2 Pharmacy Services JOB TITLE: BAND: HOURS: REPORTS TO: ACCOUNTABLE TO: Level 2 Pharmacy Apprentice (Assistant) Apprentice 37.5 (plus weekends and bank holidays as per departmental rotas) Training Team Leader Chief Pharmacist Job Purpose To develop the knowledge, skills and competencies of aseptic drug compounding, dispensing, storage, distribution and prescription processing, and to implement these in practice, meeting the requirements of current legislation, professional standards and ethics, and pharmacy department performance targets, to achieve a QCF Level 2 and underpinning knowledge in Pharmaceutical Sciences to complete the Apprenticeship framework. Key Result Areas 1. To dispense prescriptions in line with Standard Operating Procedures, ensuring that standards of quality, accuracy and timeliness are met. 2. To dispense Controlled Drugs to ensure that the requirements of the Misuse of Drugs Act (1971), RPSGB standards, and departmental Standard Operating Procedures are met. To document receipts of Controlled Drugs into the controlled drug register as soon after receipt as possible. To ensure that registers are kept accurate at all times, reporting any discrepancies to the Dispensary Team Leader. 3. To aseptically prepare and supply, under the supervision of a pharmacist, intravenous injections including cytotoxics and other aseptically dispensed items in unit dose form, with doses tailored to the individual patient, and presented as a quality product, produced to a standard which ensures patient safety and those who administer it. 4. To operate and maintain safe systems of working practices within the cytotoxic and aseptic dispensing suite in order to ensure the highest possible standards and to comply with the COSHH regulations (Control of Substances Hazardous to Health). 5. To monitor the environment and equipment within the department to determine that they conform to agreed standards, and carry out operator validation tests so that the quality of products is assured. 6. To assist with the cleaning and maintenance of the department, equipment and clothing, and to maintain a clean working environment, ensuring all items and documentation are stored appropriately in designated places, workstations and walkways are kept clear. Ensure all SOPs are followed to achieve professional and Health and Safety requirements. 7. To be responsible for the ordering and stock control systems for both non-stock and stock consumables used within the Aseptic Unit to ensure adequate stock levels are maintained.
3 8. To ensure all drug transactions are entered accurately onto the Pharmacy Computer System to minimise stock discrepancies and drug shortage situations. 9. To be responsible for the maintenance of drug storage and rotation of stock, including Clinical Trials. Complete stock checks and perform expiry checks to minimise financial loss and maintain stock holding. 10. To provide a drug and fluid top-up service to wards and departments, reviewing and adjusting stock levels (including controlled drugs) in liaison with pharmacists and ward managers, to ensure adequate stock holdings. To ensure that all stock is put away appropriately. To ensure that ward stocks are rotated, expired stock is removed and any related storage concerns are reported to the ward sister/nurse in charge. Some of this work will require the postholder to work unsupervised. 11. To respond to requests regarding stock levels of medication from ward staff which may mean overcoming some barriers of understanding of the service provided. 12. To assemble orders received from all sources ensuring they are booked out on the computer and charged to the correct department, picked promptly, accurately and checked, and are securely packed for delivery in accordance with departmental procedures. 13. To maintain and review drug to-follows to ensure an effective system of drug supply, through effective communication with relevant pharmacy and ward staff. 14. To have specific responsibility for the management of a particular area of stores eg management of stock, care trays, cardiac arrest boxes. 15. To assist with returning drugs via the computer system including returned stocks and disposing of expired or unwanted items, in accordance with departmental procedures, maximising drug usage and financial resource. 16. To assist in the receipt of all goods, including recording all receipts, issues, batch numbers, expiry dates and other relevant information, onto the pharmacy computer system ensuring that all transactions are entered promptly and accurately in order to maintain accurate departmental stock. To assist in checking stock levels and expiry dates to minimise stock-outs and waste. 17. To assemble drugs in cardiac arrest boxes recording batch numbers, expiry dates, ward usage and other relevant information in accordance with departmental procedures. 18. To provide a safe and secure medication collection and delivery service to wards and departments, where appropriate, in accordance with departmental procedures and standards 19. To initiate orders for and maintain a continuous supply of medicine bottles, containers, bags, spoons, computer labels and other consumables in the Dispensary. 20. To perform receptionist/shop duties as required, receive prescriptions, process dispensing fees, handle cash and enter prescriptions onto the pharmacy tracking system according to departmental procedures.
