Coordinators from ADHB and Counties Manukau - Members of other Mental Health Teams within WDHB Purpose : - To provide a professional, quality social work service to clients of Regional Forensic Psychiatry Services and their families/whanau, working within a Recovery focus. 2 - To maintain an effective working relationship with all Members of the multidisciplinary team. KEY TASKS 1. To practice as a social worker, providing a professional social work service to clients families/whanau and multidisciplinary team. EXPECTED OUTCOMES Provide focussed social work assessment and interventions with clients. Social work interventions and outcomes to be recorded in HCC in a timely manner. 2. To actively contribute to client care plans, as directed. Social work reports to be provided and updated as required. Social work issues for clients are highlighted to the clinical team, to encourage understanding of the social and cultural background of the client. 3. To work as an effective team member. Actively participate in the multidisciplinary team to achieve integrated outcomes for clients. To contribute to policy and clinical meetings. Attend clinical review meetings and present social work issues, as relevant. Demonstrate understanding of Recovery principles. Maintain active and timely communication between the team and relevant agencies, as required. To be actively involved in discharge planning. To fulfil those responsibilities as deemed appropriate by manager. 4. To be conversant with relevant legislation and WDHB policies pertinent to social work To advise clinical team re process and initiate applications for Protection of Personal and role and Regional Forensic Psychiatry Property Rights Act Services requirements. To provide supervision to social work students on placement within RFPS, as advised by senior social worker and team manager. To demonstrate flexibility regarding willingness to work in a variety of forensic settings, as directed by senior social worker and management. To participate in client education programmes and groups across the service. Professional issues are addressed by consultation with Professional Advisor. To maintain professional affiliations. Membership of Aotearoa New Zealand Association of Social Workers will be encouraged. To maintain Social Work Registration, as per WDHB policy and ensure that Annual Practising Certificate remains current. WDHB s commitment to biculturalism is honoured.
