To provide cover across the Inpatient Service (NSH & WTH) and into the Community setting as required. Undertake and complete Psychosocial assessments with patients and their families/whanau. The assessment would note the patient s relationship to the physical, social, emotional and spiritual dimensions of their lives. It would include supportive factors and risks for the patient in their current situation and on discharge. It would also note other concerns impacting upon patients/clients and their families/whanau. Referrals are received, recorded and responded to as early as possible. 3 Contribute to the provision of Social Work services during weekends and Public Holidays. All social work practice is ethical and of a high quality standard. Provide social work interventions to attend to risks or challenges, identified through the psychosocial assessment process, with the patient or their families/whanau. Consultation with and referral to other interdisciplinary team members, community health professionals and other appropriate community agencies. To work as a key member of the interdisciplinary team to facilitate appropriate care and support of patients/clients and their families. Attend and contribute to family meetings for patients/clients and their families. Maintain effective collegial relationships with NASC and other disciplines as a team member involved in the management of the health of patients/clients Ensure that patients and/or families/whanau identifying as Maori are offered the services of He Kamaka Waiora Clients and their families from different ethnic and/or cultural backgrounds are offered appropriate cultural services. Maintain an up to date knowledge of networks, services and other resources available and beneficial to the client/patient and/or their family/whanau. Participate in clinical, management and other meetings relevant to the delivery of social work services. Contribute to group work for specific rehabilitation groups as required. Maintain such clinical, administrative and statistical records of social work interventions and outcomes as are required. Participation in weekend and public holiday rosters. The social worker is registered and adheres to the ethics and standards of Aotearoa New Zealand Association of Social Workers and the New Zealand Social Work Registration Board Code of Conduct. Interventions are timely, appropriate, strengthbased and in accordance with ANZASW Standards of Practice and the Social Work Registration Board s Code of Conduct. Patients receive services, and referral to external agencies as indicated to meet assessed needs in a timely way that ensures continuum of care. Interventions are based on a social work care plan and the client goals using clinical reasoning. Interventions are evaluated regularly and discussed at social work and interdisciplinary meetings. Patients/clients and their families are fully supported to make informed choices regarding their discharge and ongoing care needs. Social work contributes fully with other clinicians in the care of patients/ clients. Consultation with other interdisciplinary team members occurs. Maori have access to and can receive interventions from Maori cultural services Other cultural and/or ethnic groups have access to and can receive interventions from their specific cultural services. Social work knowledge and practice is current and appropriate to the needs of the patients/clients and/or their families/whanau. Participation is active, timely and contributes to the overall care of patients/clients. The skills and knowledge of social work contributes to group work. 4 Performance development reviews are carried out annually to maintain, reflect upon and continuously develop clinical practice and professional ability. Quality of service Participate fully as a member of the social work and multidisciplinary teams, undertake continued professional development and contribute to the ongoing development of social work services. Participate and contribute to research or student placements as appropriate in order to advance Waitemata DHB Social Work Professional Development Strategy Documentation accurately demonstrates interventions, progress and discharge Discharges are timely and appropriate. Supervision is undertaken on a regular basis adhering to Waitemata DHB policies Partakes in annual review following the performance development guidelines Attends training to update and learn new skills. Takes responsibility for own learning by recognising training and skill needs and attending in-service training on an ongoing basis. Participates in peer review. Participation in multidisciplinary meetings and case conferences. Member of the multidisciplinary team. Networks with other social workers within the service and externally. Assists with the orientation of staff and with the training and education of social work students as required. Participates in the implementation of service developments. Education to Waitemata DHB and external groups occurs in consultation with Allied Health Team Leader and Social Work Professional Clinical Leader. Criteria for admission to service are followed. Relevant contributions are made and are consistent with the development social work services in Waitemata District Health Board, in accordance with ANZASW and NZSWRB Practice Standards and Code of Conduct..
