1 DESCRIPTION AND PERSON SPECIFICATION Position Title: Reports To: Registered Health Professional Programme Facilitator Clinical Manager Mental Health & Addiction Recovery Service Location: Palmerston North Adherence to the following forms part of this job description: MASH Trust Policies and Procedures MASH Trust Code of Conduct MASH Trust Business Plan These documents can be found at each MASH Trust workplace. Purpose of the Position: The purpose of this position is to provide mental health and addiction recovery services for people across the MidCentral DHB region. The services provided will include but are not limited to assessment, risk management, one one counselling, group programme delivery, and referral to other agencies. Services are provided throughout the MidCentral DHB; this includes the Horowhenua, Tararua, Manawatu and Palmerston North areas. The position delivers inter-sectorial and collaborative relationships with all referrers across the MidCentral DHB region. This will include but is not limited to primary secondary and NGO health services, Corrections and Community Probation Services, and Child & Family Services. There is a strong expectation that external relationships (healthcare agencies, health professionals, educational institutes, family/whanau and advocates of clients) are effectively developed, fostered and maintained. The position encompasses a holistic client centred view of health/wellness and will provide professional clinical practice that ensures each client receives support and care of the highest quality. The support and care will be delivered in accordance with their health professional standards, Code of Conduct, and MASH standards and guidelines. The Clinician will be expected to demonstrate a strong preceptorial ability to foster knowledge, skill and clinical practice development with colleagues and students in implementing supportive interventions.

2 Person Specification Qualifications and Knowledge: You must be a registered Health Professional for example Registered Nurse, Social Worker, Psychologist, Counsellor or an AOD Registered Practitioner with a current Annual Practising Certificate. Current Full Driver Licence Knowledge and practice experience in Mental Health and Addiction settings, procedures, processes, and interventions Ability to deliver group programmes A comprehensive understanding of the needs of people of with coexisting problems. Ability to analyse and measure results Self-confident person who can display empathy, tact, diplomacy and assertiveness when supporting challenging clients Well-developed oral and written communication skills for: the development of therapeutic relationships, for client support plan compilation, documentation, and group presentations. Demonstration of a high level of professionalism and integrity Demonstrated commitment to providing safe, ethical nursing practice of the highest quality within a community setting. 4 The Registered Clinician will uphold the following: 1. Service users of mental health and addiction services have the right and the ability to make informed decisions about their own health, care and treatment; 2. They have the right to the information necessary for this; services must be accessible, available, appropriate and acceptable to the community; 3. All decisions relating to health care policy and services must involve the active participation of health client representatives; 4. Mid Central DHB s Mental Health and Addictions Strategy, the National Mental Health Standards and Health & Disability Sector Standards will be guiding documents used in all planning, provision and delivery of services; 5. Processes for quality will include research and evaluation to ensure practices are safe and appropriate; 6. Standards and ethics are adhered to as set by clinical professional bodies (such as DAPAANZ, ANZASW, NZNO, NZAC, NZPsS) 7. Participation and input into the project is encouraged from service users 8. The concept of recovery and wellness acknowledges the value of traditional and alternative methods of health care and healing. 9. Work to the values and mission that MASH Trust believe in strongly Reporting Expectations: Open and transparent reporting is expected The emphasis being on maximising learning and sharing from experiences. The process will be one of collaboration and inclusiveness wherever possible. Key Duty Areas: 1 Develop and maintain Inter-sectoral and collaborative relationships with all referrers 2 Provide clinical triage of Mental Health & Addiction referrals, assessments, risk management, and intervention planning and programme delivery 3 Deliver care continuum and identified group programmes in all regions 4 Client Records and documentation 5 Clinical Liaison and Networking

5 Duty Area Tasks Performance Measures External Relationships: Develop and maintain Inter sectoral and collaborative relationships with all referrers across the MidCentral DHB region Coordination of information Networking Liaison with DHB Health Professionals, Tertiary Education Institutes and Allied health providers ensuring a effective relationship Liaison with service coordination and other relevant agencies No complaints from external agencies External agencies report positive, meaningful, effective dealings Continuous development of networks with other relevant community trusts, community agencies, local authorities, government departments and health providers throughout the region. Provide clinical triage Mental Health & Addiction assessments, risk management, and interventions Attending meetings with relevant agencies as and when required Liaise with the single point of entry and support triage process, identify best service fit for the person and provide services for people entering MASH care continuum Referrals and triage managed within agreed timeframes Caseload managed within agreed parameters Client Records and documentation Medication management. Nurse only. Able to demonstrate knowledge and awareness of legal and ethical documentation requirements in health Conduct medication orientation for new staff( nurse only ) Conduct three-month medication evaluation for new staff Administer medication as and when required Conduct on going medication and education for staff Ensure medication from pharmacies are checked correctly Monitor script requirements Conduct weekly medication Documentation is accurate and up-to-date Audits are completed on time according to Monitoring Schedule Ensure clinical orientation is fully completed for each new staff member Three month medical evaluations completed for all new staff supporting clients All medication is accounted for Staff receive ongoing training in correct medication administration Sufficient, correct and accurate medication for all clients according to scripts. All scripts are current Incident reviews are completed in accordance with the Incident, Accident and Hazard

6 On-Call ( Nurse only ) Preceptorship Programme count Review all incidents involving Medication errors and client behaviour Participate in the after-hours clinical on-call service to clients as and when required (minimum one in three weeks) Participate in the Preceptorship programme under a buddy system with new staff or student nurses Deliver education for nursing students from Massey & UCOL policy and procedure A quality after-hours oncall service is provided according to client needs Sound, ethical, clinical practice is role-modelled A co-operative approach is demonstrated in the programme Student nurses report stimulating clinical placements Support nursing students on placement to complete required documentation