4 21. To maintain standards of safety, tidiness and cleanliness within the department in accordance with COSHH and Health and Safety regulations. To ensure all working practices and Standard Operating Procedures are followed. 22. To assist the Senior Technician, Stores and Distribution in the co-ordination of the Drug Hazard/Recall procedure in the event of a drug recall, where appropriate. 23. To participate in weekend and bank holiday work in accordance with departmental rotas. 24. To undertake such other reasonable duties as may be required from time to time. GENERAL 1. To carry out the duties placed upon employees by Health and Safety at Work legislation. 2. To take reasonable care for the health and safety of themselves and of other persons who may be affected by their acts or omissions at work. 3. To co-operate with their employer/responsible officer as far as is necessary to meet the requirements of the legislation. 4. To not intentionally or recklessly interfere or misuse, anything provided in the interest of health, safety or welfare in the pursuance of any of the relevant statutory provisions. 5. All employees are subject to the requirements of the Health and Safety at Work Act, with particular reference to ensuring that their work methods do not endanger themselves or other people. 6. The prevention of hospital-acquired infection is a vital concern for the Trust. Infections harm patients. Infections also harm the Trust. The post holder is required to ensure, as an employee, that his/her work methods do not endanger other people or themselves. All staff must be aware of infection prevention and control policies and guidelines, and follow them at all times. Any breach of infection control policies is a serious matter and may result in disciplinary action. 7. All employees are also subject to the stipulation of the Data Protection Act and the Trust s confidentiality policy. 8. 6 CLINICAL SERVICES DIRECTORATE PHARMACY SERVICES JOB TITLE: BAND: Pharmacy Apprentice Level 3 (Technician) Apprentice HOURS: 37.5 REPORTS TO: ACCOUNTABLE TO: Training Team Leader Chief Pharmacist JOB PURPOSE: To develop the knowledge, skills and competencies of drug compounding, dispensing, storage, distribution and patient counselling, and to implement these in practice, meeting the requirements of current legislation, professional standards and ethics, and pharmacy department performance targets, and achieving a QCF Level 3 and underpinning knowledge in Pharmaceutical Sciences to complete the Apprenticeship framework. RESPONSIBILITIES: 1. Participate in dispensing individual prescriptions for inpatients and outpatients, including clinical trials and controlled drugs, meeting legal and ethical requirements and department standards. 2. Develop knowledge and skills of patient counselling to meet required competencies for this activity by the end of the training period. 3. Participate in and develop the knowledge and skills of making special sterile and non-sterile products and made to order products for individual patients to achieve the required competencies for unsupervised dispensing by the end of the training period. 4. To determine and compile ward stock lists in conjunction with ward sisters and pharmacists, in order to carry out a topping up service to a number of wards, thus ensuring adequate stock holdings in ward areas. Check ward stocks on designated wards on a regular basis and to monitor and update the stock-lists. Generate stock orders under the topping-up service to clinical areas. 5. Ensure personal working practices, behaviour and use of any equipment or facilities meet professional and Trust/department Risk Management and Health and Safety policy requirements to minimise harm to patients, colleagues or self, and damage to buildings or equipment. 6. Participate in the effective planning and organisation of dispensary work, making suggestions for improvement and identifying and completing tasks which contribute to the increased effectiveness of the service.

Maintain a clean and tidy work environment, ensuring items used or documentation are always stored in their correct place, requests are placed for items due to run out and no work is left unfinished. To participate in checking stock levels and expiry dates in the department. 8. Make all necessary records and complete all necessary documentation to ensure requirements are met and work progresses smoothly. 9. Participate in the processes for drug receipt, storage, distribution to wards, departments and outside hospitals, learning, understanding and meeting the legal requirements and professional and departmental standards for these. 10. Assemble cardiac arrest boxes and emergency drug kits as required. 11. Develop a working knowledge of pharmacy reference textbooks and other information sources and utilise these alongside practical skills to increase knowledge of information relevant to the job and its correct use, keeping a continuous record of achievements and evidence of skills appropriately applied within stated requirements of the training programme. 12. Develop and implement skills in effective verbal and written communication, making contributions to the team and supporting and helping other team members to achieve an increase in overall team performance and fewer work errors or duplication through poor communication. 13. Take part in Department and Hospital training initiatives necessary to develop required skills and for successful completion of the training programme. 14. Participate in the provision of extended hours cover including weekdays, Saturdays and Sundays on a rotational basis and Bank Holidays when required.
