Head of Outreach Services Full Time (37.5 hrs) / 25 days + Bank holidays The Norfolk Hospice Tapping House Role summary: This is a new role designed to work initially with our Hospice-at-Home Team supporting patients to achieve their preferred place of care, and supporting their relatives and carers. Additionally the post-holder will be expected to work flexibly across our other care services to develop their knowledge and expertise caring for patients within our hospice facilities. Our current care services and families will be expanding to incorporate an inpatient unit at our new hospice currently under construction in Hillington, West Norfolk, PE31 6BH. Job purpose: To work as part of the multidisciplinary / multi-agency team providing co-ordinated, individualised, evidenced-based patient care and psychological support; plus support to families and carers in accordance with the hospice palliative care philosophy. To act as a role model for junior staff in delivering high standards of specialist nursing care and with the support of the Junior Sister / Charge Nurse, working within a framework of evidence-based care to develop practice. Duties: To work as a member of the multidisciplinary team providing specialist evidence based nursing care to patients and to support families and carers working on a duty rota which includes internal and community-based rotation.
2 To assess, plan, implement and evaluate patient care applying an holistic, patient centred approach and to continually review and update nursing care plans and documentation. To evaluate documentation of self and others and ensure that at all times it is accurate, legible, appropriate and complies with NMC guidance and Care Quality Commission requirements. To ensure that effective communication is established with all disciplines and external agencies and to patients and carers. To ensure that confidentiality is maintained at all times when communicating complex or sensitive information to patients and families. To develop and display a high level of clinical and technical palliative care expertise which will include drug management and use of syringe drivers acting always within NMC guidelines and Hospice policies, procedures and guidelines. To assist in maintaining an environment in which staff work, patients are cared for, and relatives visit, which complies with current Health & Safety legislation, taking appropriate action within the limits of their responsibility or reporting incidents or concerns to the Head of Outreach Services or Facilities Manager. To bring to the attention of the Head of Outreach Services any accident or untoward incident, including near misses, and to deal with palliative care emergencies as they arise. To provide advice, within the limits of their knowledge and skills, to health professionals from other organisations. To contribute to effective communication within the team, and with external agencies, by participating at meetings as directed by the Head of Outreach Services. To take responsibility for own personal development and maintain continuing professional development through individual development review and clinical supervision. To participate in the development of practice through clinical audit. To undertake other duties commensurate with the role as directed, from timeto-time, by the Head of Outreach Services.
3 Physical, mental, and emotional skills required Physical skills required include a range of dexterity and sensory skills necessary for patient care and well-being, e.g. Moving and handling of patients and equipment in a safe manner Fine motor skills to manipulate equipment requiring accuracy e.g. syringe drivers Able to organise own time and that of other, more junior staff within the team Able to initiate liaison with other members of the multidisciplinary team and external agencies and supervise others on a day-to-day basis in the maintenance of care standards Able to use own judgement to analyse situations and develop plans for routine activities Able to form positive relationships with colleagues, patients, and carers to communicate complex and sensitive information and provide reassurance Capable of working in an environment which may involve long periods of standing and walking with frequent requirements to exert moderate physical effort for several short periods during a shift There is frequent requirement for concentration when handling and administering drugs / other treatments, when communicating sensitive and complex information to colleagues, patients and relatives and, on occasion, where the work pattern is unpredictable Responsibilities of the post-holder Responsible to the Head of Outreach Services, the post-holder will be required to plan and organise, on a day-to-day basis, a range of care activities to ensure effective delivery of care to patients and psychological support to relatives. To participate as part of the multidisciplinary team in the development and delivery of patient care programmes To maintain a safe environment within the workplace, particularly with regard to manual handling, infection control and the administration of medicines ensuring at all times adherence to Hospice policies To take responsibility for own continued personal and professional development through participation in individual development review, clinical supervision and other educational activities To contribute to audit and research activities as required by the Head of Outreach Services.
4 Freedom to act The post-holder will work as part of the multidisciplinary team under the indirect supervision of the Head of Outreach Services and operate within the standards, policies and procedures of the Hospice and the NMC Code of Conduct. The post-holder will have access to medical advice. Outline of Working Conditions Community working, including attending patients in their domestic / residential settings Occasional exposure to bodily fluids and blood Frequent contact with distressed patients and relatives, exposure to patients who are agitated and confused; and occasional exposure to aggressive patients or relatives Potential to work within an inpatient unit which complies with the Hospice Health & Safety Policy Internal Communication To promote at all levels The Norfolk Hospice Tapping House vision, values and strategic objectives. To promote and maintain effective channels of communication at all levels of the organisation in order to ensure that the main purpose of the post can be achieved. Hospice Policies To keep abreast of, and work in accordance with, Hospice policies as outlined in policy files, both paper and electronically. Health and Safety The post-holder has responsibility to take reasonable care of self and others in relation to managing risk, health and safety; and will be required to work within the appropriate policies and procedures.
